  1. Depends on the person. Some will 3rd ride for more than 3 months. Myself I third rode 3 shifts and only did the 3rd shift because already scheduled as I was cleared before end of second shift. But I came from a service with much more aggressive protocols,so I only had to dumb down rather than increase my knowledge. The majority do at least a month of 3rd rides.

  8. Yes chemistry is important. Without it a relationship is doomed. Actually it makes sense that at least basic chemistry would be beneficial as we are introducing chemicals into patients bodys.

  10. Starting the lytics does not delay transport yet saves minutes of cardiac muscle because even the treatment at a stemi lab would be several minutes getting started. But I may be wrong as as you say I'm a long way from all real world services. Also I was told if you can to get your 12/15/18 lead prior to oxygen it best. But not to deprive someone struggling to breath. Is that true or proven to your knowledge?

  13. I have had one partner fired for texting and driving, yes I walked in the office and said this person is fired and the supervisor did the paperwork. Patient on board you better not touch anything that is not required to get me and my patient to the hospital. If you are driving at all you better not be texting. Texting and driving is an automatic firing offense in my opinion.
  14. Was that meant as a challenge? OK here is what I have. I make more as a single role 911 Paramedic with better benefits than any fire fighter single or dual role makes within 500 miles of me. I am involved in the education process and we are able to be focused on what we do. As a result our medical care provided is constantly expanding based on evidence rather than tradition. We stay at a station rather than staging as that system has been shown to be of limited value. If we need help guess what instead of a fire truck showing up we have another ambulance with more Pre-Hospital Medical Professionals. The fire services I see tend to have limited access to actual medicine and just rely it seems on the diesel bolus therapy. Which is better? Based on what I have seen non fire based EMS as it allows you to actually focus on practicing Pre-Hospital Medicine rather than just being a taxi driver.
  15. Some services now after interpretation of the 12 lead even start the treatment in the field saving many many minutes. Think about it. Even if you go straight to the cath lab you have 10 minutes to the hospital. Get there the cath lab will run additional 12/15/18 leads, draw labs, etc, then start pushing the drugs. Thats 25-30 minutes of dead heart cells. So by interpreting the 12/15/18 lead and treating your patient beyond just MONA you may have given the patient a change of returning to basically a normal lifestyle, and may have even kept them alive. Sadly though many services refuse to accept that responsibility and will not carry the drugs and instead just choose the diesel bolus therapy that has proven deadly in more than one way.
  16. Thank you all for the good insight based on your experiences. My experience has been you do Paramedic as a fire fighter then try and get promoted to the recliner umm I mean fire truck and never see an ambulance again. But I still do not know why if the IAFF thinks they can be dual role Fire EMS, why they say you can not multitask when it wrote the paper opposing requiring fire to be law enforcement. That it will harm the public. Seems very hypocritical.
  17. Are the Paramedics actually Police officers and pull police duty rotations as well? A service near me required you to do both. And if you were on patrol and another ambulance was needed you were sent to get the other ambulance. So your whole mindset would have to change quickly or so it would seem to me. Yes spenac is turning over a new leaf and is trying to understand other ways of doing things rather than fighting.
  18. Very good point. I was one of the most sought after contractors for years before deciding to pursue EMS. Now I am recruited by many services to be a Paramedic for them. But I am not doing both at the same time. I am focused on doing only EMS at this time. Perhaps I will start contracting again and do EMS as well. But w/o doing it I already see how each job is distracting from the other which could mean my customers in both fields might not get my best. I have met a few people that can master multiple different fields but not many. I have met many that are good but not great at the different fields they have chosen. I want to be a master of my chosen field not just good at it. And if my family is sick I want them seen by a master of the profession not just someone that is good. But Lone they are wearing out these large trucks responding to EMS calls not fires. Stop sending fire trucks to do EMS calls and use the EMS money to staff more ambulances. I mean no disrespect to those that make every effort to be masters and are good at what they do. I was a firefighter before I joined EMS. But fire was separate from EMS though stationed same house. I guess I just live in a dream world where everyone is allowed to chose their focus. Thank you to those that have chosen to try and let this be a civil discussion rather than a civil war.
  19. Actually we do have some areas of the USA that do have police based EMS. They are dual role just like fire based. And admin thanks for trying to keep it on track.
  20. In response to Ruff and for us to air our opinions here is my reason that I for the most part oppose fire based Paramedics. In the illustration Ruff gave the fire Paramedics were strictly working on the ambulance, fine that allows them to be focused health care providers. But when forced to be multitaskers it is hard to believe they can be the best when divided, ie jack of all trades master of none. Also the Paramedic that is forced to fight fire may just do the minimum because their heart is not in fire and the fire fighter that is forced to be a Paramedic may as well. I also get frustrated when I see so many fire services just raping EMS by taking the money brought in by EMS and instead of making more improvements on EMS they spend it on more fire toys. Another gripe has been seeing the fire services and the fire union actually stand in the way of advancement in EMS. OK that is just a start of this topic look forward to a peaceful discussion.

  22. I was able to attend a Bob Page 12 lead class a few months back and learned a lot. He had a neat idea, they approached the hospital and asked if they could carry hospital gowns. They now have the patient put that on before IV is placed. This also helps them be able to make sure lung and heart sounds are done on skin. Hospital is happy as they don't have to thread the patients clothes over the IV bag risking snagging and pulling it out. Makes it easier to do your 15 lead as well. Well just some thoughts.
  23. Yup that is a good change as one person that called me out on the forums started finding every post and hitting negative. In one hour I was at negative 13 or so. So I am happy admin made that change. Now it will take 13 days for me to get to negative 13 unless everyone hates me.
