I agree with you dust, except what lesson is learned by the losing team when the coach forfeits the game ? "Quit so you dont embarass yourself ?" The real answer is that this sport should have a rule like little league baseball/softball has that ends the game when you get too far behind. I do not fault the winninng coach for continuing to play, but according to the story that coach showed poor sportsmanship when his team scored. -

You asked what would i suggest for someone who is new to driving any vehicle (lived near subways). I would say get a couple of your classmates together and go rent a Uhaul box truck (smaller one), and drive it around town and in small streets. That will get you used to driving something of that size in relation to other traffic, low limbs, narrow streets, etc. I would also take it to an abandoned business that has a good size parking lot, and practice backing it into parking spaces, or beside poles. It is not the same as driving an ambulance, but it is much closer than practicing with a pick-up or car. http://www.uhaul.com/guide/index.aspx?equipment=truck-17 This truck is like $30.00 and 1.00 per mile. Split it up, cheaper and more effective than EVOC. -

So what you are saying is that if you were transporting a "stable" back pain patient, and you rolled up on an MVC with entrapment and ejected patients, you would just roll on by ?

The tactic of not hiring firefighters and then sending them to an academy or school on the department's dime. I dont see why they should be any different from EMS, I had to pay for mine, they can pay for theirs. -

My guess is you have never actually started an IO in a real patient and attempted a fluid bolus. If you had, you would know that pushing D50 would nearly be impossible. The risk of infection, fracture/cracking of the bone, and or tissue infiltration (if you dont do it right, which is highly possible in a skill you do once or twice per year -- remember D50 has a necrotic issue) are far higher than that of puncturing bowel with an ETT, and even if you did, it is rarely life threatening in the last 6 inches of bowel. You have to put your patient above ego.

It is not abandonment, you will have to find another way to get your partner fired. And by definition this was an MCI - "a call that overwhelms your resources". Both patients were transported, no patient was abandoned. Please read this article from JEMS: http://www.jems.com/news_and_articles/colu...E8A06F81C5179EA Wendy, you should also read this column regarding not stopping at a critical incident (i agree that you should not stop while transporting a critical patient to check on the walking wounded at a minor MVC )

I believe most of the UFO sightings are just future military craft that we have never seen. I have never seen a UFO or an alien, but at the same time I think it is arrogant to believe that we are the only form of life in the whole universe (especially since we do not even know the boundaries of the universe, new galaxies are routinely discovered). So how about some of you out there in UFO-Central (midwest US), have you ever seen one ?

No it is not abandonment. The patient was in sight of the original medic, and you were in the vehicle with the patient (you should have gotten in the back with the original patient). Both patients were transported, so there is no abandonment. If this were abandonment, you would never be able to work an MCI. The first crew on the scene of any MCI, has to leave some patients intermittently to assess and treat other patients, until additional help arrives. It would be no different than if you had driven up on an MVC with no one on the scene.

They were wrong. It was verified by 911 that a live person had called for help, they should have verified that no one was home. Had it just been an "alarm" that went off, with no verification of a person being at home, they might have had an arguement. Better to have to replace a $100.00 window for someone who wasnt home, versus what they spent in legal fees to defend the case (they were lucky that the jury didnt award damages). Cynic, you can not write a protocol for everything you encounter in EMS, sometimes you have to think on your own. -

Sounds like the typical EMS attitude. We want to do more "stuff" but dont want to do the necessary training (lazy). Paramedics want to do RSI and thrombolytics (without showing proficiency in inubation or the ability to read a 12Lead). EMTBs complete their "first-aid" training, and then think they should be able to start IVs and push meds. There should only be three levels of EMS certification: EMT I Paramedic Advanced Paramedic (flightmedic or MICU like training), would be allowed to do RSI and push more meds. -

Dont beat yourself up, at worst she got an unnecessary helicopter bill, and a diagnosis of pneumonia instead of MI. A little voice in your head (or other part of your body) cautioned you, I always trust that voice, even when everyone looks at me like i have a penis growing out of my forehead. The rhythm is a simple BBB, which she may have had for 20 years or acquired today (was she on Lanoxin ?). It will be interesting to see what her final diagnosis was, but regardless, you did your job. One day we will have lab and xray capability on the truck, until then you have to trust your intelligence and your gut. -

Go ahead and complain to the politicians, who will call the Chief to answer the complaint. I am sure he will pick up the phone and call you for employment the very next day. NOT !!! That is like saying I would love to come to your superbowl party and eat chicken wings and drink all your booze, but I will not come because you asked me to bring some ice and a 2 liter. I imagine many employers will start employing this tactic, as it is expensive to send these folks to school. Every fire department has about 400 applicants for every vacant position, so I do not blame them for saying, "you pay for your own school". The reason they want you to be fire/ems is so that they do not have to pay you overtime. Again, the Chief sounds like he is running his department on a tight budget, like most are, and is making wise decisions. -
And I am sure your patient will appreciate you drilling into bone and giving them osteomyelitis, because you think the anus is yuck !

the one thing i did with all new EMTs was to place them on a spine board (sheet between them and the board), and strap them to the stretcher (no cspine), and then i proceeded to drive them around the block. I never broke 35mph, but i hit the brakes hard, i accelerated too fast, hit a couple of curbs, and took curves too fast. Another technique is to set a FULL cup of water on a flat surface somewhere in the truck, and tell them to drive without spilling it. -

I have no problem with the other meds listed. I was aiming more at those that have NO protocol for treating hypertension in the absence of any other symptom. Cant tell you how many times EMS brought a patient to the ER with B/P of 280/140 or higher with no treatment, because they didnt have a protocol. And I am not talking about people who have a pressure of like 200/100 --- but in the extremes above that, we should be treating.

The american business market during the 80s-90s. It is why your economy is in the shape that it is now. Profits at any costs have destroyed your companies but made your CEO's and shareholders millionaires, while your dad probably had to retrain for a new career at age 50. Anytime, you are doing something at the expense of someone else, you really need to think about what you are achieving. It is like a prize-fighter delivering the brain-damaging blow to an opponent that is already "out-on-his-feet". You win the fight, but put a man in a coma, what have you won ? -
Funny, I remember all those patients I treated with a squirt of procardia under the tongue, not a single complication. I think the pointy-headed ACLS faculty have overthought this one.
