  1. What the hell do I need 900mg for? Call for the order, 150mg in a 100cc, PVC-free bag of D5W with a PVC-free IV line, set up the pump, and run it over 10 minutes (yes, that is my local protocol).
  2. I would think that being able to rule out hypoglycemia in a suspected CVA patient would be helpful pre-hospital, particularly if ALS is unavailable or too far out (by way of example, I know not germane to this scenario).
  3. Where did you get the idea that I approved of teenagers on an ambulance? I do NOT approve of holding a teenager to the same expectations as someone who's done the job full-time for 20 years or whatever. And as you said, sometimes even then you hit a breaking point.

  5. My first thought was "KED him upside down and slide to the board." Maybe, maybe not. Sounds like you did a helluva job though. This is definitely one situation that crossing the straps over the shoulders would be a MUST for!
  6. I'd highly recommend checking out QCC in Worcester. I've heard nothing but good things about the program, and from what I hear their graduates are the only ones that Worcester EMS allows to do ride time with them. If I still lived there it'd definitely be my choice. I took PHTLS with ALSI. Learned a lot. Don't know if I'd be able to put up with the office manager/instructor for a year though.
  7. FT job: once dispatched, we are required to make the scene. If we can't find the patient, so be it. But we do not cancel enroute. PT job: haven't run into it yet. There's this concept called NIMS. It's only a few years old.
  8. I think expecting a teenager to NOT be affected by an effing pediatric trauma arrest is a little cold. You must have a fantastic bedside manner. :roll: Like some of the others here, I implore you to seek professional help in dealing with this issue. There is no shame in it.
  9. Well, the report said "serious injuries," so we can't discount the possibility that rapid extrication was indicated. It also looks like they had zero help whatsoever. Sometimes the book doesn't cover everything.

  12. Imagine- not every place in the world is just like yours, for a variety of reasons. Amazing. If you haven't noticed American/Canadian/European EMS differences yet, well, you just haven't been paying attention.
  13. Don't immobilize someone "to cover my ass." Immobilize them because its the appropriate treatment for their presentation. If you go boarding people just because they fell then you're no better than the guy I know who tried to give O2/ASA/NTG/Albuterol/Lasix/Morphine to a woman with pedal edema. And don't turn their clothes into handkerchiefs just because it's "OMGTRAUMATHEBOOKSAYSCUTEVERYTHING." If you weren't taught how to assess for pelvic injuries without trauma shears, you need to go retake the class somewhere else.
  14. NREMT does not "govern" anyone. Not even the ones that have NREMT certification. You can't even call them the national certification body for EMS, because quite a few states and thousands of providers don't have NR.
  15. There's another company in California called Doctor's Ambulance. Are you proposing that not having MDs on the ambulances constitutes false advertising? It's a name. Get over it.

After looking through the state EMS websites for the Northeast, here's what I got: http://www.maine.gov/dps/ems/boards/invest...ee/minutes.html (No names, just the sanctions- based on the law violated sometimes you can get an idea of what it was.) http://www.health.ri.gov/hsr/professions/amb.php (Name and sanction. Only one person's offense identified.) http://www.health.state.ny.us/nysdoh/ems/sanctions.htm (Name and sanction.) In my experience, those unfortunate souls on the "wall of shame" are there for non-EMS criminal violations that are brought to the DOH's attention. I don't know how other states deal with that. Is it possible that they are EMTs who keep working after their cert expires, and the state law defines that as impersonation?
  19. As with everything, absence of evidence is not evidence of absence. I sort of put belief in God/gods and belief in extra-terrestrial intelligence in the same category- I don't know for sure, but it seems awfully arrogant, give the size of the Universe (even just the parts we know about), to believe that humans are the most advanced or highest form of life in existence.

  21. It's a bright sunny day at my old private ambulance job, and my partner and I are on our way to a scheduled discharge. Lo and behold, a relatively minor MVA appears in front of us on the divided sort-of highway, lefthand lane. "Hey, beats doing a taxi ride." Hit the lights, block the lane, notify dispatch. Some dude in a pickup with an orange lightbar says Staties are on the way. After confirming that one person walking around has neck pain, I call them back myself and add Fire to the response. There's also a mother an 2 kids in the car that got rear-ended, so we start assessing them after throwing a collar on the first patient and telling him to stay put. Mom has neck and back pain, so I start walking back to the truck for another collar- thankfully the kiddies have no complaints because we've only got one pedi collar. All of a sudden this mid-90's Nissan roars up in between the back end of my truck and the Jersey barrier- good thing I hadn't gotten there yet! This kid in his 20's flies out holding a stethoscope with this look his eyes that just screams "OMGTHISISITTHEDAYI'VEWAITEDMYWHOLELIFEFOR!!!!" It was so ridiculous I actually stopped dead in my tracks (but also because I wasn't convinced he actually took the time to put the heap into Park). "Do you guys need a hand?" I turn back to the scene and review the situation. Partner holding c-spine on mom, waiting for me, and trying to make sure the kids don't freak (both of them car seat/booster seat age). Neck pain dude, collared, leaning up against the barrier talking on a cellphone. Can't hear Fire yet, but the Statie did pass us on the southbound side heading for the next available turnaround. "No thanks, we got it." You'd think I had just stabbed his puppy. "Be careful when you back out please, we don't need more problems than we have already."
