  3. Don't forget, in theory the clot is no longer there since we gave tPA. Look at the whole picture and try not to focus on one number. The hypertension may have nothing to do with the brain. There is a cardiac issue at play here too.
  4. An ischemic stroke is caused by interruption of blood flow to a part of the brain. The most common cause is from a blood clot, such as when someone with untreated afib throws a clot and it ends up in the brain. The going theory, last I checked, is that the hypertension you see with an ischemic stroke is from the body trying to increase perfusion to the ischemic brain by using more pressure. In the days after someone has had an ischemic stroke, the part of the affected brain becomes mush (for a lack of a better term). The vessels are leaky and weak and the tissue itself is not very durable. On top of that, you have added a drug that prevents clotting so you have leaky vessels that have lost a good portion of their ability to clot. You are now set up for badness. If you add to that a high pressure such as the one in this case, those vessels are going to bleed and you end up with a hemorrhagic stroke from those vessels bursting. Bleeding is a known and possibly fatal complication of tPA administration. It is not the wonder drug that the neurologists would have you believe it is.
  5. Just because you have bleeding doesn't mean you will have increased ICP so you won't see Cushing's triad. Could it be afib? Sure. Could the pt throw a clot and have a second stroke? Sure.
  6. As a former BLS provider I would say, "Thanks but no thanks." This pt is sick and not stable for transport. He needs ALS transport. He is probably having a hemorrhagic stroke after the ischemia/tPA.
  7. I agree that anyone we care for should be treated with respect (unless they have given us a reason not too). As you say, most legitimate gun owners are reasonable and if you explain that for safety reasons both EMS and the hospital have a no-gun policy and aren't a dick about it, I would imagine they would be pretty understanding. Now if you are dealing with someone from the Limbaugh crowd, just tell them that Obama wants them to bring their gun in the ambulance and the hospital and they will gladly give it up.

  9. No problem Iowa. I didn't realize you were on a phone. As for the screams for help, no one has been able to agree on who's those were. TM's family say they are his screams, GZ's say they are his. The problem is, with a murder trial, there are two sides to the story, one of which can never be told.

  11. It doesn't matter if his actions were against the law or not, what matters is if his actions resulted in the death of another. Now Kaisu, if you are correct (and I'm not saying you are or are not because I honestly don't know) that GZ was walking away and getting back in his car then I would agree that TM had no right to attack him. Again, where is anyone getting their info from (we are usually pretty good about providing sources on this website)?
  12. I agree that the original situation has nothing to do with race and resulted from a buff/wacker who got a hardon getting to play cop. I disagree that GZ was not at fault. He made the decision to follow TM and is was GZ actions that led to the confrontation. It doesn't matter what kind of kid TM was. If he felt he was in danger he had every right to defend himself. He sees some stranger following him around a gated community at night in a car and on foot. The guy is obviously looking for trouble. TM had every right to stand his ground, which is what he did. Had GZ not created the situation TM would still be alive. I think federal charges are ridiculous. The guy was let off by a jury and that is the American system. It sucks but it is what it is. I have no faith in the juries of Florida anymore. Iowa, I'm curious where you got your "facts" from. From everything I've read, the autopsy shows he only had a trace amount of THC, which could not have been used that night. I think that hardly makes him a drug addict. Again, even if he was a drug addict, it is irrelevant to the case.   15. Eh, I think more is being made out of it than it deserves. Was BinLaden, McVeigh, Kazinski or any of the hundreds of serial killers ever on the cover of Time or Newsweek? I would assume so. The ones bringing attention to it are the ones that are giving him his fame. If no one made a big deal out of it most people would never have know (as we can see by some of the comments here). If you ignore something long enough it will go away with little fanfare.
  16. Agreed he doesn't HAVE to obey it, but he SHOULD obey it and if he decides not to, he should be responsible for the results of his decision. As for personally disobeying a dispatcher, I would never go to a beach in East Hampton on a nice summer day when told to post for a discharge out of Southampton Hospital. I can't believe there are people out there that would do such a thing.

  21. OK, I stand corrected. I thought I had heard an audiotape of the 911 call that had the call taker telling him to stay in the car as the police were on the way. Sorry about that. EDIT: So, doing some more Monday morning quarterbacking, errr, investigating, there are copies of the transcript of Zimmerman's call to 911 that go something like this: Zimmerman: No you go in straight through the entrance and then you make a left, uh, you go straight in, don't turn, and make a left. (expletive) he's running. Dispatcher: He's running? Which way is he running? Zimmerman: Down towards the other entrance to the neighborhood. Dispatcher: Which entrance is that that he's heading towards? Zimmerman: The back entrance…(expletive)(unclear) Dispatcher: Are you following him? Zimmerman: Yeah.   24. Based on what we have seen from the juries in Florida, I'm not surprised. This is one of those situations where there i single event that would have changed the outcome. That event was the decision to follow Martin. He sees a stranger following him in a car and then on foot. I think it could be argued that he was "standing his ground." Zimmerman set this whole fiasco into motion with his decision to disregard a direct order from the police to not follow the "prep". If he had listened to the police order, this whole thing would have ended at that point. I don't care if Zimmerman had a head lac or not. From what I have heard (I did not follow the trial very closely) it is reasonable to argue that Martin felt his safety was in jeopardy and was acting in self-defense. I also agree with Mari that we know a lot more than the jury did, so maybe it would change my opinion.
  25. Did someone look in a mirror and say, "Crotchity" three times? I grew up in the shadows of NYC and I can say pretty confidently that there were a few minorities around. I don't base my comment on anyone's race but on how this whole thing has evolved already. It's a media circus and more powder is being added to the keg. Personally, I don't think it had anything to do with race but the media has spun it that way. To me it was just a buff/wacker who couldn't get a real badge that wanted to play cop and got burned. EDIT: Mari, totally off topic, but where did you live in upstate NY?
