  3. Problem here is that OSHA here doesn't require us to have hearing protection. Also, what about the patient? For those agencies that have hearing protection for the crew, is there any for the patient as well?
  4. A few years ago I conducted an evaluation of various ambulances to determine if hearing loss could be a risk for paramedics. It's a very real possibility, not from the siren, but simply the ambient noise during travel. A brand new ambulance with zero equipment in it was just below the threshold for long term exposure. However, an equipped ambulance that had over 60,000km on it would be in the low 90 dbs. With the siren on it didn't change much unless the siren was a roof mount like back in the 1980s. Looking at this chart, exposure to 90db for 2 hours can cause hearing loss. When I travel at highway speeds with my ambulance, we are well over that threshold for 4 - 5 hours. I wanted to conduct actual evaluations over a 5 year period with rural medics, urban medics, and the standard norms but I couldn't find an audiologist to assist me.   6. Actually, you have it backwards. A torn limb will tend to bleed more than a severed limb because the arteries are unable to quickly constrict or develop clots as well. In a clean cut the artery has the ability to constrict immediately and will often withdraw into surrounding tissue because of its elastic nature. A jagged and torn blood vessel is unable to do that as effectively. However, this is simply a short term problem because the body will also respond with vasoconstriction in the global sense and restrict blood flow to all of the limbs, not just the damaged one.

  10. I don't see this being used until the percentages become higher. All it's good for now is to confirm a bleed, but there is too much room yet for a false negative and missed deeper or smaller bleeds. 3.5 cm isn't very far into the head. Until it's more reliable to rule out the presence of a bleed I can't see it being used to treat stroke. Also, at a cost of 19k for a device, I can't justify such a purchase when it'll make no difference in the treatment we provide. I'll keep watching though. STEMI treatment is becoming more and more common in the field, with TNK, Plavix, Heparin, and more being added to our arsenals.
  11. Well, not quite, but this seems interesting. As the 12 lead did for STEMI treatment in the field, do you think this could eventually be used for treating a stroke in the ambulance as well? I have a message into the company regarding the cost, but has anyone here seen or used one?

  18. I've been researching both Tranexemic Acid and QuikClot and I'd prefer tranexemic Acid. The data supports the drug far better than the dressing. However, now that the risk of burns has been eliminated from the dressing, one could use both!!
  19. I'm still perplexed. If I recall, there were 2 unexploded devices but the video images only indicate the boys carrying one bag each. That means there must be someone else involved, doesn't it?
