I am glad he's not dealing drugs at that instance. But he is damaging public property and will probably be paid a hefty amount of tax dollars when he sues. I am all for being active but do it at places set aside for it. And please nobody say if they have signs up saying no skateboarding they wont have to pay because they don't work. In all honesty we should not have to put up signs or pay for stupidity.

OUCH!!!!!!!!!!!!!!!!!!! Reposition. Monitor pulses. 14g IV only because no 12's on the truck. Immobilize. Depends on how polite as to whether I pull out the Morphine. As long as people like that breed I have a paycheck.

JEMS should be a hit with the basics...

Guns and grenades is the best. OOPs wrong forum. -
Now lets not hurt peoples feelings by getting personal. We like to discuss and disagree but in defense of NREMT-Basic, I am sure they act more professional than the militia pics.

And we'ld not want it any other way. Reminds me of how I used to walk to school and back bare foot in the snow uphill both ways. Oh the good old days. But in reality it pisses me off that somebody got paid to set up the 911 system here and I really believe they never set foot in my county. Dispatch does try, they'll say comp says x address. But patient says somewhere else. So while I pick on dispatch they are not completly to blame, but just mostly to blame.

Dispatch to EMS 1 you are being requested to the XYZ residence for low ETOH syndrome. Directions are to turn left at the monster truck with flat rear drivers side tire. Pass the trailer with Piss on Gordan logo. Take a right after the Bud 8 trailer. And the patient will be in first trailer on the left with the big UPS logo. If you get to the Intimidator trailer you went to far.

Number 1 I have seen to many falsified Medical Necessity forms to give them any value. Number 2 I am not saying all transfers do not need an ambulance. Just from my experience and from talking talking to those that own businesses that do transfers, most of these patients do not need an ambulance. The little old lady that wants to get to an appointment in order to get meds refilled does not need an ambulance. The guy needing emergency bypass surgery but he's at a bandaid hospital and needs to get to a cardiac hospital ASAP needs an ambulance. Services and doctors that write up transfers that are not emergencys as if they were are guilty of fraud.

Welcome to fraud 101. Sadly many Doctors will regardless of where they are make false statements on the forms in order to make patient happy. Just document your findings even though they contridict the doctors. Rural EMS is much more fun and challenging. That is if you are really rural. You have to use your education, you see the results of your actions, where in the city even if you treat most of the time your already at the ER prior to seeing the result.

In my part of the world dispatch better get directions. Even most of the calls that come in showing an address are wrong addresses. Don't know who the idiot that set our system up was but they ripped the tax payers a new one. You even ask the callers where they are and they say X's house. Don't know it, Wellit's next to B's house. What street are you on I don't know. We do a lot of turn at the dead tree with the barking dog next to it. Why you making fun of me for failing geography. Just kidding favorite subject next to shop class  .

In that post I mention that 3 services are going paid. One of the services only did 17 calls last year. Town is less than 200 people. You are paid for every hour on duty. Pay B=$13hr, I=$15hr, P=$18hr originally was supposed to be higher but we worked it out. I go there and spend 48 hours I'm paid 48 hours. So how did this happen. All the small towns and the county got together. We now will be a county wide service. This county a year ago had one paid staffed ambulance, with an on call crew for second ambulance. Currently 3 staffed 24 hour ALS ambulances. On Jan 1 there will be 5 staffed 24hrs ALS ambulances county wide. The 3 towns that were volly each have an ambulance or ambulances and station, but often nobody responds. All equipment is there just needed money to fund salarys. The towns and county decided that many of the fun things they paid for are not needed as much as an ambulance services. I actually think somebody with money dialed 911 and no ambulance came because no vollys in town, but have not confirmed this. Now you say what a waste, well guess what all ambulances will float, in other words if town A has to go out of town to get to the hospital Town B ambulance will drive out part way so they can make reasonable response to both locations. Also now instead of taking all 6 trauma patients in my 1 ambulance by the time we get them triaged another of our ambulances should be arriving. This lightly populated county has gone not only paid but will be ALS at all locations and the towns and county budgets have not really increased.

Exactly it robs innocent tax payers of money that could be used so much better. It would be much cheaper to give medicare granny a taxi voucher than what the cheapest "ambulance"( I use the term extremely loosely ) can provide a ride for. This is sadly mostly legal fraud caused by the way medicaid, medicare, and even many insurances word policies. If this abuse was stopped they could afford to pay us at least a decent amount of what a 911 transport costs.

If you keep making the best observations you just might convince me that not all dispatchers have their heads up their butt. These groups are just that. They give a very false sense of security. They waste resources that could be better used locally on a day to day basis. "Ill give ya thte equipment drugs and funding" Thanks basic. If the drugs were distributed at the various stations based on number of people in area they would be given quicker than your group could organize and travel to my area. Been through education on administering these emergency magical drugs that you want to hoard in one location. What happens when the terrorist blow up your supply house, or a tornado blows it away. It is bad strategy to ever place all your resources in one location. Thanks for the funds. Now I can staff more ambulances. Now when a disaster happens I have people that know the area here to start the process. And shortly those other services that received funds trucks and people start rolling in. So we stage them in safe places. Also if you have ever read my posts you could see that I am with a paid department. But sadly as much as I disagree with volly services I think they would put the funds to better use than groups such as yours. As you are new to your service I commend your falsely placed enthusiasm.

Again this one instance that still may be blown out of proportion is no basis for hauling all people in ambulances. Are you going to start coming and driving me to work because I might have a heart attack on the way? There is no justification, it is a waste of taxpayers money.
-
DING DING DING! WRONG. Common sense says bad idea to have so many people doing the same thing. Waste of money and resources. Do you know how many government auctions I have gone to and bought basically brand new equipment that has sit just in case in some warehouse? To many to count. Lets not add to government waste, if money is available give to to areas with limited resources to use daily and still be available for disaster response. Oops my bad no common sense allowed.

Help me convert Ruraltown EMS from volly to paid.
spenac replied to spenac's topic in General EMS Discussion
Well we have a new batch of vollys claiming we want to let people die because there is just no way to support a paid service, thus I bring this post back to life tonight. -
Wow we got you beat. My primary service area is over 3000 square miles with mutual aid on another 3000+. Population last census in our primary about 6000 people, Almost all live at or below poverty level. We are paid. Basics about $30000 yr, Paramedics about $37000 yr. There is no excuse for a volly service any longer. EMS is not a self supporting function. Most services lose money. Your county needs to fund you. As you already have everything only new expense to county would be salary. Look for the my topic Help convert ruraltown ems to paid.
