I disagree with Vent and Crotch. The minute I was told we would have to wait they are no longer emergent and we would have left and pressed charges if they dialed 911 again for this. This patient was stable enough to wait she was stable enough to travel in the family car. Even If I did transport she would go to the nearest hospital regardless of her preference. I am doing my job. If patient calls 911 at that point it is my patient and home health is booted. If patient and home health refuse then they do not need me. Just because patient history does not mean those items qualify her for 911 at that moment. I am not a taxi and refuse to allow people to treat me as such. I am as professional as any medical professional you have met and I do my job. Being a taxi is not my job.

Crotch I have had a woman start crowning we stopped ambulance and baby disappeared we waited a few minutes then drove the final 20 miles to hospital and I delivered in the OB hallway. There is no dependable way to know. I think it was my first delivery patient was 20 minutes apart on scene. We delivered 30 minutes into transport. I have had others under 2 minutes make a two hour transport. Again just can't tell. Do to distance we load and drive till crowning. Yes I have delivered in homes and in hotel rooms as well as at the hospital. Even checking for dilation does not tell how long till they deliver. Some go from 1 or 2 to 10 in just minutes. Others will linger at 7 or 8 for hours. Oh and water break is not any indication of imminent delivery as many have had water break before calling for ambulance and we take them to hospital. $ hours later when back with the next patient they still have not delivered. So my advice is drive till baby shows. If cord is wrapped try and remove if not able to clamp and cut.

I hear you. I hate the warm body with a patch policy of so many services. Hire people that can do the job.

Sadly we don't get a say. The service says t's t's it is. I made a discussion along the lines of design a distinct professional uniform for EMS and like everything EMS there was no agreement on anything.

Some of your topics are worth discussing but the way you go about it is leading to no benefit for anyone except admin as there are lots of posts, valueless posts but posts non the less.

I think they knew as with many things that something worked but with limited resources they could not prove anything beyond people got better. Sadly the experiments sometimes cost lives as someone would say hey wonder if this works? You eat it and lets see. Now with improvements we eliminate many risks by lab testing prior to human tests, but eventually it all comes back to hey eat this.

Yup he took credit for treatment that had been utilized for centurys. I mean he refined it but really only scientifically proved that it worked.

I've delivered in the ambulance at least 6, plus a bunch more at the hospital while waiting for doctor to arrive. In the ambulance at least 2 of those had cord wrapped around neck. Thankfully was able to pull it over head and off neck so did not have to cut. So based on my stats there is a 33.3333% chance you will deliver a baby with cord around neck. I agree 1 OB is not even good for 1 normal delivery. Every ambulance should have at least 2 kits and lots of towels. I hope none of you are cursed to deliver in an ambulance. It is a slippery slimy messy process. You actually do nothing but clean up on a normal delivery. If you want to deliver do a shift in the OB department where you have room and people to help control the mess.

Your right. Sorry did not intend disrespect to those that do transfer patients. All Mobil Medical Professionals should have pain management available. Patients should not have to suffer because of those that are not qualified to do anything. Get rid of those that refuse to stay up with current medical standards, rather than lowering protocols.

Mix up flour water, etc like you are going to make a loaf of bread. Place in cool dark place allow to starting growing mold. Scrape off mold and place over infected cut, bite, etc. Actually is like PCN. In fact people with PCN allergy that accidentally eat moldy bread, tortillas, etc will have allergic reaction.

If it will not slip over babys head baby will die a painful strangulation if cord is not cut. Sorry in my humble opinion your medical director failed. Perhaps he misunderstood that this is a last option and not the first option.
