  4. Yup I've had that done to me on the subway. Have to be careful too, you get caught up talking to a really interesting bum or something and bang your two hour limit on MetroCard expires and you gotta pay agian before you can get out. .... or just go through the gate after a guy with a bike but I don't condone that; if only my chips from the Connecticut turnpike still worked on the turnstiles

We picked up an old Indian gent one night who had some nasty looking ST changes with chest pain, spoke no English whatsoever, boy that was tough coz his Son also spoke limited english .... turns out after we got him to ED I found a "non English emergency phrase book" on car, FML.

  10. Hmm I agree with what you are saying. Now I was taught that compressions should be continious, that whoever is doing ventilations should do 8/min and that intubating is not a priority because an LMA should work just fine. Having said all that out there in the real world we know it doesn't work like that and that it's easier to drop an ET tube and get it out of the way because it's easier to transport, provides better protection of the airway, is an alternate drug route (!) etc I've generally seen Intensive Care Paramedics intubate fairly early, often before an IV is established if they have turned up first-in and not responding to back up another crew.
  11. How come you say you'd have to interupt compressions to use a bag mask? I could see if you were on your own then perhaps maybe but with two or more ambo's at minimum it should be ok; heck it should be dandy right coz of the good ole ambo trick of "more is better" and by the time you get on the stickies, confirm VF and start pounding away, two cops, eight firefighters and the medical director have shown up!

  13. I don't believe this is acceptable at all given the long delays where no compressions are taking place. Drop an LMA if need be rather than peeing around trying to intubate if its that difficult.
  14. When I was in San Diego it was Rural/Metro as the third party provider; that's the extent of my knowledge. Oh I lie, they have red ambulances.
  15. 28/1/08 Redirected to a priority one (AMPDS/ProQA strikes again!) turned out to be a renal failure patient who felt unwell for three days. Pretty much set the tone for the rest of my jobs!

  17. I've heard around some hospital systems will remove field IVs and start thier own; although I've never seen this myself.
  18. I was taught'd with the edumkuashin that we should no longer assess for rebound ternderness, as to why, search me .... I'd venture an edumoocatered guess that it's probably of little value and considerable pain to the patient.
  19. We have fentanyl for pre-medication prior to ketamine or midaz for RSI. There is talk that it will be introduced to replaced MS. Oh and for the record, if I have severe crushing chest pain or a life threatning asthma attack in LA City or the greater unincorporated areas of Los Angeles County covered by the rescue squads from LACoFD; I'm driving to Portland or Phoenix, heck maybe even Reno ... really, really, fast!
  20. It would not. Perhaps our friend works in one of the backward ass systems I have heard of who do not use saline or hep locks and run a bag of fluid whenver they start an IV. You know the old ambo trick of "more is better" has a perfect application here, not.

Love it, they need to show the two fire trucks rolling up and eight guys in all thier getup barrelling thru the door with the almighty "everything is ok, highly trained professional Firefighters are on scene now!" look. .... cut to six guys standing around doing nothing, one doing a blood pressure and the other talking to the two cops who have shown up.

No It seems the children of today expect to be spoon fed and have education at least, if not most things, handed to them in broken down, nice, bite sized managable pieces so that nobody fails or ends up looking stupid or "left behind"; much to the deteriment of higher level cognitive learning. One of the presenters at the International Roundtable on Paramedicine speakers here in NZ last October mentioned the "trophy children" generation who expect some form of reward for just doing what is expected of them; the two are maybe not hand-in-hand concepts but are closely related IMO

Thats true, does it mean every time I walk through a crowded mall with my backpack on or drive to the market I should be detained or somehow "intercepted" by those responsible for "national security" coz heck I might blow the whole place sky high? Don't look now but that noise in your roof might just be Osama charging up his laptop by plugging into mains power .... his kidney machine probably needs some juice too you know
