Well the strip interpetation is wrong as there are definite P-waves, though multi-focal, which eliminates A-Fib. Rate is irregular. Almost looks like an irritable junctional foci is firing as p wave is inverted, but its not the more I look, then you are getting a pause as heart resets then you get back to atrial rhythm. QRS is normal. Axis is normal. Almost leaning towrds a Type 2 Wenkebach as lead II appears to have a widening pri then drops a beat. There is that confusing.

We are required to tell the patient that we are a student and part of our education is performing skills such as IV starts and blood draws(or what ever skill) under the supervision of someone experienced so that when we are alone we will be able to appropriatly take care of an emergent patient. If it is OK with you I will now start this IV(or other). Suprisingly I have not had anyone refuse to allow me to perform any skill. I have had some people with chronic health problems even give me valuable advise. So never lie to your patient, and listen to them. Many of them will say no one can start an IV except this spot and point. It might not be the hand might be upper arm, who knows but they often know. -

We did dummy arms then idiot (student) arms before getting to go do clinicals. We had to do at least 5 sucessful on fellow students prior to doing clinicals. It was stick for a stick. I feel sorry for the patients that your school is letting the patients be your first live sticks. -
The effects of the herb may be reduced by propantheline. And yes always ask for the herbs, etc. In my area we have the for a better English word Witch Doctors. They do some weird stuff. We also have to get all meds they buy in Mexico as sometimes they only hand us their meds from the USA.

Based on stability he will mainly need observation until wears off. As a precaution I want to establish 2 IV's, Keep a close eye on quality of respirations, get my BVM and intubation equipment ready in case. Cardiac monitoring. BP q 3-5 minutes. Nice quit transport.

Thanks Wendy. Here in west Texas we had something happen like this as well about a year ago. You would think th epeople selling would make sure what they cut with didn't hurt the customers so they would get repeat business. -
Let him rest he's just ignoring his nagging wife. Seriously now. Any response to voice? Painful stimuli? EKG, 12 lead preferred? Pupils? Does she take any medication? Is it possible he actually took hers? Did he recently get a refil on his meds? If so are the pills in the bottles the right ones?

The only tangent was yours you want a be a real medic. You asked questions. I answered. I mentioned what a system like yours is called based even on your description. No tangent. Sorry you fail. This whole topic was a complete waste of everyones time but interesting to see some of the ways people think. Also easy to see many can not think out of their limited box based. As soon as anything out of their comfort zone they lost it. -
Nothing could touch the Ford 7.3 Power Stroke. Then they went to the 6.0 powerstroke and the ambulances got more miles on the lift than the highway. Ford used to be the only choice now its become the last choice.

The great country of Texas. I do not respond in any thing other than an ambulance. If we get to places which an ambulance can not go then we load key eqipment into the ranchers pickup and continue. Our ambulance is a little out dated we only have LifePak 12 12 lead with capno, NIBP, etc. One day maybe we can get a fancy big city new bell and whistle LifePak 15, or a phillips or zoll. I diagnos and I treat with one of my more than 50 drugs. Sometimes I treat then tell my patient to follow up with their local doctor in the morning. Sometimes I do an exam and then tell them they do not require my treatment or an ambulance. Thats right I practice medicine. Yes my medical director trusts me and allows me after educating me many things you will never dream of doing. My protocols are more like guidelines and I can treat according to patient needs, not some exact step 1. step 2. algorythym. Based on your posts you are scared to practice medicine and support the typical horizontal taxi clan. It really dissapoints me to see your last several posts because based on your discription of work area you guys should be doing more than driving. If I have misundrstood your statements I am sorry. What the hell is a stomp2? -

Thats worse than cookbook medicine being in a mother may I system. Glad I am in neither. I have guidelines and can do what needs done. Radios and cells don't work in the wild frontier where only real medical professionals can survive. -

We have very long transports. Plus just as easy to lock the supply of morphine in the ambulance as it is to lock up at the station. -

Illegal Drugs - What affect on the EKG, the 12 lead?

BP was around 150/100. Patients rate dropped to about 115 with vagal. Pain was 9/10. By hospital about 3/10. Never found out drug screen. They did ship him to a large hospital with a cath lab though. The part that threw me was that they were saying was not MI but just a reaction to the illegal drugs. I mean the way I see it if it looks like a duck, quacks like a duck, its a duck, but asically they said it was a chicken. Hope that illustrates how I was feeling about that one. Patients PCR already sent in so could not get 12 lead for you guys. Seems V2-V5 were elevated, II,III, aVF, depressed, but I may be confused as I've been wearing out the 12 lead. Thanks for all the help. As a student as well as being allowed to operate at the Paramedic level I sometimes still get a little lost. -
Well on thread about the worthless EMT we learned that in the UK you can't work during break. I think here we learn that in the UK all Doctors are allowed to beat up one Paramedic per month so frustrations for lack of manhood to not cause them undue stress. The sad part was learning that the Paramedics can only appear to resist again to make doctors think they are actually capable of doing something for themselves.

Thank you for a post that does nothing to contribute to what has been an interesting discussion of peoples views of what is a very current issue in the USA. What country are you in? In the USA we must file a seperate report of suspected child abuse from what the doctor does.

I hope never to aspire to be so lazy as to only know the minimum. Sorry I disagree and darn it I'm just a student and I already do much more than that. Plus I already have more in my Cardiac protocols as an intermediate than MONA.

Note the titrate to relief. I would rather not compromise my patients respiratory system. So start with 5mg healthy and honestly even most severe pain paeople are fine after that dose, and it has caused respiratory depression, so was actually to much on some. If not out of pain in less than 5 minutes later I push more. Yes I understand some people could need a ton to get effect. I also understand that I may need to bust out one of my other pain killers. But again you have helped prove my point 300mg is not an extreme amount. My only problem is his lack of education. -

My cousin from way south I think we could work well together. I get tired of people saying lets get them loaded and roll. No I am going to take care of them where they lay so 1 they might get some pain relief. 2 As you mention they are prepped in case they crap out. And I to agree definitive care is the ultimate answer but it does surprise me how often my patients have significantly improved because I went ahead and treated rather than just load and go and let doc figure it out.
