  3. Yep another ANZAC day comes and goes, my great granddad was in world war one, tough old bastard only had three fingers on his shootin' hand but that didn't stop him! lived to be like 99 or something crazy like that too!
  4. I have absolutely no idea but his cause for unconsciousness is not rapidly improving and his SpO2 is consistently low so I am thinking about anaesthetising, paralysing and intubating him.

In the case that the Registrar died last week from fatigue and has not been replaced then the answer is ask the Consultant Yes, I am taking the piss Good luck bro
  6. We do not have a "Medical Director" per-se in the American sense Here in NZ we have a big ambulance service and a little ambulance service plus the Defence Force Medics. The big ambulance service has a Clinical Management Group which consists of four Physicians consisting of one dual Consultant Anaesthetist/Intensivest who is the "Clinical Director" and three Consultant Emergency Physicians; the little ambulance service has a "Medical Director" who is a Consultant Emergency Physician while the Defence Force has a Consultant General Practitioner who is the Director General of Defence Forth Health. Each of the three entities looks after clinical issues specific to their service. Scopes of Practice for each level (Technician, Paramedic and Intensive Care Paramedic) and the Clinical Practice Guidelines are nationally agreed to between the two services and the Defence Force and national issues such as these are addressed through the National Ambulance Sector Clinical Working Group which consists of three members of the big ambulance services' clinical management group, the clinical manager of the big ambulance service who is an Intensive Care Paramedic, the medical director and clinical manager of the small ambulance service plus the defence force GP and one or two other clinical types. The clinical director for the big ambulance service is an absolutely brilliant gentleman, he has taken the ambulance service leaps and bounds and so far from where it was ten years ago it's not even funny. He is a dual qualified Consultant Anaesthetist and Intensive Care Medicine specialist (FANZCA/FJCICM) and has forgotten more about medicine than I can ever hope to know! He's one of the most approachable and down-to-earth people ever and will go out of his way to help where he can, although he managed clinical issues with his team for over 3,000 Ambulance Officers so might tell you politely to talk to a more appropriate person but he will always respond to you personally. I have much respect for him and think he does wonderful work, because he does! Man love for Tony Smith or what right here?

I am thinking thermoregulatory (heat stroke) or metabolic partic hypoglycaemia Now my cousin never was the same again after working laying linoleum because of the glue fumes but I don't think it's roofing tar ....

  10. Pardon my Kiwi-ness but I see threads like this frequently Shouldn't it be up to your employer to provide the necessary tools of work to do your job? Ambo's here are not expected to purchase anything because they are employed by the Ambulance Service so its a requirement of the Service to provide their staff with what they need to do the role they have been employed for. Now in saying that some do buy a better quality stethoscope than the hunk of junk that comes in the resus pack but it's not a requirement

It is also the domain of Californian "Emergency Medicine Residents" who look more like a surfer than a Doctor Note interestingly the Medical Board of California licenses one as a "Physician and Surgeon" which here are two totally different things and one cannot be the other; e.g. a GP cannot perform surgery and a surgeon cannot set up shop as a GP
