    you wold have JVD because of increased interthoracic pressure... tracheal Deviation is a late sign, Resp distress for sure... wheezing comes from constrictied bronchi... not typical with a tension pneumo. Are you a basic or paramedic? Basic- place a blanket or have him hug one which will somewhat relieve the pain. medic- chest decompreesion.
