I have found in the past that if you can give a good history and physical info over the phone, tell the doctor what you are thinking adn what you are wanting to do and WHY!!! then most of the time they have no problems giving you orders or they may say, sounds good but try this first. That's why we have access to the doctors, I'm betting that they have had to take care of whatever you are calling about at least one time before and they know what they want to do and what they want you to do. Just remember, Your lifeline in cases where there is a question is the doctor. Use them, that's what they are there for.
  8. Fiz I absolutely agree. That was the reason why I contacted the doctor about the pain medicine issue. It was beyond my scope to do that so I relied on the doctor. For any other patient I would have followed protocols. I also believe that our protocols are what save a lot of us from having to go through a lawsuit. If you follow your protocols and the plaintiff's attorney see's that you did then you are more than likely not going to be named. Deviate from the protocol and patient suffers, your ass is theirs. When in doubt follow the protocols, if that fails then call medical control.
  9. rockstar post +20 I also believe that you take the patient first. But it's a fine line. Luckily, I used to work with the doctors who received my patients. They trusted nearly all the medics we had and we rarely had to call for anything. I would only call if I needed a 2nd opinion such as having two drugs and each were good but one was better for this condition and I wanted to make sure that my thinking was right. I had a case where we had a lady who fell outside her house. Ankle was really badly fractured, open and hanging by a couple of strips of skin only. She was allergic to almost every medication out there but she needed pain control to allow me to splint her. She said she had a slight rash when given fentanyl but was deathly allergic to morphine. I discussed this with the doctor and I suggested the treatment plan of dosing her with 50mg of benadryl first and then give 25mcg of fentanyl iv slow. The doctor agreed. This was the right course of treatment because she ended up not getting a rash and we were able to give her the other 25mcg of fentanyl on the way into the er. This was collaboration at it's best and WAY WAY WAY outside our protocols. Was it the right thing for this patient? You bet. She was able to tolerate the splinting of her ankle and did not suffer from any ill effects of the fentanyl allergy. Her doctor was none to pleased at the administratoin of the fentanyl but once he saw her fracture he said "Wow, good call guys" and fixed her ankle. Do I think that we could have killed her, yep, if her allergy was any more significant but it all worked out. Thinking outside the box is something that we do not do well in EMS.

The first letter you send needs to be to the management of the service who transported the kid. Manipulating a fracture is bad medicine. the second letter I would write would be to the state or whoever licenses medics in your locale. Good luck

  13. I think that's old news or old recommendations. when our daughter was born, the doctor said that she should be in a rear facing seat at least until 18 months but preferably 2 years. She also said that if we could keep her in the rear facing until we got up to the weight limit that was even better. This is really good info to have for anyone getting ready to have a kidlet. We are keeping our daughter in the rear facing seat position for as long as we can. She doesn't mind, honestly. Makes my wife feel safer.

  18. Let's address this point by point, first to point out that your demands are just the tip of the iceberg. I say why not 2 million, why not 40 acres, a mule, a car, a boat and 2 chickens in every pot. If you deserve the above in your list, then why not the other stuff. I'm just guessing at the number of african americans in the US but let's say 80 million. I could be off but not by much 1. Free scholarship for the college of their choice. Average tuition is around 20K per year so 80K for 4 years. That would cost 6,400,000,000,000 Well that's 6trillion 400 billion. 2. No taxes for 200 years - Can't figure that one out as it would be too many zeros 3. 0 interest for 200 years - uncalculatable 4. 1 million per black person = That would be 80 with 12 zeros after it 5. Workforce of blacks at 25% - I'm good with this one. So the grand total would be over 88trillion dollars. NOpe aint gonna happen. Never in a million years. There is no way that even your black congressmen who supposedly work for you, there is no way that they would even float this out and try to make this a law. Actually you are just talking stupid now. Let's be a little more realistic and come back with something that won't bankrupt our country. YOu see the fatal flaw in your logic is first off, you shouldn't be able to do the math because since the white person held you back that made you too stupid to pass any type of test with math in it. And the second flaw is where do you think the money will come from? It sure isn't coming from your race since you won't be paying taxes. So how do you expect to get all this free money. And one more question - Does the 1 million dollars per black person continue in perpetuity? If a black baby is born on the day after the money is divvied out, should they get their own 1 million dollars. I say no, because at the time themoney is divvied out the playing field is level so the reparations should end at that time. But somehow I think you will find a way to say that even children born after the money is given out should get it. But at least you have a house lone. All those black folks don't even have a house. But don't worry, as soon as the million dollars is given out, and the home loans are 0% then they can have a house like yours too. Wait a second, just what are those black kids doing in front of those really nice schools that we have built? I was doing a delivery for one of my fathers old co-workers at a inner city school just the other day and when I entered the school, most of the students are black or hispanic at this school, a whole bunch of them were sitting in a classroom at their desks. I can't for the life of me figure out what they were doing there...... hmmmmmm maybe getting an education. Or am I wrong? Seriously Crotch, you are just talking dumbed down in the above type of response. Did you go to school? Did you go to college? The school I went to was 20% black, 4% other. The blacks got the same education as I got. Same classes I took, same teachers.

According to my family tree and all the research that I have done, none of my families ancestors owned slaves so why the hell should I take any responsibility for something that I or my ancestors did NOT do. Crotchity, if you are going to try to force us to take responsibility for past actions of our ancestors then you should also take responsibility for what your ancestors did which was a number of blacks were involved just as deep in the slavery trade as they were the ones who captured those said slaves. Amazing that you can chastise us for our blood on our hands but seriously not take responsiblity for some of your ancestors actions. So I'll ask you this one more time, but I think that you are too cowardly to answer this question since you seem to only want to stir the pot - What is the answer?   22. I am thinking you are near Bremerton or thereabouts. If I were you, I would continue schooling to the medic level and then look to the job market. You are still young and you have plenty of work years ahead of you so take this opp to get all the education you can. You will find it more difficult to do so when you have a wife, two kids, a mortgage to pay and a full time job to juggle. There is no rush in getting into EMS as those jobs will always be there.
