  1. I drink extremely rarely and only socially. I don't mind if people drink but it's not my thing, but then again I'm biased having seen enough drug abuse in my short life to last ten lifetimes. Now, if Dwayne would get his ass down here from whatever leper colony he is hole up on in the South Pacific I would be glad to fire up the barbie and get a bottle of something smooth from the liquor mega store for him.   5. I am physically young but mentally older than time itself. I have seen and experienced much more than a person of my age should and fight a constant daily mental battle to get the hell away from it and pretend the last quarter century of my life has, for the most part, not existed. No Around the place, I have a library of over 100 medical text books (Bates, Guyton and Hall, Rosen, Williams, every subject from Anaesthesia to Zoology (Anatomy)). I read and research extensively and in the past have had several friends in the Ambulance Service; I've got friends who are Consultant Physicians and they helped me out by getting me as much practical experience as they could but most of my knowledge comes from my own self-teaching. A few years ago I did complete some of the Paramedic (ICO) modules when it was still the old ICO course and not the "new" ILS pathway (which as soon as a degree provider is finalised will formally become a Degree qualification within the Ambulance Service). I think I've still got some of the work books and such around here somewhere. A certain shaggy haired bloke who will remain nameless has also hit me in the head with the oxygen and gone "you dumbshit, this is how you turn it on!" and one or two other things. It's amazing what you can teach yourself; not that I consider I actually much of anything really. I know very little and I'm not taking the piss, honestly, I don't think I know a lot. I have had people tell me I do, but I don't believe them. Ouch man, I have to go find soul analgesia now, best get on the biophone and ring up teh medical control about that one... I've known several Paramedics and spent plenty of time with them discussing a multitude of clinical and operational issues both pertinent to here in NZ and worldwide which has been both online and in person, sometimes even on the couch at the ambulance station. I've even been known to put on a yellow high visibility jacket with "AMBULANCE" written on it and lug a big green back pack around or put a drip into somebody but it did cost me pizza Oh and thank heck Stryker stretchers are used now those Stollenwerks were a pain in the ass to get out of the vehicle! "Our" or "we" was always in reference to New Zealand. I would refer to anything from the UK as "theirs" or "them" because I'm not from the UK. Good to see you found Tony Smith; you should talk to him as he is an extremely intelligent Physician, dual qualified as both a Consultant grade Anaesthetist and Intensivent (ICU) who has been involved with StJ since just before 2000 if memory serves correctly (I did read something like than in the Northern Region Forum magazine a few years ago). He is an outstanding and truly brilliant man very passionate about pre hospital care (heck not unlike me) and clinically the ambulance service has come leaps and bounds under his eye, and that of a group of dedicated and passionate people such as Craig Ellis, Doug Gallagher, Mark Deoki, Sue Gullery and Sarah Werner. At the turn of 2000 it was reality that "authorised competencies" for a National Certificate Officer (Ambulance Officer) included aspirin, salbutamol and entonox. Nasopharyngeal airways were an "authorised competency" for an Advanced Care Officer (Paramedic) and bloody hell New Zealand was still using PR stesolid (rectal diazepam). A decade later and viola, a third of a masters degree is required to become an Intensive Care Paramedic, New Zealand has one of (if not the) highest success rates with rapid sequence intubation (a procedure banned in many parts of the world because it can't be done properly) and we are unfortunately comparing ourselves to parts of the world where salbutamol and entonox are still considered "advanced" procedures despite the fact they've been entry-to-practice interventions here for maybe 20 years?. There are many, many positive and leading-edge things the Ambulance Service is doing here, many place our little nation at the forefront globally and a great deal of it is thanks to Tony Smith. So yes, you should talk to him because your time will be far better spend discussing clinical issues and developments than worrying about me, who really didn't do anything anyway. Sure seems it mate, you dragged a whole lot of other crap in here that has nothing to do with anything we are talking about and that isn't really accurate anyway but that is a discussion for another time and place What lies? Because I am not privy to the inner sanctum of your mind and it's cognitive processes. It would be fair to say that an argument which sounds reasonable will be believed until proven otherwise but I can't guarantee that so you are free to make up your own mind regarding what I say. I'm surprised you haven't had a go at me about my avatar; for the record, that's one of the old patches that were used pre-2002 and I chose it because it is not current and does not have the name of the issuer on it as do the new ones. Why did I do that? Because I'm not affiliated with the orginisation whose name appears on the new patches. Please provide specific examples, you will more than likely be very hard pressed to do so, I may have said once or twice "X does this, Y does not" but that's totally different than saying "I know X does this because I work for them". How can I possibly associate myself to orginisations I've never mentioned using the terms "we" or "us"? In your interpretation Am I the only person you tell in such a matter-of-fact tone what they are thinking or what they were trying to do at X point in time? I wish I had your ability to know what other people are thinking and why so, it sure would come in handy! I won't disclose my comments of what I think of you, because that would be rude. I was trying to think of a screen-name that was not taken; I couldn't think of anything more original and seeing as how this is a medical website I dno at the time "medic" was the best I could come up with. I'm doing nothing more than trying to make meaningful contributions to rationalised clinical discussion and debate, something I think you could say objectively I have done successfully I am not sure. This breaks down to 524 legs, 354 ILS approaches (including 27 Cat II and 19 Cat III), 129 visual, 8 RNAV and 33 VOR or VOR/DME. Yes, I still have my log sheet. I have my CPL-FN(G), Met and HF. It was a pre-req of my work that I actually understood what I was dealing with so y'know that's how that came about. I'm not here to seek respect from anybody nor do I respect it and in fact, I generally shy away from it and am the first person to point out I know very little and have only a small amount of practical experience with a small sub-set of my knowledge. I've never claimed to be anything I am not here, if you want to get searching through the 2,000+ posts I've made to call out specific examples you're most welcome to get to it, it'll take you a while because I haven't done it. The "medic" part probably was a poor choice back in 2007 but I couldn't think of anything else to be honest because "Kiwi" was taken, I've actually been thinking of changing it since like about 2009 but I can't think of anything else to change it to. I know somebody else on here who has a user name containing the phrase "medic" and they are not a qualified pre-hospital clinical person; so why don't we go berate them about it?
  13. I bet most of these students are also very young and inexperienced? In general my conversations and readings with Canadian Paramedics it seems there is a much greater respect for knowledge than in some other parts of the world, generally, exceptions will always exist.

I hate to say it, but two or more hospitalisations in a year for asthma put you at an increased risk of death from asthma (as would previous intubation for asthma) Glad to see you got your asthma under control Bloody asthma plans and patient education, the rate of life threatening asthma has dropped off quite a bit here it seems ... what happened to the good old days?
  16. I am curious, have you ever been intubated for asthma or have you been in hospital more than twice in any one year for asthma? Ketamine is actually the preferred induction agent in asthmatics due to its bronchiodilatory effect but, intubating an asthmatic can bring on a lot of bad ju ju as I've commented elsewhere at length about. It's something with a very fine line between help and hurt.

  20. I completely agree and strongly disagree with the "inverted triangle" that has traditionally been Ambulance education. The model Ontario has adopted with the two year University level education for Primary Care Paramedic is top-notch and one of the best in the world because you get all your basic bioscience downpat before you even touch patients and the experience base gained between PCP and ACP ensures you can actually apply that knowledge in practice! Damn Canada, so awesome eh? Woohoo 2000 posts, and I use up my 2000th post talking up Canada? Bloody usurping Canada, I should have been up talking us New Zealanders ... but I can't hate on Canada too much, being our northern cousin and all.

  22. Can you go to Ottawa and tell the Ministry of Health and Long Term Care that the Symptom Relief Package designed in 1995 is no longer appropriate and given the high level of education for Primary Care Paramedic in Ontario there should be a sensible increase in scope of practice? Thanks!

This is exactly how many places in the world (AU, NZ, SA) are configuring their systems, combined with an aggressive approach to up-educating and upskilling their non "ALS" providers so that the top-tier ALS people are kept free to see more crook or potentially crook people to maximise use of their specialised knowledge and skill set. The OPALS study tells us what we already know or have long suspected 1. No difference in cardiac arrest survival when ALS is included, 2. No difference in major trauma survival when ALS is included, 3. Decreased morbidity and mortality in respiratory distress patients with ALS intervention, and 4. Decreased morbidity and mortality in chest pain/acute coronary syndrome patients with ALS intervention
