I always give 12.5, you can always give more if needed. When you give it, push it extremely slowly. I've never had anyone say anything about "burning" when ever I've given it very slowly. However, I did watch a nurse "slam" it and then laughed after the little old lady slaped her across the face. Turned out that the little old lady was a former nurse herself. I don't know the exact age that is required, but if you give it to a child who is to young it will cause them to freak out.

I've came across a few people who have only done the compressions and didn't bother with the airway since they didn't have a barrier. If this is accepted, then I wouldn't worry to much.

As far as the buckle straps on the long board, ETMC uses them. :shock: What ever happened to gold webbing, people look at me funny when I tell no we are going to web. I prefer webbing, if you've ever had to deal with any kind of rope rescue; webbing is the only way to go. Webbing can be done by two people in under a minute, it just takes practice. At both of the services I work for we carry spider straps and webbing. The spider straps are good for the "Grandma was found in the middle of the floor and has no pain anywhere calls," but webbing is what I prefer. -

The problem thus lies how do we determine what is a true heart attack and what is just gastric reflex, that way we aren't over loading the few true cardiac hospitals. Our other option would be to make every hospital a cardiac style hospital (isn't as expensive as everyone thinks). Take for instance, Bay Shore is the local hospital that we transport too. It does have everything needed to treat AMI, and pretty extensive cardaic service ability; however there is a hospital out where I live that doesn't even have a cath lab. I guess we are lucky here in Houston, almost every hospital with the exception of maybe two or three can handle an AMI with no problem. -
No it was the other one...good ole Ben Taub General Hospital. :wink: Since Baylor pulled out it is all UT residents, so the problems with Hermann have spread over there. BTW, what the hell is up with Hermann's nurses being really cold and crabby? I remember when they use to actually enjoy their job, now most of them seem like they would rather be somewhere else. Granted every time I go in there I see new faces all of the time.

Heavens no, I've never been to court for an EMS related incident. It isn't that big of a deal, just be glad it isn't you that is behind that big oak desk. :shock: Only tell facts, if you can't recall something that is no big deal; it is better to state that you are unsure about something then to go with your "hunch" and then be proven wrong later on.

Texaco, I thought they only made motor oil and gas? :shock: At Bay Star we have Valium, Versed, and Toradol. At City of South Houston we have Valium and Nubian as our only controlled substances. It would be nice to gain morphine at both services for the cardiac (chest paint/CHF) aspect as well as pain. Now I have another question, since Nubian is a synthetic morphine, what is keeping it from being used in cardiac chest pain or CHF?

I don't believe that method is recgonized. We use spider straps and webbing, with webbing being used on anything that is remotley major and spider straps being used for the "c-spine percaution" type calls. Both are accepted, and you can't get much quicker then spider straps and still have a patient truly immoblized IMHO. Why is that so many people are to lazy to web these days? -
Nubain (nalbuphine) is a synthetic version of morphine. It has similar interactions and side effects, however it is a huge no-no in suspected head injuries. Not that many services carry it, we do because it is cheaper then morphine and we don't use that many narcotics.

Maybe a "general information" section should be made just above this one so people could reference the various previous questions that have been asked. We would delete the "useless" posts in the question so it is just the question, and the location to the answer. That way even if they couldn't use the search feature, they could at least "browse" the various topics a little easier.

Background... Responded to a call of a 44 year old female who was involved in an auto-ped that was a hit and run. AOSTF said patient laying supine c/o left sided pain and bilateral leg pain. Patient was found to have road rash all over, fx humerus, and open Fax of the radius/ulna all on the left side. Patients husband reports patient has had 12 beers (she says it was probably more), and no one really knows how she got out there. Patient states she walked out of beer and then woke up where she is now. Patient also states she doesn't remember what I was talking about when I asked her to remember a simple word. She had an abdomen that was soft, distended about a good 6 inches. Patient takes no medications, is allergic to no known drugs/foods, and has a past medical history of a hysterectomy one year ago. Witnesses state patient walked out of bar, crossed two lanes of traffic and a turning lane and was struck by a car. I could only get an 18g in her RAC, she was stable (for now) and the bleeding was controled. Her pain was still a 10, but she admits that it was lower after we splinted the fx bones. I did not give any nubain because things just weren't "adding up." When I got to the hospital, one of the residents ordered 4mg of Morphine, then told me I was ignorant for not giving anything for pain. Well one of the staff doctors walked in, saw the order, and then jumped all over his butt for giving morphine to someone who had 12 + beers and her blood alki level was unkown. Question... How many services address the use of pain medications patients with suspected ETOH use? One of the contraindications for Nubain is ETOH. Would you have given her the Nubain or left that for the ER to handle? Keep in mind that we have no other findings with the abdomen other then it is distended.

Last time I checked there wasn't a county in this country that had the power to control clinical times for an EMT class. That would be regulated by the state health department, are you sure your not talking about a first responder course (which is known throughout the country to not require clinical time). Not having the requiirement for clinicals about a good idea as teaching someone how to build a car without ever touching one.

I work in the heart of the city, when we do have down time we take care of our station duties. Some work on homework and study for the NR test as they advance levels. Surf the internet, run errands, play footbal, PS2 > *, take really long sh*ts and read the paper (this isn't a group activity), sleep, watch movies, just about anything. Today I went to the driving range for two and half hours. :wink:
