I think she knows we have all gotten that point by now or we are to stupid to ever learn one of the two. Thanks to Vent I think many here have been reminded how little airway education we really get in EMS. I know me and her have had some disagreements but for any new people that read this contact her for your airway questions.

Shock as it is the area we in EMS can have the greatest effect on patient outcome. Break down the many types of shock, what s/s you expect with each, how they progress if untreated down to various organs and cells, and then describe methods of treatments for each type. Not only will you be better from doing the research your patients will benefit as well.

Vent I am aware of that statute. My point is if the employer does chose to limit the right to defend oneself as allowed by law then if a medic is shot it could be argued that they might have had a better outcome had they been able to defend themselves. The lawsuit you mention was one before the fact, not after someone was shot, if I read correctly.

Good advice. Even though I am aware of Texas law my answers would not provide defense in court. You need to get the written policy's and laws so if something happens either way you are protected. If the policy says no and you are harmed because of it you can sue and show written proof of the denial of right to defend. If you are carrying in a place that you are legally allowed and ticketed or arrested you can show the law in court and have it thrown out and if need be sue for lost pay etc. Yes I am in a law suit mood today.

There are no laws against a gun in the ambulance in Texas. Some services will set policy against which is a bad idea as they open themselves up to lawsuit if a medic is shot and was denied right to properly defend themselves. But also keep in mind that hospitals in Texas are no carry zones so if you are carrying a concealed firearm you will need to secure it in a lock box in the ambulance prior to entering the hospital.

Wow I was going to argue that we do make field diagnosis, regardless of what Rid thinks, but now instead I want to know what kind of "common scents" that we do not get with a degree? W/o knowledge one only has experience based on ignorance.

Also beware many services that claim high hourly wages also require free standby often equal to the number of hours worked so you get paid 24 hours then you have to be available 24 more free unless called back so good chance you can cut the hourly rate in half. Now in Texas huge range from in Paramedic pay some work free even can you believe that? But pay actually can be less than minimum range when you factor in the free standby some have to well over $25 an hour. Now these are strictly EMS some fire may pay more but sounds like you want to be a pre hospital medical professional. If you factored in benefits (free insurance for me my family for less than a $100 more including dental and vision, great retirement, etc ) at my job we are probably over $30 an hour and cost of living is cheap. A trauma code is a dead patient. On scene you can decompress chest, push fluids, intubate, probably no need to shock because odds are will not be shockable rhythm, IV/IO, push your drugs, etc. Same thing the hospital is going to do. They are not taking the patient to surgery unless they get ROSC. Even AHA now says no to rolling codes. If patient gets ROSC then get them to the hospital.

Codes that are worked should be done on scene or in the back of a parked ambulance. No transport should occur unless you get ROSC or if organ donor and then no lights and sirens as they are dead no need to risk lives. If no ROSC then call it in the field.

Why are we placing IV's on plants? Think you have wrong forum we only place IV's in viens. And your statement is more proof that basics should not be doing advanced skills such as IV's and EKG. Get the education so you understand why, how, etc. A skill just to do a skill is a recipe to harm patients.

Have you ever seen the look of terror when a doctor pushes sucs w/o versed or other? I have. They are aware of what is happening but unable to do anything.

I say yes and that is why you must administer a drug to sedate and hopefully block the memory prior to using sucs or other.

I think it would be better to have rotating calls and sleep when no calls. Amb1 responds Amb 2 is next up. Soon as Amb 2 responds Amb 1 is next up. Otherwise one crew gets all the runs as it seems calls come in cycles. Plus the crews in your method would be trying to sleep while the other crew was up and would be hard to get rest. From a dead sleep you can still can be rolling in 90 seconds.

I wish EMS in the USA actually had some education standards like Canada has.
