  1. Kate, he's young, he thinks that everyone retires with 10 million in the bank and the means to travel anywhere they want with no effort. If that was what the OP actually wanted, I believe his question would have been more along the lines of "I'm recently retired and want to see the world, but where can I go with the crap pension I have?"
  2. Sigh, here we go again. Tell you what, you have so many questions about being an EMT, why don't you go to the Hospital and discuss their education program and EMS system with them. Coming in here and asking us about it is a waste of your time because you have practitioners in here from all over the world and you haven't even narrowed down what country you're asking about let alone a specific region. The answers you've already gotten here are all over the map and haven't solved anything for you, and now, when people start providing some constructive criticism you get all pouty and defensive. That crap don't float here boy. You're obviously only about 15 or 16 years old and interested in this as a career, we get it, but if you want to be treated with respect you're going to have to earn it, because we see guys like you come and go around here all the time. The option is yours, choose to learn from the people here who are actually trying to help you and taking time from their lives to answer your questions to the best of their ability, not pick at you, or go hang out at EMTlife.com.   4. No, but his lawyer will have a great time with your spelling and grammar after your patient takes you to court for some perceived wrongdoing. Many EMT classes place a minimum educational requirement because they want to make sure their students can spell "Hospital"
  5. Nope, you also have to be tested on scenarios. Usually one medical and one trauma.
  6. Depends where you are, in some locations the minimum required training for a driver is First Aid/CPR. Most jurisdictions would require EMT minimum. Sometimes the paramedic will get to drive. The majority of ambulances are automatic. There are very few standards.

  9. But would we want any positive airway pressure if we can't rule out a pneumo? I'm not really comfortable providing any treatments based on what someone else told me the X-ray or Labs said. Problem is, I suspect sepsis and a likely pulmonary effusion as a result, but I can't confirm it. Her temp is just below our threshold, her heart rate isn't high enough to qualify, but with beta blockers it wouldn't go up now would it....Otherwise she fits the criteria. Bilateral IVs, titrated to maintain her systolic between 90 -100. If I can't get the BP up I'll consult with my doc to start the norEpi, if the temp goes up any higher I'll also start the antibiotics provided she has no allergies. Acetaminophen 325 PO if the temp exceeds 40C. Continue monitoring lung sounds closely in case a pneumo develops. Oh, forgot my new portable ultrasound. I'll check her over with that.

  11. I'm jumping on the Kiwi bandwagon. I'd almost...not quite, but almost consider CPAP. Monitor lung sounds closely, but with the increased temp and dercreased BP, I'd consider a norepi and vancomycin infusion. Any chance you can call ahead to the receiving hospital and ask for the labs from them? That info would really help right now.
  12. I concur, the Classic II is what I've been using for decades. It provides clear, crisp sounds while being relatively affordable. also, the addition of a cozy will help reduce extraneous noise. The cardiology scopes are more expensive for no real gain.

  19. Mike, when you go to bed tonight, take a few moments to lay flat on your back and look at your bare belly. Let me know if you notice anything interesting. Doing a thorough abd exam is certainly important, but palpating without knowing what you're palpating for is like driving through a dark tunnel with no headlights. What will you do if you confirm Appendicitis? AAA? Cholecystitis? Hepatomegaly? UTI? Full Bladder? Chrons? GI Bleed? Bowel Obstruction? What does the doctor do in the ER once you're done all your poking and prodding? Take your word for it that it's a gall stone and schedule surgery? We can determine most of those abdominal pains without poking the belly by conducting a proper history.
  20. I never deep palpate the abdomen if there is a complaint of pain. If they have abd pain, what would palpating it reveal? They'd guard against your palpation, so it'll be rigid and it'll just hurt them more. Why would I want to intentionally cause greater pain to my patient? Light palpation might reveal a AAA, but so could visualisation.
  21. Increased Pain with cough? Pleural Effusion secondary to pericarditis. However, since I don't have an x-ray or labs to look at myself I'm just gonna monitor her condition and transport, treating complications as they arise.

  23. I wanna see the X-Ray and the labs, if they aren't on paper then I wanna see on the computer. Even just to snap a pic with me smart phone to look at once I complete my assessment. Temp? Which 4 ribs? Any Sub-Q signs? Smack the docs for letting her have beta blockers if she has S/S before she got her meds. I think it's Pericarditis, but the X-ray would help.
