I always have been a proponent of cameras in the unit. Mine all have patient compartment cameras with recording devices but no video output. The digital card is a 16mb and can hold about 120 hours of video, the camera only comes on when there is power to the module, thus only when a patient is on board. The only time it will ever be referenced is if there is a specific complaint registered. We have had two complaints in the last few years, but were rescinded when the plaintiffs were informed about the video system because both accusations were false. The State of Maryland minimum age is 16. I was certified when I was 17, the youngest ever in Sask. Shortly after that they mandated 18 y/o and a complete grade 12 education. I have no idea why they did that though.

Shrug, I did my first ambulance call when I was 11, but that's a whole different story. It sounds as though your partner would fit well with Queensland EMS and the bullies over there. Sounds to me that he is bullying you and it's time you stood up for yourself. Tell him that if he's interested in starting a fight then he should pull over so the two of you can go at it, if he does, run like hell. I suspect he won't call your bluff though, verbally abusive bullies are like that. Either way, stand up for yourself...ask him where his facts are. Ask if these are facts or his feelings. Tell him to get a new partner if he doesn't like you. I know that he is intimidated by you because you know more theory than he does, but don't forget, he also know some of that information is a waste out on the streets, and he knows the "Real" EMS job better than you. (His likely attitude)

I figured you were going this way after that last one. No Need to amputate. At least if I am envisioning this in my head properly. Header Augers are surprisingly flimsy, with proper leverage it can be bent up with a pry bar releasing the individual from that area, then the feeder belt can be cut and the pulley turned in reverse to permit the legs to come back out. Now....if you were to change your scenario so that the farmer was unloading the combine and went into the tank to kick grain down into the auger and slipped...you might have a more difficult extrication, but by that time, it's no longer an amputation for EMS to do, the auger has already done it for you. Here's a good one. Farmer drives an old john Deere where the PTO shaft runs out of the engine and under a cover right beside the seat rather than out the back of the tractor. Mr. farmer removed the cover sometime long ago and knows that he can not step there. Mr. Farmer steps there and his leg wraps around the shaft up to his knee 3 or 4 times. All that is holding the lower leg on is flap of skin 3 - 4 inches across and some badly traumatised muscle tissue. The tractor is shut off and connected to a log splitter so you can not slide him from the shaft, nor can you unwrap his leg. The farmer is standing on the ground at the back of the tractor with his left leg entangled in the PTO shaft which is about waist height. It is mid winter in sub zero temps and he is heavily dressed, conscious, alert, very pale and feeling week. The leg does not appear to be actively bleeding aside from some occasional droplets. Whoops, sorry...one scenario at a time. Back to the combine

RuffEMS...just wondering...do you actually know all that much about combines and how they are constructed? I don't ask to be facetious, I ask because it would be a relatively simple extrication that could be completed long before Fire arrives. I work in a rural agricultural setting, the scenario you describe is not all that dissimilar from my experiences. Is sounds as though you are describng him trapped under the header auger and pulled into the feeder belt, is this correct? If so, how far into the feeder has he gone?

We contact the coroner and ask the family which funeral home they want to come get the body.

I've been in EMS for 30 years. It seems to me that those who would do it have some sort of military background from what I can tell.

A paramedic on scene able to treat the ailment in 5 minutes, then transport in 15 minutes, or having to wait for the paramedic in the ambulance to show up in 15 minutes and have no preceding interventions?

That's just poor documenting, not because of the type of vehicle used. Besides, the Pickup truck was an official El Paso FD vehicle from what I understand. Not someone's POV.

Isn't that just the same thing as a "fly car" or "rapid response unit", supervisor car, what ever you want to call it? I see them everywhere, I even used to have one. A medic driving around in a GMC Yukon or a pickup with a topper, some even in cars. They respond to the scene and treat the patient as they wait for the ambulance to arrive and transport. This doesn't sound any different to me, they were forced to take the ambulance out of service, but they still had medics to respond and treat the patient until transport arrived. It's not like they were transporting in the pickup, even so, out here we are allowed to transport a patient in a properly equipped "Alternate Emergency Vehicle" but only to the nearest access point for an ambulance. I've had to use my Dodge Durango for that purpose a few times to access areas the ambulance could not. Heck, if I had to, I would transport in the pickup truck to an access point, what other option would there be?

Impressive posting Kaisu. You sound like a medic who learns exceptionally well from her experiences and is very much open to taking the advice of those whom are asked. You're wrong. Although that may be the case in your area, In Saskatchewan the protocols are the maximum allowed procedures for the practitioner of the applicable level. Functioning beyond protocol is the same as beyond scope. Hope this might help avoid some misunderstanding.

It's a tough call and everyone screwed up. That is a rare occurrence but try to bounce back as best you can and learn from your mistake. I will never push any med I did not prep myself or at the very least have the individual who did prep it show me the container the medication came in so I can verify it myself. remember the five rights of drug administration: Right Drug Right Dose Right route Right patient Right Time You can't verify that it was the right drug if you didn't see what it was.

OMFG!!! DUDE!!! You just said that in your previous post!! Is it a compulsion for you to argue, even with yourself??!! I am completely stunned!! I have never, ever seen someone turn on themself so quickly...even a delusional psych patient has the common sense to argue with a hallucination. Man, this is just too much. There is cooking by the book, then there is thinking you can cook the turkey faster by cranking the oven up another 100 degrees.
