This poster howzyourchi made a very brief appearance on the forums yesterday, making a total of three posts. 2 were directed at me personally. This one : http://www.emtcity.com/index.php/topic/17328-my-path-has-been-decided-for-me/page__st__20__p__233126&#entry233126 and one that has been deleted (thank you Admin) essentially stating that I had too much time on my hands at work seeing as I would "bitch" about a canceled Christmas Dinner. The 3rd post was a thread this user started (also deleted ) threatening posters on the city with HIPAA and lawsuits. All three entries displayed such inflammatory language, lack of critical thinking skills and immaturity that most on this site quickly discerned that the poster was in fact a troll. Now the interesting part. I went in for my 48 hour shift this morning and was told that I was suspended pending the results of a compliance investigation by corporate attorneys to see whether I have violated company policy and/or HIPAA with my blogging and internet posting activities. Holy cow! All of this I believe has been instigated by the Grinch that stole Christmas posting. No one will tell me what posting (I don't blog) is the offending one, how long I can expect to be suspended and whether or not my suspension is paid or not. My supervisor and the shift supervisor had no information to give me, my supervisor having received a yellow post-it note this am with the bare minimum information (that which I had been given) and my shift supervisor having no knowledge of it at all. So now I am paranoid. Was that troll attempting to incite me into statements that would be incriminating? Gosh I would so love your comments. I am feeling pretty shaken.

LOL.. on a more serious note, I would think that the FD would shy away from libelous statements on the sides of their vehicles. I think PETA would have difficult proving the "Chickens are burned to death at KFC" statement. Imagine the publicity tho... Here in the land of the free and the stupid law suits.

The thing is Dwayne, the way this "profession" currently stands, you really don't have to be very smart. Limited scope of practice, minimal educational standards etc. and very little required in the way of actual medicine makes a strong back and a thick skin way more important than smarts.

It's funny, but just a couple of sets ago, I had this conversation with my co-workers. While I can handle myself physically, I did not get into this business to hurt my patients. I made clear to all my partners that if this stuff gets out of hand, we are leaving the ambulance and calling the cops. It is not "my" ambulance. It's a tool I am provided by my company to do my job. I would abandon it in a heartbeat if it means my personal safety and the safety of my partner. I ran an ALS intercept night before last for a BLS crew coming out of one of the Indian reservations. These BLS crews would prefer that the patient be transferred to our rig, allowing them to return to their quarters. My company prefers this as well because it enables the company to bill for the transport mileage. We intercept the rig and this 31 year old was messed up. Extreme ETOH and major head trauma. The guy was covered in blood, had torn through his straps and ripped off the headblocks and C-collar. The BLS guys were both well over 250lbs and pretty tough. They had replaced the patient, had tied him down with physical restraints and the one in the back was sitting on the patient. They sincerely wanted me to transfer this patient to my rig and go the 40 miles to the hospital alone in the back of my rig with him. ... Ahhh that would be a no! I pulled out my drug box and hit him with 10 mg Versed IM in the deltoid. I have learned the hard way that we do not do these patients any good fighting them. 10 minutes later, I was able to get my IV and do a complete assessment. None of that would have been possible if I had not sedated him. PS.. a crew taking another patient to the hospital about an hour later saw doctors, security guys, and just about every available spare hand holding this dude down in the ED while the ED doc was trying to staple his scalp. That medic told me the patient was kicking butt.

As soon as we go enroute to a call, a text message with call no., address, x streets and chief complaint is forwarded to the rig phone. After we call complete, our call no. and times are texted to us as well. It's useful.

Oh please - most of our rigs are between 150,000 and 200,000 miles. We run urban and rural with the largest coverage area in the state. Our "managment" feels that we don't take care of our rigs adequately and thus give us crap. 3 crews run an AVERAGE of 60 calls per 48, with 1 third on unpaved goat trails, washes and sand. What changes? - personnel - as last year's hires get burned out and/or gain the experience to go to fire jobs or other professions Edited to add the change part - cause that was the original question. -

In my neck of the woods, we do not request a pronouncement unless we get asystole in 3 leads. I don't understand why this is not a standard everywhere, seeing as it's too much to ask that emergency health providers understand non-perfusing beats.

It's taken about 1500 calls in a year and a half for me to figure out that this 911 paramedic stuff AS CURRENTLY DONE is not rocket science. It's great to understand the physiology of what is going on, but most of the time, on a call, what I am required to do is pretty limited. I try and go above and beyond, figuring out what is really going on with the patient vis-a-vis physiologically, socially, economically, psychologically etc. Is any of that necessary? Not really. There are hundreds of eager young things in the pipeline that want to go tube somebody. I am interchangeable with any one of them. I want to use my brain and knowledge to really make a difference, in a position where what I have to offer is necessary and required. I really like the idea of community based prevention and the opportunity to provide some answers to the situation as it stands. Let fire do the EMS thing - let those of us that want to do more find ways to do it.

Ambulance Ass definitely exists. If you have even the slightest inclination to use food to relieve stress, boredom then EMS will certainly afford you the opportunity to stuff fatty, sugary, delicious things into your face at the least provocation. As disciplined food/exercise habits decline, so does the inclination to maintain them. Vicious cycle - the fatter you get, the less you want to get up and move that lard.

I would so love the job of home visit Paramedic. I could do health checks, have time to talk to the patient, really practice medicine. I am so over the blood and guts of 911. Does anyone know if that kind of opportunity exists anywhere is this country? or how to go about creating this kind of position?
