Skylife Helicopter
Just Plain Ruff replied to Just Plain Ruff's topic in Line Of Duty Deaths & other passings
Amen Mike, amen. This is the absolute worst time to lost a loved one, it just seems that this compounds the sorrow but honestly, like you said, no time is a good time. What I'm hoping I read about it being very foggy is not the cause and that the fog came in after the crash. One of my best friends flew on the helicopter that is based at my former employer, Every day he worked, I said a silent prayer for a safe flight when I heard that bird start up. So far I've lost at least 5 friends/acquaintances to helicopter crashes in the past 15 years. -

I went to the website outside of clicking on it via the link the yahoo/spammer provided in his post. It brought up a physician recruitment website. Bless his little ole heart, I'm sure he's got some quota he has to meet to satisfy some innate sense of purpose. I as well have been wondering where the report button has gone as well. It's gone from Dwayne's site as well. -
RIP to the Crew and patient of a SkyLife helicopter on 12/10/2015 out of Kern County. The article I read did not give the state. The crew was supposedly transporting a critical patient when it went down. Reports of the area being very foggy where the helicopter went down. Most of the article mentions Fresno and Bakersfield which one might assume this was in California. Before anyone makes any rash judgements on the cause, just lift these victims up in your thoughts and prayers if you pray.

Paramedic Grad w/o Experience Needs Job
Just Plain Ruff replied to reddsky's topic in General EMS Discussion
WOW, great recruitment website that has absolutely NOTHING to do with EMS whatsoever. If you are going to spam our site, make sure your employment website has something to do with helping EMT's and medics get jobs rather than Doctors getting jobs. Now Just go away. -
Problems Getting to Your Phone

we don't even know what your prototype is. I'd would definately be interested in being a "betatester" but you really haven't given us a idea of what version 1.0 is. At least give us an idea of what version 1.0 is. Think of us as your seed money guys - you have one elevator trip from the lobby to the 110th floor, you have about 3 minutes to WOW us with your idea in order to get your money. Or consider us your Sharks on Sharktank So WOW us. -

Smartwatches for EMS are a HORRIBLE idea. HORRIBLE with a capital H. I have a list of reasons why which I will post if someone wants to know why, but I bet many can figure them out already. -

is that to me, Mike? or to the OP? I already have a smartwatch linked to my phone and there are HUGE and I mean HUGE limitations to that. I can write a book about those. -

I'm still not seeing anything but a conglomerate on your profile picture on here. You can send me a picture (ruffems@gmail.com) or whatever and I can try to post it better for you. I'm still not seeing the problem you are trying to address. My phone is the Galaxy s6, It has a case, it's pretty big with the case. If you add something in addition to keep it on my wrist, I'm not going to use that product at all. I have the samsung neo watch which connects to my phone via bluetooth where I can make phone calls, text etc. Your product had better be revolutionary in order for me or many of my contemporaries to use it. We just dont' bring our phones onto scenes with us. but in the ambulance with us, that's a different story. Here is where your product needs to be better 1. size - it needs to be small enough to not get in the way of me doing my job, if it gets in the way of my intubating someone, starting an iv or just causes a nuisance, I'm not going to use it 2. comfort - if it's uncomfortable for me to put on or wear on a daily basis, if it makes my arm sweat or causes me to want to take it off on a regular basis then I'm not gonna wear it 3. ease of putting it on - it better be easy to put on 4. will it stay on? does it stay on? If it slips around, does it stay where it's intended? If it doesn't do that then it's worthless to me. 5. infection control - how clean can it stay, how hard is it to clean? 6. durability - does it break easily - is it cheaply made? 7. price - I'm on a budget - if it costs more than 14 or 15 bucks, I probably cannot afford it, this isn't a necessity, it's a luxury for me so it needs to be cheap but NOT cheaply made. But with you saying it's going to be a extra battery or external battery, it's not going to be cheap, Plus do you have the expertise for both the apple and the android build? Do you have the financial backing in order to get the Apple branding behind it which I would think you might want. What kind of financing do you have on this so far? -

Looking at your profile, I'm seeing about a 1/3 of what I should see with your picture. Any chance you can post a better photo of this item. I'd really like to know your replies to ParamedicMikes questions I'm also curious as to why you think that the currently available Off the Shelf options like the Otterbox or other cases are not good enough? Is it because there is no strap that goes around the arm? Forgive me but someone has to say this, you are not making a good presentation for this whole deal. When I worked at Cerner, I got caught in the elevator with Neal Patterson, he liked to do certain things to employees and he did this a lot. He made me explain my area of expertise in the elevator ride up. I had 45 seconds to do that. I didn't do it very well. You have an open forum here with a group of really experienced providers and it would be great if you can provide us with more information rather than just piecemealing it one little bit at a time. Again, no one here (and it's really only been mike and I) is busting your balls, but in all honesty, your presentation isn't very good. Let's see some more info please. But like I said in the first post all the way at the top, I don't bring my phone into the scene with me. -
Problems Getting to Your Phone

Here is one thing that is missing with phones. You are driving down the road, you have your phone on the seat next to you. You drive off the road and into a ditch. You are trapped in the car. Your phone is on the floor of the car just out of reach. It is 2am and the road is very lightly travelled. How do you get to your phone to make a 911 phone call? Aside from Onstar there isn't really anything. The only thing I know of that bridges that gap is the samsung watch that I wear or I think the apple watch but only if you have the emergency notification function enabled. You press your watch button three times and it sends out texts or 911 calls depending on how you have it set up. Not sure if this is something the OP is wanting to work on but probably not. I daresay we need much more info from this poster before we can help him more. -

Just Plain Ruff replied to TheWolfman2112's topic in Equiqment and Apparatus
I dont' get it. If the phone is to be held indefinately then what's the point? If the product is to hold the phone indefinately, is it meant to allow you to get it out while working or what? More clarification as to what your device is meant to do would be good. My advice is to give us some better ideas about what you have in mind for your product. Plus not busting your balls but your survey is flawed. Question 2 is required but I voted No on question number 1 and it made me answer question #2 when the actual question was if you answered Yes on question number 1 I was to answer question #2. My answer was No on #1 and it should not have made me answer number 2 -
Now that's a good start, seems like the news media is learning, but..... maybe they were prepped for the article. but in all seriousness, at one of my services, we had the ability to lock the doors, and then push a button some where on the ambulance to unlock the doors when we returned to the ambulance. About 5% of our crews ever locked it that way, guess which of the 5% or the 95% I fell into?

Problems Getting to Your Phone
Just Plain Ruff replied to TheWolfman2112's topic in Equiqment and Apparatus
I don't carry my phone into emergency situations, I spent too much money on my phone to risk losing it to a job that won't reimburse me if it's lost. -
Again Melvin R, I was not busting your balls or criticizing you, I was just asking why you were questioning the manufacturers numbers. Here is what I would do I would contact a number of manufacturers of IV solutions and ask them the exact same question and see what answers you get. Then take the numbers you get and develop a range that your system is comfortable with. It seems like your system is operating out of the safe operating range. So that's what I would do. But one caveat, you run the risk of running into trouble and legal issues if you heat the fluid of let's say Baxters fluid to higher than what Baxter says is recommended for their fluid based on let's say "medtronics" numbers. So if you heat baxters fluids to medtronics numbers of 114 degrees yet baxters numbers are 108 degrees and you use medtronics numbers, then you run legal and medical risks. Does that make sense. My opinion is to only warm your fluid to your suppliers recommended temperatures and no higher. I'm sure your risk management team would say the same thing.
-
Melvin, you have the manufacturer's recommendation, do you not trust that? Just a question not harping, but Baxter is very very well respected and they do provide a vast amount of IV fluid to the healthcare community. I think I would go with what they say. Are the other sites you are referring to, are they manufacturers sites or are they just sites that are out there that you found? Honestly, not harping or busting your balls here, but why are you not trusting the manufacturers information?

thanks Bernhard for the firsthand perspective. I think that if those in the US had that information, we might have a somewhat different perspective on this situation.
-
My thoughts have been changing on this matter, I have more to say but I'm at work. But after all the hateful meme's, the hateful statements and from what I have been reading from non-judgemental websites and the sites that I believe are truthful and provide information that is valid, my viewpoint has changed, but it's tempered with caution.

Please keep this civil as this is a very hot button topic Syrian Refugees - what a hot button topic - Obama wants them to come to the USA - 10K or more if he can get that many here 27 state governors have said NO, not in our backyard Immigration law is on Obama's side here though, the states can symbolically refuse the refugees but they cannot legally keep em out. At least one terrorist in the most recent attack in Paris was from Syria, but the others were supposedly French citizens or from Belgium I believe - the narrative is fluiid and changing each day. But the Boston bombers were refugee's I have to say, I for one am more worried about the single or pair of shooters who go into the movie theater or the school and open fire than I am the syrian Refugees because they are not here yet and are not in our cities as of yet. I have yet to hear of any terrorist attacks by those refugees in the countries that have accepted those syrian refugees except France. Correct me if I'm wrong though. But I'm on the fence here, I want to embrace the american ideal and american dream to these people but on the other hand, I don't want them here based on the off hand risk that some of them do pose. This I believe will be the defining moment of our sitting president. It will be his swan song legacy and if there is one terrorist attack on his watch from a syrian refugee or a group of refugees, his legacy is toast and the legacy of anyone who supported him is done. Again please keep this civil.

I had a patient one day present to the ed with a tens unit with one pad on each of his temples and the unit set to 8. He said it was the only thing that would help his headaches. You know what, I believed him. And he never once asked for dilaudid.

If you have the room and if you can be trained by a cardiologist or someone who knows the procedure then I don't see the harm in trying this on your patients who have SVT who are not critically unstable.

TWIAGE - new app coming
Just Plain Ruff replied to Just Plain Ruff's topic in General EMS Discussion
I agree with Rock Shoes, I can't remember the last time that I didn't give a good phone or radio report to the hospital. If I'm really busy, my driver is going to call the hospital and tell them what we have and say that I would have called but "he's too busy with this type of patient and you all need to be ready" Sometimes technology can be a hindrance and after re-reading this and doing a little bit of thinking, I know I know, Ruff's thinking again, I liken it back to this, this is actually double documenting unless this get's interfaced into the electronic pcr that you use. If it isn't interfaced into the patient chart that you write, and let's say you put something down on this Twiage app and then you put something else down in your ambulance record, there is a discrepancy in your records and that could lead to trouble down the road in a lawsuit if heaven forbid there is one filed. I'm not sold on this yet. If it interfaces into your patient record then I'm a little more on board. -
What if there were 2800 locations in your jurisdiction that you could not go without the police? Surprisingly, there are 2800 locations in Scotland that their medics cannot go without the police coming along. Read more about it here. http://www.emergency-live.com/en/news/in-scotland-there-are-2800-addresses-where-ambulances-are-not-allowed-to-go-without-police-escort/ This brings up the question - does your dispatch software offer the ability to flag an address that will alert the dispatcher to send PD with you? Or does Dispatch rely on the type of call to determine that? Maybe it's time that we get software that does that. It might just make our jobs a tad bit safer.

Listen, then we can't get away with anything doc. I have the worst case of Tinnitus, even my eent says that my upper register hearing is about 50-70 percent gone. Of course that upper register hearing accounts for the sound range of the woman's voice. I actually have a medical excuse for not hearing you women when you talk. What's your excuse doc?
