Surgeon? I could prolly use one of those. I'm the type of person that hym's and ha's everything to death, uses WebMD's symptom checker 37 times, clicking on different stuff, while pushing on whatever hurts to determine if I think it's going to be immediately fatal if I self diagnose. I'm not concerned about run on sentences, spelling, grammar, word usage, et al.. So, if that's a problem, first, suck it; second, %#@! off. Third, while you're %#@!ing off, grab me a Mtn Dew, I'm too lazy to get it myself. So I took some alka seltzer earlier, that stuff tastes like shit, I don't care who you are. Plop plop fizz puke. WTF. Orange? Orange What? Orange flavored baking soda in seltzer maybe. It's been a REALLY FUCKING BAD DAY.. week.. so far. I was supposed to go get scoped.. yanno... SCOPED. On thursday, as I seem to be leaking. Yanno, human oil, from places you shouldn't leak it from. yeah, not gonna happen. So I was having chest pain earlier. Prolly stress like tension, yanno.. So I sat there for three hours and debated on driving myself to the ER, then another two hours wondering if I should call someone. While sitting in the station, surrounded by other providers. Finally I did an EKG on myself, but not really. Well, the print out says "Not for diagnostic purposes". You hook an AED up to yourself, sit still for two minutes, maybe thirty seconds. Let it get to the point where it tells you to start CPR, but obviously you don't... then sit there... then shut that shit off, and hook it up to your favorite AED download program. We'll call it DT Express. and print out a case summary, hold the papers together, for anyone else that only knows three rhythms.. Flat, normal and "i think we can shock that". So they both look almost the same, sans a random really long "R" and really low "S" every couple minutes. Ok, so I may have sat there for six to ten minutes, while silently giving myself oxygen b/c I was more or less freaking out. So then I pulled a Ben and popped a 10mg Diazepam and the world slowed down.. I bet we can get this page to 100 by 1/1/14.

Sooo.. Monday I started a new diet. Three days in a row, my BGL was low, 38 Thursday, 45 last night and 44 just now. I think I'll take my chances with being 20lbs over weight.

Can't say I would have handled it any differently. I run into this sort of thinking a LOT. Older folks, most of the time it's about the money. Either not wanting hospital bills, or not wanting ambulance bills - think back to my comment previously about ppl who just want a stretcher ride. Not always folks who are abusing EMS, some truely would benefit from pre-hospital intervention. But they just don't want it. Patient can't run away, so unless kidnapping, restraining and transporting against his will is legal - then it's their call. Can't really see where you could play a "danger to himself" card, unless the injury was self inflicted or he said I did this to myself because I want to die. Sometimes the biggest enemy in a medical - or trauma case is the patients mind, people are strong and out going their whole lives, because they have to be, take care of themselves, just to survive. And when it comes to weakness, they still want to fend for themselves. Troubling case I had, involved an older gentleman I'd known all my life, he was sick. Sick-sick. He was almost definetly having a massive MI, vitals signs pointed toward it, signs/symptoms, EKG. We had him speak to a doctor via phone, begged him, one of the EMT's was tearing up. He was well with in his right mind, and didn't want to go. He was in a bad way, but unless he went unconscious, our hands were tied. He didn't want to die in the hospital. His family was begging us, bawling, but we couldn't force him. He got his wish about 15min after we left, he died at home. Family made us work him, but we couldn't do anything at that point. Happens too often, they wait till the very last minute. Couples years ago, I had two in one week; waited till they couldn't hold on anymore. Both arrested shortly after we left the scene- in the ambo. Got one back, but arrested in the ER.

Had something similar w/ an AAA. Patient had eye movement, facial movement and moved his fingers during CPR; then we'd have ROSC/R for a few minutes; and he'd arrest again. Happened on scene, all the way to the ER; in the ER. They applied the MAST in the ER, and patient survived to surgery then died.

A victim of a walk-in "trauma alerting", it is not a pleasant experience, especially when you know about 60% of what they're doing is not necessary. Walk in (dressed nicely and fully alert) expecting about a $2,500 bill, walk back out (dressed in scrubs with a bag of shredded clothing and slightly dazed from "something to help me relax") - four hours later,.... I signed out, AMA of course, because they wanted to keep me overnight for observation. What did I have to show for a $13K bill? A cheap little arm sling and 30 800mg Ibuprofin. I should have stayed home, a crew member already had it nice and snug in a sling/swath anyway. In fact, if I wanted to be a dick, I'd consider it borderline assault or a near Cobra violation. I was told if I didn't allow it, then the ER doc wasn't touching me. Some folks came in, and bitched at the guy that drove me b/c I wasn't boarded. Like, wtf, we came in a car, not the ambulance. There wasn't even an ambulance involved, it was a freak accident, and just happened to be in the company of my buddies - which namely consist of other providers. They put a collar on me and put me on a board; moved me to another room where it was taken off three minutes after they cut all my clothes off.. Then when I questioned it, they considered that I may have a head injury, and I could possibly be combative.. so they dosed me with Diazepam.. in one of the two IV's they started. I had an abrasion to the scalp, and I only went b/c I thought my arm was broken, from being struck by an object. There was no blood loss, my vitals were fine, I had no loss of consciousness, no swelling around the abrasion, it was just a small rub mark, that didn't even bleed. Head to toe, I had arm pain. My chest was fine, my abdomen was fine; and I certainly could have pissed in a cup, and removed my clothing prior to the onslaught. I haven't been able to find another pair of ambulance boxers since then, so I sewed them back together. Call them my trauma shorts

Not to get off topic, but I'm well experienced in difficult extrications. Not well schooled, it's hard to be well schooled when you're limited to resources. The auto industry changed before our eyes, and the fire service (rescue) was not prepared, or equipped for the change. You can Google this, Boron Steel and Vehicle Extrication. The UK's fire service has been twenty years advanced, since the 1970's. In the US, before tools like the Jaws, you had saws that made sparks, and hand tools; and more often than not, if there was entrapment, a wrecker service would be doing the extrication. HURST Jaws were the first hydraulic cutter and spreader systems, and all models stemmed from them, breaking away with better ideas. They work really well, and so do the competitor models, in fact of all emergency tools, these will almost be gaurenteed to outlast your trucks, etc, by 20-40 years. They're built solid and really hold up. Until now. We had a wreck prolly two years ago; new car, ultra modern technology, electric, made of boron steel. Made to survive a crash, just not a violent one where the patient is severely injured and cannot be moved too much. Didn't remove the roof, but the B post had to go to get the driver out. Could not cut it. Brand new rescue tools, cutters clamped down, and stopped. Broke out the SawZall (Saws All).. Yeah, not anymore. Took four diamond bit blades to cut it. Now firefighters in the UK have been using a common tool in construction, it's a spark-less saw, the name escapes me, it cuts clean, and it cuts through Boron steel like a hot knife in butter. In fact, you cut an entire car in half in about 15min with the saw. There are blades for this steel for a common Reciprocating saw, and that's what everyone upgraded to. But the first time I went looking for how the hell to cut this stuff, I found a video of UK firefighters they cut a car, literally in half. It was the most amazing saw.. And that's what it looks like they used. BoundTree UK sells Kendrick KODE II (the improved KED). The Dual Saw.

Even in wrecks where the patient has been dead for a while and is impaled by, wrapped within and general squished by metal within the car; it doesn't take us 2 hrs to remove them. Perhaps they didn't have the right equipment, but still they were already walking. It's not like they sat in the car and became paralyzed. There isn't any fixing to that.. It's done been totaled. There is just no reason under the sun to do that, none whatsoever. Nothing that anyone could say would make it justifiable. I'm just flabbergasted by the degree of ignorance, it has got to be the most asinine story I've ever seen in my entire life. There is not one person on that crew that is innocent. They all look like complete douche bags. In fact, they should all be required to fully retrain for vehicle rescue. Every time I see that picture, I become so disgusted that I get a headache. It seriously just blows my mind.

Yeah, I don't go to see my doctor, he's an idiot. If I can't treat it or diagnose it from home, it ain't happening.
