  1. What is interesting is that while you say it will not end up on American TV, similar ads in aussieland have had a positive impact on reducing the road toll. Interviews with teenagers old enough to get their liceses have shown that they relate to the graphic nature of the ads, especially when used as a reaction to a minor spike in young road fatalities. We need to stop pandering to the moral minority. This shit happens in reality. If US censors think it doesnt, then they need to have their heads removed from their asses & taken on a shift or 3. Show it, if it upsets the poor little dears GOOD maybe they will be more careful on the roads. How long do we have to protect our kids? Are the next generations gaurenteed to be wimps cause they are not allowed to make a decision for themselves???????

  4. 2 Things. Abandon paid fire in favour of paid EMS. Dont combine them. It doesnt work. Fire will always try to justify their existance by trying to do more of everyone elses work cause there isnt enough fires (their core business) to put out. If they struggle, make them volly, or put them on a retained system where they are paid by the call. That way you will have more than enough money to pay for EMS. Secondly, volly v paid ems. Community first responders should be volly. But that is anyone with a damn first aid certificate. If we expect people to undertake further education, then expect them to use those qualifications to benefit others, should they not be paid for that? Would you expect your local town planner to work for nothing? He has lots of qualifications & he expects to be paid well for them & maintaining them. Isnt pre hospital medicine the same? So why should we have vollies in EMS Herbie, you tell me.

  13. I believe you did the right thing with this patient. I think that this is not just an immediate health issue, but moreso a whole of health issue. While many 70 year olds are more than capable of living unaided at home, I think in this case questions need to be asked of this patient & maybe supported care is more appropriate to ensure meds are taken, meals provided (substantive meals) etc. I would also question the possibility of diminished mental faculties (dementia of some sort). Dwayne, it sounds like you have made a decision based on the patients presenting condition as a medical emergency, you saw a need for hospitalisation & acted appropriatley. Well done.

  17. Maybe it is time to get rid of Med control & allow EMS to run as a professional autonomous body instead of reporting back to someone who cannot eyeball the pt. This is a relic of yesteryear that, with education should be removed & allow those on the streets to treat & make appropriate decisions regarding the patient in front of them.

We are fortunate here in that these patients are classified now under the mental health act & , if we believe they are not making a decision that a 'normal' person would make, we can the enforce transport under the mental health act & force treatment without consent, for their own benefit. I have only seen it used once, but it was without ramification & was needed at the time.
  19. This is just a back door way of fire building their empire & trying to be subversive. Now answer this. If EMS provides most of the work, therefore most of the income, should Fire not then become a part of EMS not the other way around?

  22. We have all seen what is described here, only the time & place may vary. I will ask but one simple question. What are the differences beteen a patient with a head (brain) injury & someone who is described in this thread? What was described above has happened here. The pt was in custody & left in custody. He was found deaed from a sub arachnoid haemhorrage the next day. He had no obvious external signs of trauma, but further police investigations showed he had bee staggering & fell earlier in the night. He looked like he was sleeping, he roused easily. He died. Now, what is your answer?
  23. There is a difference between First Responders & full ambulatory services, incliding paramedical services. First Responder is fine. Basic EMS. Nothing more. I have no problem with that. It was initially, from what you describe, a quick fix. This is reality. As I stated earlier, it is about Fire justifying their existance. Nothing more. Now when you say why have they not been revamped so that EMS takes control & fire is the poor cousin? I will scream this from every rooftop I have to, PRE HOSPITAL MEDICINE SHOULD BE NOT FOR PROFIT, FREE TO ALL PEOPLE. In some cases, we are all they can get to. Fire will not allow this to happen because they will continue to push thier hero banner, get themselves on TV looking like heros, when reality is EMS gets in & gets out as fast as possible. If fire calls are dropping, cut their budgets & chanel the money into EMS. start from the ground up & standarise all training, get rid of the mills that churn out moron medics. Maintain the fire department to the number of calls they attend. Then maintain an EMS service to the number of calls they attend. See who bitches about funding etc then. The only way this can happen is for thenm to be seperate entities. The Ego of fire (hero mentality) is too big & while they believe that they are the be all & end all, the utopia you are deluded about will never & can never occur.
  24. Whats right with a combined service? Does your local bank plan on opening a McDonalds franchise in the lobby? Or will the local child care centre become a guns & ammo dealer? That is how different EMS & fire are as professions. Why combine them. I am yet to hear 1 solid argument showing they are better combined. When will people agknowledge that EMS is a profession of its own & as such be a seperate agency?
  25. Peer review can be a 2 edged sword. It can be useful for those new in the field to assist with their development, great. As has been said, some who have been in the job a while may see it is an afront, but properly handled, they can assist with educating others as to their course of action. Thre can also be a dark side in this profession. Especially if there appears to be inconsistencies for the officer concerned. All I will say is if you want to peer review, be prepared to peer support as well. PTSD is real & reviews can be as real for the person as being there
