  1. Well I see what is keeping EMS down more than anything has shown itself in this discussion. The it has always been this way so I will continue to do it this way attitude. It just makes me sick. Does no one want to see our job actually advance to where it is a true medical profession? Are we content with the same old crap even if it does no good for us?
  2. Doc can you provide some references I could submit to my medical directors. The services I work for are rural and have long transport times. We have progressive protocols except when it comes to back boarding. We have to backboard any accident, even if somebody just rolled off a low bed and has no pain. I have seen many patients no pain on scene have severe pain by time we get to hospital. After removal from board pain quickly diminished. I feel we are doing more harm than good but I need science behind it to have any hope of making a change.
  3. No one besides another ambulance wants your boards etc and if you have your name on them they are easy to spot. But every nurse would snatch that BP cuff in a minute. Even with your name on it it would disappear.

  7. Should work. Are you going to take it out before taking patient into the hospital? Service might get mad at you if every time you backboard they have to order new BP cuff. I always try and slide a towel in the small of the back, behind the knees, and anyplace else I see daylight. Not the best but it at least gives some support on the long ride to the hospital at what is now my part time job.
  8. First off it's not a bus it's a taxi. Why because it is designed for most of the time having only one fare. Alright on the serious note. I agree we need to start calling them ambulances. It is obvious the author is fire as he is fine calling them "rescue". Rescue is not an ambulance as very seldom does an ambulance crew actually rescue anyone, we leave that to the hero FF. We are not "public safety", that is the FF's job. We are and should be emergency medical professionals. Really even ambulance is not the best term to convey what we are and do. Especially in areas that have more aggressive protocols, including the treat and release, should rename the ambulance Mobile Emergency Medical Center.
  9. Great question. Where I work it's all or nothing. Do not know if that is based on any facts though. A question I have is for someone that had a witnessed trauma to lower back would leaving there neck free to move cause additional injury to the lower spine? Say patient is sitting eating lunch on construction site. Board gets loose in the wind and hits patient on back. Patient does not fall but is lowered gently to ground by workmates. Nothing to indicate c-spine injury. In fact lets say you have a call God card and he confirms no c-spine injury. So would leaving c-collar off potentially lead to more injury of lower spine? Any research to support your statements?
  10. They actually sell a pad that sticks on. I have also seen an air pad that you inflate after placing patient so it supports all areas.

  12. Actually there have been many studys. From what I remember many seem to point that backboards and c-collars are used way to often and may be even causing harm. Many paramedic services now have protocols that allow for deciding against use of back boards and c-collars. It seems that some of the research was posted here so a search may provide you the answer.
  13. Welcome to rural America. My community of 7000 has 3 police officers. My county of almost 6000 square miles has 4 sherifs deputies. There are a lot of hours that no one is patrolling. There is someone on call but that does not help when you get called for pneumonia an hour from town and you find someone with as we call it "traumatic" pneumonia, in other words they had the hell beat out of them. You are all alone in a dangerous situation. You always have to be prepared to defend.

  15. Until we develope our own identity as emergency medical healthcare providers and pay a professional wage there will continue to be constant turn over. Shortage is probably the wrong term to use. For example in my county there are about 50 people with some sort of EMS certification, yet only 5 work in any form of EMS. So there are people with the paper but many have decided it's not worth being the beaten step child for low pay.
  16. I like the old Secret Service style. Ear Piece and talk into the cuff link mic. But back to the speaker mic. Yes a uniform should have a place for them for the rare cases you need to use it. Looks bad just having it on the collar.

  19. There are some big services that offer tuition reimbursement. Austin Texas is one that comes to mind. You have to work for them a while before eligible.

  21. The reason to get a degree is the future. We all understand that most services do not care where you got your card as long as it makes you a warm body to fill a spot on the ambulance. With a degree you can look at a change of career much easier. Plus if you decide to go further with your education it will transfer where the certification will not transfer to any reputable school.
  22. Days of rolling CPR are over. If they do not revive on scene the are called on scene. Why put yourself and everyone else at risk for a dead person?
