Are some rescues just not worth it?
Just Plain Ruff replied to Just Plain Ruff's topic in General EMS Discussion
Well, I don't know but some of the tone I inferred was to ridicule faith in general. AK and I have always butted heads over this one topic and I don't expect it to ever change. I love his philosophical arguments but this one seemed a little bit too condenscending. I'm not in the head of AK and I'm sure he has some logical response to what I wrote to him but it seems that every time I mention anything remotely religious that he jumps on it. I don't stop because he does this, I just know that he will pounce unless he's not reading the forums. -

Damn, AK, that word just gets your knickers in a bunch every single time. Just the mention of GOD makes you sick to your stomach doesn't it? I was using a figure of speech when I said "by the grace of God". Would you have made any comment if I had not used that word you hate so much? Every time I say anything about my faith or anything like that you jump all over it and diminish it or ridicule it. I guess that you must feel a much superior being for doing so don't you. I'm so glad you set me straight for using the word "god" (oops I said it again). I'll try to not put my faith out there for you to ridicule as you seem to have so much fun doing so. I answered your question on how my church would respond about the money yet you never replied. You really are a piece of work. -
the biggest drawback of the IPAD versus the Android tablets is flash. Without flash you really cant' go to Youtube, or watch any videos other than those you find for the IPAD. I don't know the particulars other than Flash and how the movies work but if the video requires flash player to work, you are out of luck on the IPAD and probably always will. Apple hates flash I think. If you want a usb, the usb cable provided connects directly to your computer and the other end goes int he Apple slot. Sort of backwards but if you are wanting to plug a usb stick in your IPAD, that also will never happen. I have used the xoom, the samsung galaxy tab, the Archos 5" internet tablet(piece of shit) and many others in my quest to find the best tablet for my ER docs and nurses to use when documenting on their EMR system, and by far the best so far is the IPAD but it's not a panacea. I hope this helps.

Not every act, nor personal failure needs a law created to allow you to escape responsibility quoted by Dwayne, well you are wrong Dwayne, we have to have a law to protect every single act. It's what's mandated by those less fortunate. And Dwayne, you are white so you can't say the N word here.

Yep if this was run by the deputy chief then it could be considered work related. He made the statements to his deputy chief. But seriously, this guys an idiot, saying all those negative things to a forum that his chief reads, that's asking for trouble. What I want to know is what group culture did this group have? He obviously felt comfortable and thought he was safe in posting this stuff, so I wonder what other things have been posted on this facebook page that have not been reported? And what has the chief said on that page? Sounds like a shakeup from medic to management needs to happen. Probably some doozies. -
Not that I'm aware of. With every state having different requirements and nothing being standardized across states, it's hard to say. I would add on to the above discussion by looking into the National REgistry of EMT's and contact them. They could help you determine what they require from out of country EMS training. I hate to say this but it's a hell of a lot harder to get licensed in a country other than the US than it is to license here. Most other countries have much higher educational and training standards than we do.

What state do you want to work in? Once you figure that out let us know and we can point you to the right location to get your answer. One good link is here. It's a google search. You will have to do more investigating via each link. http://www.google.com/search?q=EMS+requirements+by+state&hl=en&source=hp&aq=f&aqi=&aql=&oq= One other search - Go to google type in EMS licensing in florida or type in ems licensing in missouri Here is the result for missouri http://health.mo.gov/safety/ems/index.php that is directly to the state of missouri EMS department. You now have the tools above to get what info you need, it's up to you to follow through.

Military approves Tranexamic acid for major hemorrhage
Just Plain Ruff replied to Doczilla's topic in Patient Care
that was exactly my question Dwayne. Also, what happens if this medication is give while blood is being given at the same time? I'm really curious as to the long term effects of a patient given this in a urban setting, 10 minutes from the trauma center. What does the medication do to the body if blood is given close to when the patient is given the TXA, which occurs in many urban settings. Is the military allowing medics to give the TXA or is it being given at the field hospitals where a physician is present? I'm all for this but there are ramifications for the Civilian EMS world. I would be reluctant to administer this drug in the field until EMS field trials are done. What are the benefits in a urban setting, what are the benefits in a rural setting? Will EMS be able to give this? Should EMS be able to give this?- 15 replies

It seems that this could have been done differently but I defer judgement until all the facts are in and not just the reporting of one news reporter. It's been a deadly week for EMS Crews. This gives us time to focus on what/who is worth saving. I already posted a question but it comes down to this, we put our lives on the line every single day. We make judgement calls that sometimes bite us painfully, very very painfully in the ass. Are some rescues too dangerous? Are some helicopter scene flights or transports unnecessary? The public expects us to risk our lives to save some idiot who decided to go do something that 100 out of 100 people would never do yet when we determine that it's too dangerous, it is us in EMS/Fire or law enforcement who get the shaft for putting our safety first. The Darwin Award runners up all have public safety to thank for their still being alive. (well most of them at least).

Did I read somewhere that there was no-one in the car when they got to it?

Didn't think so. NO worries. -
You know, it's been many years since I entered EMS. About 20 to be exact. He was my partner for about 6 months off and on. We had worked about 50 shifts together. He was my FTO for the first couple of weeks. No new hires so they kept me on with him. I lost touch with him when I went to medic school. Met him again about 4-5 years later but we just never kept in touch although I knew where he worked but just never got up to his aircraft base. When I worked at a small service in north KC area, I was hoping to meet up with him on a air transport but it was not meant to be. EMS lost a great medic.

No one knows if he was at home when he posted this. I sure don't, especially from the article itself. If what he is participating involves the admiistration of the service then some could construe that it was a condoned facebook group that was condoned by the department itself. Perception is everything, If the Chiefs are doing it and a majority of them are involved, then it could be construed that tacit approval to this was given by administration. -

Yeah, that teacher's suspension was overturned once the first amendment got into the mix. -
Hugely broad brush crotch.

I got some pictures of before and after on a river. First picture is a idyllic river and the 2nd picture is a raging torrent of a river, huge undulating waves. Consider there is a person (alive by grace of god) being swept down the river. The river is only going to get worse downstream. do you attempt a swiftwater rescue or not? Are there some rescues that are just not safe enough.

Why is it that when you should be inside a house, riding a storm out that you end up needing an ambulance? Were all those calls truly emergencies?

The majority of lion fish envenomation are from Salt water aquarium enthusiasts. Really really cool fish but quite agressive and if you put your hand in the water and not pay attention you can get stung. NOT a fun thing. We had a aquarium enthusiast who got hit by not only one but two lionfish. He was a hurting unit. We ended up flying him from scene to the level 1 trauma center and he didn't fare too well but I don't think he died. Didnt' get too much follow up but I did see him wandering around the city once or twice a couple of months after the flight. We did the hot towel treatment and copious amounts fo fentanyl and when it ran out, we went to morphine. DAN network was called on this and they recommended the level 1 trauma center. Not sure why when a level 2 would have probably worked but that's who they recommended. Maybe they had a marine envenomation residency or something like that. Who knows.

This happened to me last night

I know all about metformin toxicity. Been there done that. My doctor checks it every time I get my labs drawn which is every 3 months. I fit the profile also. -
Very clear day, no inclement weather. Rumors are flying, some saying that the pilot just let it run out of gas, others saying that it had a leak. I keep telling those people to shut up and let the investigation run it's course, they are only fueling the fire. Questions as to whether this person really did require a helicopter to transport or could ground have been better? NOne of those will be answered until the investigation is over. I'm not going to make it to the funerals but there is a statewide memorial happening this friday at 10am. I'm trying to get the info on it now. It will be at a church north of the hospital in St. Joseph MO. I will be going.

It's my understanding that this was a closed group on facebook. If it was closed, then I can imagine that it only had those approved by the group owner and he had probably expressed these sentiments before. Assuming though. I operate 2 closed facebook groups and they are available for a few select friends and relatives. No one can see the posts on these groups without my permission or unless someone shows someone when they are online and accessing the page. So someone had to have ratted him out. I know this guy, or at least have met him a couple of times at various conferences and he seemed to be a pretty good guy. What lurks under the surface is often different than the public persona that they project. But idiocy doesn't just come in stupid sizes, if you call out specific members of where you work and you do it in a easily accessible medium, (someone always has a axe to grind) then you deserve what you get. This is a prime example of not posting anything you wouldn't want your wife, brother, mother, father, jesus, your boss or anyone else to see. What you post, can and sometimes comes back to haunt you. It's never deleted off the web. Take this as a lesson learned people. Like doc said, if there is a policy against this in the organization then this guys toast, but I'm sure that same sleazy lawyer could make an argument that this guy was only expressing his first amendment rights, policy or no.
