I am 100% with Beebs on this. I've seen medical and nursing staff giving judicious amounts of morphine 1-2mg in cases where I'd start at 5 and give another 5 if the pain persists. I've gotten the patient to my ambulance for an interfacility transport several times who are still having pain and I take care of it until they can tolerate it. Then I'll stay on top of it to prevent breakthrough. As long as they're still breathing they'll get pain meds, and I don't care if their pressure is 90/50, they'll still get pain relief...just a little more cautiously.

Not aware of any studies, but I can throw an observation at ya. I see no reason to have called the police in your circumstances either. i deal with intoxicated people all the time and never have the need to call. -

That's a bargain. When I bought my LP12 10 years ago it was all decked out with 12 lead, trending, NIBP, etCO2, SPO2, modem, pacing, AED, and more. The only things on the list it didn't have were the orange/black display and the two invasive monitoring channels. $42,672.29 -

Yup, definitely possible. As Vorenus mentioned, there's a variant of the LP12 that was not 12 lead capable and typically AED. I have no doubt that there is a model of LP15 that is not 12 lead capable either. It's substantially less costly than the 12/15 lead enabled machines. WHen the LP12 first came out IIRC the 12 lead was a $12,000.00 option. Edit: Did the doc at least hand you a towel to wipe the egg from your face? -
It relates to the way that each blood pressure is detected. In the case of a manual pressure, you are auscultating the korotkoff sounds of the artery as pressure is released. When the cuff is re-inflated it can result in a false lower reading. It's kinda like blowing up a nice new balloon. The very first time it's rather difficult and requires greater pressure to inflate, however, after letting the air out it takes much less pressure to inflate the balloon. The artery typically needs a few minutes to regain the resistance and resilience it previously had. Oscillometric devices measure the amplitude of pressure changes in the occluding cuff as the cuff is deflated from above systolic pressure. The amplitude suddenly increases as the pulse breaks through the occlusion in the artery. The pressure at which this occurs is very close to the systolic pressure. As the cuff pressure is decreased further, the pulsations increase in amplitude, reach a maximum (which approximates to the mean pressure), and then diminish rapidly. The index of diastolic pressure is taken where this rapid transition begins. Because the oscillometric BP measures when the blood breaks through the occlusion rather than the sound of the pulse against the artery it tends to be more reliable when re-inflating. However, your preceptor was nit picking because the variance in blood pressure cause by re-inflating the cuff is really not that significant.

Considering Mama was on her way home from school at the time I wouldn't consider her a freeloader. Sounds like a case of burnout after many years on the job....he finally just kinda "snapped". I've been guilty of similar over reactions in the past...following which I would realise that maybe i was just a bit overboard.

I dunno, those handles look a little flimsy. I think I'll stick with my 20 year old tried and true method of using the scoop straps to lift with. More robust and probably cheaper too.

Yes. Try to keep the communication line between you and your client. Having both of you rapid firing questions at him would tend to cause confusion and something will get missed. You will also be distracted from doing your assessment because you're trying to hear the SAMPLE history. You may also want to develop short term memory skills that will help you to remember your client's history in case your partner isn't writing it down for you. He may be off talking to bystanders and getting a history from them, etc. To control the scene you need to have that rapport with your patient, your partner shouldn't confuse the issue. I am assuming that you were following the ITLS standard assessment when doing this scenario. At what point is the SAMPLE history taken?
