Has anyone ever dealt with Appendicitis?
aussiephil replied to xstreetsweeperx's topic in Education and Training
I agree, you have used the tools provided to you to assist but can you confirm any of these? No. We treat what we see. If they have pain, we give analgesia. It is not up to us to determine a final diagnosis. On a side note it is well width considering that there are a number of ambulance services using telemetry to broadcast 12 lead ECG's to hospital, providing antithrombolytics & going straight to cath labs. Again the diagnosis of a cardiac event is made by officers on scene, they use the tools but the determination is made by a doctor, not the emt's. -
Why? Only 1 person can answer that. "Smith claimed to have had sex with as many as 3,000 people of both genders." This is his claim for 3 years. This works out to 3 different people per day, every day for 3 years. He either had viagra on tap or he is dreaming

aussiephil replied to xstreetsweeperx's topic in Education and Training
We are talking pre hospital care. Treat what you see. Is it up to you, at best an intensive care paramedic, to make a differential diagnosis without the appropriate tests, scans etc? Simply no. That's why doctors earn more than we do. We provide a provisional diagnosis that is to be confirmed. Experience may tell us what it is but do we have the semi with all the pathology labs, ct scanner, X-ray, & ultrasound available to us? Until we do we treat what we see. Pain. Dehydration. Vomiting. We posture the patient & transport. That's what we are paid to do. -
Personally I wish ALL politics was more like the Italian politics. I mean it's sort of exciting & at least when Illona Staller, aka La Cicciolina, was there it was worth watching.

In thoery, I can't argue with this, however, fire in Australia have a nasty habit of entering into different areas via the back door, appearing to be offering a stellar service. In NSW they have sucessfully taken over rescue, & the Arson squad from the police. These are 2 major examples of wheat they did to gain pay rises through productivity gains. Rescue in this state was founded by the Ambulance Service, & I always thought that criminal investigation was the job of Police (my US friends, I refuse to use the wanky expaned terms like Law Enforcment Officers, they are Police, have been for hundreds of years & the dictionary definition is police(po¦lice) Pronunciation:/pəˈliːs/noun [treated as plural] (usually the police) the civil force of a state , responsible for the prevention and detection of crime and the maintenance of public order.) This is the same across the country. The problem is in the hospitals, the answer is not to allow fire to take on medical, they will require legislative changes to be able to transport. all that will happen is that people will be waiting with a Big Red Truck out the front of their house instead of a discrete ambulance. Currently only the statutory designated authority are allowed to transport. So do the community want the fire brigade & ambulance resources tied up because the health system cant cope?the fire brigades wont accept it. The answer is increasing funding to public hospitals, enbracing & enhancing Extended Care programs to reduce hospital presentations, not handing the fire brigades a reason to claim they can run ambulance services. Coronial inquests will always change policy & action. Especially in the case you described. The problem is the media focus on what sells their news, not what is reality, not necesarrily the full story. They are after sensationalism. The graduate programs are failing, they are failing because in reality, the degree means nothing. I can take a nursing degree & work as a registered nurse anywhere worldwide. The same cannot be said for paramedical sciences. This said, anyone seeing a tertiary education will not look at something that is not as recognised & the disparitry between what is taught in the Uni's is truly varied. Look at how many graduates there are from all the Universities & look at the numbers needed, once you look seriously at some of the graduates you see they will never be ambo's, for a number of reasons. I understand what you are saying about ambulance services in smaller communities, I have been there, but, the answer is NOT fire, in many cases they have less resources readily available than Ambulance Services. Timmy, you sight AED's as being a positive & we know from evidence based practice that early defibrilation will increase the possibility of a positive outcome, but, lets not kid ourselves, the survival rate is still less than 2%. Cardiac Arrests represent, overall, a small percentage of our role, can you really see our fire brigades responding to spit & dribble? They have said here that they only want the glory stuff, Trauma & Cardiac Arrest. The stuff that looks good on a TV Camera. Richard, when you said it is an interesting concept, & these were times before the communication options we have today. It is fantastic that you have the respect from you fire colleagues, here, unfortunatley, we do not get the same respect from fire, even the police have issues with them, but that is another story.....

There are not Pro's to fire based EMS, never has been, never will be. EMS is as associated to fire as Security is to Fire. There is no relevence, except EMS can finacially justify Fire. The stats can be bent to suit their argument, they always will do that, but simply Fire is a Profession, EMS is a profession, why do we insist on the conversations when in reallity it is a disrespect to EMS to insist that they are better if they are fire based. Let me turn this around. As EMS do more work than fire, generate more revenue than fire, should we not look at fire being a part of EMS, making them the poor mistreated cousin?

In reality we can all be accused of prostitution, we sell ourselves everyday we go to work. But, i was refering to not all women, just those who manipulate to get what they want through sex. You tell me, & be honest Annie, only if you want to, have you ever used your sexuality to manipulate a situation? Isnt that the same thing?
-
Herbie, marriage is prostitution as well. Women know if they want something, they just have to give it up & they got it. I think this is about more than a girl losing her virginity. I think it is sad when a person has to go to these lengths to educate themselves. So much for living in the first world.

Herbie, my condolences. This is an ode said by Aussies to remember our fallen, I hope it meas something to you as well FOR THE FALLEN With proud thanksgiving, a mother for her children England mourns for her dead across the sea, Flesh of her flesh they were, spirit of her spirit, Fallen in the cause of the free. Solemn the drums thrill: Death august and royal Sings sorrow up into immortal spheres, There is music in the midst of desolation And glory that shines upon our tears. They went with songs to the battle, they were young, Straight of limb, true of eyes, steady and aglow, They were staunch to the end against odds uncounted, They fell with their faces to the foe. They shall grow not old, as we that are left grow old: Age shall not weary them, nor the years condemn At the going down of the sun and in the morning We will remember them. They mingle not with their laughing comrades again, They sit no more at familiar tables of home, They have no lot in our labour of the daytime, They sleep beyond England's foam. But where our desires and hopes profound, Felt as a well-spring that is hidden from sight, To the innermost heart of their own land they are known As the stars are known to the night. As the stars shall be bright when we are dust, Moving in marches upon the heavenly plain, As the stars that are stary in the time of our darkness, To the end, to the end, they remain.

I read this thread from a slightly different perspecctive, not a legal one, but a Politically Correct one. In the not too distant past we had race riots that brought about so called equality, women burned the bra etc. Now, we cant go out & have a gay time, because it means something completley different, God forbid if we had a gay time at the Dyke!!!!!!! Yes, times have changed, this i find offensive, but that aside, would we be having the same discussion if it was in a predominatley black neighbourhood, with a white mans effigy hanging surrounded by effigies of blacks? To me it is just as offensive. The brave new PC world is changing us, not to be more accepting, racism is learned behaviour, acceptance comes naturally, but to be more resentful of those it is meant to protect. Positive discrimination is appropriate in some cases, but to exclude under this guise is more offensive than the scene in the newspaper story. I think the story shown is only the tip of the iceberg. I am proudly a white caucasion male of anglo saxon decent. I am, i believe, in the majority in my country. I am treated as a leper, with assistance available to minority groups hand over fist, but what assistance is available to me? Almost none. Does this not instill some resentment within? Of course it does. As First world nations we have responsibilities that I have spoken of ad nauseum, we need to offer protection, but, & this is a big but (much like mine), not at the expense of those who also contribute to our society. Equality should mean equality. I do not consider myself or believe that I am racist. I have friends whos origins are diverse. We all look at each other as equals, as we should. Do they expect me to show them more consideration because they are in a monority? No. Much of the PC mumbo jumbo being preached to us is from people who have no idea & think they know how others feel. They percieve there is a problem, or our politicians concoct a problem to divert attention away from real issues that could effect their re-election. Is it racist, probably, will you change the guys beliefs? Nope. Should we force our ideologies upon him? Well, lets look at the thread about the mosque at ground zero......... Phil What I have typed here is from the heart. My US friends who know me, you know what I believe, to those who don't, if I have said anything offensive, please accept my humblest apologies, it was not intended & is most likley a cross cultural issue, not intended to offend. Thanks.

To be honest Richard, Emergency never came to mind, the lap was amazing & stood for 7 years, & was only beaten this year. Considering the advancments in engine managment, aerodynamics etc, even though this is a street circut that is used once per year, to have a record stand for that period is an amazing effort & testiment to the fantastic drive & cohesion of the team to maintain the car to then allow the drivers to maintain an average speed of approx 95 mph for 161 laps is a great effort.
-
Here in aussieland we have just had our premier motorsport event. The first video is to show you 1 lap, the record lap of just over 2 minutes so you get an idea of the circut. This one is at the bottom of the straight, it is just at a right hand 290Km/h+ bend & not the ideal way to take the turn http://www.youtube.com/watch?v=ZwPgPqTMeco Testimony to the strength of race cars today. Beats making left turn after left turn hey.

Do You Have An Answer To "Privitization" ?
aussiephil replied to crotchitymedic1986's topic in General EMS Discussion
I think we need to take a step back & ask ourselves some questions. We are talking about services provided in "first world" countries. There are questions that arise here over profit or people. There is also the question of provision of services to those in our society who are unable to afford care for themselves. While government run services, are more expensive & may appear to be poorly run, the emphasis is, in my experience working for such an agency, more on patient care than on revenue & profitability. They tend to care & be the main port of call for the less fortunate. They have the ability to control far better a scope of practice & standardization of training. Government run services also (can) provide a safety net for communities. This can be gained through cross training & skills increases that are standardized. -
9/11 - Where were you, what were your initial thoughts ?
aussiephil replied to crotchitymedic1986's topic in Archives
Being on the other side of the world, it was late night & I was watching the late news on TV. The newsreader, a consumate professional, I had never seen make a mistake prior, or since, when she stumbled over words & they went to a live feed. Initial thoughts were that it was a freak accident & after watching for a short time, I went to bed. On waking, we put the TV on to come to the realisation that it was a terrorist attack, & that both towers had been attacked. I went to work in a busy office & that day was quite eerie, the phone didnt ring, no one came through the door, we were fixated on the TV. I hope never to be witness to anything like that again. -
Personal experience Annie?

Responding to a Nuclear Detonation
aussiephil replied to Richard B the EMT's topic in General EMS Discussion
My response to the detonation of a nuclear device is simple. Turn around & run away screaming like a little girl.
