  3. Bro, just take the USMLE steps you need (I think only 3 but might be 2 and 3) and practice medicine In saying that, medicine means your speciality i.e. general or family medicine, are you a Consultant Physician or still a Registrar?

  9. What has she eaten recently? Let's pop her on some O2 and get a 12 lead ECG before we do anything else Standard alphanumeric soup dine in please, we are in no hurry - RR, BP. HR, EtCO2, SpO2, temp What are her activities of daily living like? What was she in the hospital for? Who is looking after her medically wise, does she have a GP, can we give him a quick call and have a chat? What are her wishes regarding medical care and transport?
  10. This is almost the worst case of fail I have ever seen! ... and it gets stronger. You do realise "BLS" and "ALS" are not actual things right, you know, things that mean something? They are backwards nonsensical words that mean nothing and are foreign to medicine yet are held so dear by the Americans as if they are some sort of Biblically inspired concept? Go do some research, trauma is an increasingly non surgical disease.

Rubbish, I have only seen one bloke have a bad trip on ketamine ... none of us in AU, EU or NZ seem to think its a problem And if your people can't be trusted with ketamine because they'll nick down to the local rave with it, party up large and die of water intoxication or something silly like that then you should find new employees.

No evidence of trauma - i.e. wasn't traded for the mailbox by a bunch of baseball bat carrying teens or no Koala bear marks? I am thinking either cardiac e.g. bradycardia/hypotension, metabolic e.g. hypoglycaemia/DKA or neurogenic e.g. stroke/TIA
  15. 1:100,000 adrenaline IV as an infusion (1mg in 1 litre of 0.9% NaCl) run at 2gtt/s titrate Agree that an EtCO2 of 32 will tell you not a lot about his PCO2 but it may be slightly high I'd be ringing up for an Intensive Care Paramedic to rv with us and taking this kid to the hospital without waiting for them to come to me ... with much of the expediciousness
  16. Killing patients again mate? I had a quick squiz at the book and it seems the Lifepak 12 can be configured to remain in sync mode or return to manual (async) following a sync shock it seems. My personal preference would be for the defib to return to async mode, less chance of error because we both know AOs are taught to select ON, ENERGY, CHARGE, SYNC, SHOCK so you get used to doing it, so if you have to do it again to re-zap them (medikal term right there...) you do the same thing again and not shock them in async mode. Oh PS where the bloody hell are my sedatives ....
  17. For cardiac arrest we use 1:10,000 adrenaline or for everything else we use 1:100,000 adrenaline
  18. Is this the same in all of Germany? Brown thought Germany had EMS Doctors (Notarzt-Einsatz-Fahrzeug) and that they were the only ones authorised to do invasive procedures? Oh, das Germany gut ja
  19. For now, Ambulance Officers are "authorised" to practice. When Paramedic and Intensive Care Paramedic become restricted health professions (soon) they will be registered and must hold an annual practising certificate

  22. In the setting of cardiac arrest I don't see the point in intubating the patient really, some still do here but thats more old dog syndrome. Intubation, in particular RSI, is something we should not get rid of from the Paramedic box of tricks but we must use it wisely.
  23. Intubation is a waste of time, nobody in cardiac arrest ever died from not having an ET shoved down thier gob Put in an LMA or King-LT and you're set
  24. For crew who work 8.5 hours (black/yellow watch) they are paid for 8 hours (thirty minute lunch break unpaid) and for the crews who work 12 hours (red/brown and blue/green watch) they are paid for 11.5 hours with one of the two thirty minute breaks being unpaid
