  3. I am not exactly sure what you are looking to develop. Skill sheets that are different than the ones already used for training? To remove personalities and possible bias you might consider skill sheets where the skill to be proven and/or scenario has no variables. You can check the NREMT skill sheets for examples. That is what I base our evaluations on. Some guys have developed their own skill sheets to meet particular equipment or local needs. EDIT: Programs I have been involved in did not particularly "test" students. The proctoring were more was focused on some overall abilities that students had already been tested on either by their school or NREMT. Students who could not pass their theoretical and skills portions of their course could not qualify for student ride outs. The "working evaluation" was more general pointed toward other aspects like attitude, willingness to follow orders, and generally how the student handled himself in a real work environment.

  5. This narrative is probably going to wrap up this scenario pretty quick. The wife whose name is Sherri is home on leave from the military. She has only been home a few hours and has been looking forward to spending some quality time with her hubby Dan. Dan has been anxiously waiting for her to return all week. He was never really comfortable with her joining the military but has always loved her and respects her decisions. Ever since his truck accident 5 years ago, Sherri's absence is almost unbearable. Dan was hurrying home after a long day at work because Sherri was returning from a tour of duty in Iraq when he fell asleep at the wheel and rolled his truck. After a prolonged recovery his only permanent injury is a T-3 spinal cord injury and the amputation of his left leg. Sherri still has nightmares about Iraq but dan works hard at understanding and helping her. In preparation for his honey's arrival Dan chills a bottle of Merlot (Sherri's favorite) and mows the lawn. He really wishes he cold trim the hedges but they are just a little out of his reach. After a dose of L-arginine and a quick trip to the gym he comes home, He is grateful for the good health he has enjoyed despite his injury. takes a shower and removes his Foley and takes a Viagra. Sherri arrives home at around 2100 hrs. Takes a shower and gets dolled up while Dan has a couple of drinks and cleans her weapon for her. They always enjoy going to the range together and he is looking forward to spending some time shooting with his wife. Sherri doesn't even have time to finish her glass of wine before Dan entices her into down the hall and into marital bliss. As they begin to have intercourse the crisis begins. It was the craziest feeling he had ever had . The blotches are of sudden onset and cover most of his body but seem worse in chest and shoulders. He is in relatively good health in spite of his T-3 injury and amputation. Works out regularly and is in great cardio shape. No chest pain. Sinus Bradicardia at a rate of 40 Lungs are clear. HE took pain meds for a while but has discontinued for about 4 years now. Occasional Advil or Tylenol but not today. Dan does not take antidepressants. He shared some of Sherri's Lexapro for a while about three years ago but swapped them for hard workouts. Dan does not take beta blockers or statins He is really anxious and short of breath. As you work over him he gasp "Please help me, I feel like I am going to die" Do you think the L-Arginine could potentate the effects of Viagra? EDIT Spelling

  8. The way each of us deal with death is defined inevitably by our country, culture, experiences, raising and spiritual bent. For most of us the realities of death are shielded from our lives as we grow up. In the United States when grandpaw passes away the funeral home takes care of everything, by the time we get to see the open casket, many times days have passed and the body looks better than the last of their living days. In a lot of ways this shielding of the harshness of death makes the the people we see die more dramatic and traumatizing. The Aztec people have a very calm acceptance of death. When a person dies in the village the family cares for the corpse. Family members cram cotton balls in their loved ones orifices, bathe and dress the body and lay them either on a bed or often times the only table in the one room hut while the diseased is measured and the pine box casket is made. Often times the body will lay on the table for hours until the casket arrives. In the sub-tropical heat of the area, and due to the absence of embalming, bodies must be buried within 24 hours or fluids will begin to leak from the slits in the rudimentary casket. A rooster will be tied under the casket. As friends and family arrive for the wake the other families of the village will show up with a chicken, coffee, wood for the fire or tortillas so that everyone that arrives gets to eat in the yard right outside. From the time of death till interment, the family sits with, cares for and pray over their diseased loved on. The new generation, that have spent time away in the city, will break into wailing and tears as the casket is carried from the home to the village graveyard. The old timers are sad but composed. As they leave the house someone will kill the rooster. Nobody really knows why but a very old man told me many years ago that they kill the rooster so he can crow at the gates of heaven to announce the arrival of their friend. The casket is lowered by the men of the village into a grave that they all dug together. Everyone walks by and tosses at least a hand full of dirt into the hole. When you have seen death in this light for a lifetime, it takes on a different light. Death doesn't bother me. Suffering breaks my heart.

As you enter the room you can see in the light from a lamp on the nightstand a find a clean and well kept bedroom with a middle aged white male laying on a king sized bed. He is covered with a sheet from the waist down. From the bulges, or lack of you can see that he is a left leg amputee. A beam of light is shining through a slightly opened door to an adjacent bathroom. His heavily muscled chest and shoulders are covered in sweat .... The patient is restless and nauseous with a pounding headache. His face is flushed, his vision is blurred. The patient presents nasal congestion. His skin presents red blotches and goose bumps. SPO2 94 BP 240/138 Pulse 40 Temp 99 degrees Fahrenheit Medications Bayer Aspirin .81 mg, Viagra .25mg, L-arginine 6 grams
  10. So the police arrive and do a walk through. the patient is not armed and the only visible guns are the ones already mentioned. The wife makes no verbal objection, only an exasperated look, to placing them in a gun cabinet that is against the wall beside the TV. "Please, please help my husband" she says. The scene is clear.
  11. No worries about your English. I have been trying to learn how to pronounce Rettungassistent since forever. In the US contacting Medical Direction is a common practice but not by means of video or streaming feed. What you are describing is more like having a Physician in the ambulance with you. Watching and commenting. It seems that you would not so much be his eyes or ears but more so his hands. So you would consider that it has had a positive effect on patient care?
  12. I would love to participate but I am so AWESOME it would be a shoe in! All joking aside, the scheduling is bad for me. So at least as far as I am concerned I am in no way blowing off the offer. I just would hate to compete and not be able to fulfill the commitment. I would most definitely be competing if I thought I had a chance. I never met Dust Devil but can tell that he was dearly loved and respected by many here. The love that this scholarship is offered with is not lost. I encourage anyone that could work it into their schedule to jump on this opportunity. So, everybody start writing about how AWESOME you are and submit.

  15. I had to chuckle at the redneck riviera. That is funny. The wife seems genuinely worried and does not seem to be intoxicated. She was clear and direct although she turned quickly as to lead us through the house.

  19. The wife made the call. Here is the portion that csboulder11 saw and was edited out. Sorry I screwed you with your post. Feeling a little guilty. You are received at the door by a beautiful middle aged woman in a bathrobe whos concerned continence belies her hair and make-up that are perfectly made up. You also notice pendulous earrings and a necklace that looks like what would be worn to a party. With urgency in her voice she says “please hurry, I have never seen him like this before” “Something is very wrong”. As she leads you through the living room, you notice an open bottle of Merlot on the coffee table and a wineglass that is two thirds empty. On the end table by the recliner there is a half empty bottle of Jim Beam next to a scotch glass that only contains some partially melted ice cubes. The glass is on a coaster where a good sized water ring has formed. Next to the glass is a Desert Eagle laying on a wooden box that looks like a gun cleaning kit and a book titled “Living with PTSD, a soldier’s story”. In the corner there is an AR-15 equipped with a tactical scope and a sling. In the light of a single lamp you can see that the house is clean and well kept. As you follow her through the living room you sense a slight sweet smell of gardenia bath lotions. The smell reminds you of those Christmas visits to Bath and Body works for a last minute gift. The lady says “Please hurry, he is in the bedroom, just this way” She heads toward the hall … EDIT: Spelling

  22. I am gonna move slowly through the scenario because I think it will be more fun. We will have the answers to these questions in time. I don't know? How would we clear the scene?
  23. I have never experienced this in an prehospital setting but have observed surgery observed and directed by a surgeon in a different location. The operation was successful and with the intended results. I think that direct medical supervision could be good in the sense that patients get a better care. This should not be problematic from a legal standpoint since we already work under medical direction and as an extension of their practice. The downside that occurs to me, is that it could propitiate the deterioration of critical thinking by prehospital personnel. It is an interesting concept. I will be very interested in knowing what the results of your pilot program are, assuming that this is a pilot program.
