Welcome to the forum! If you are trying to get a taste of EMS you might want to talk to some of your airforce medics and possibly some of the local fire and EMS services to see if they will let you watch them work. I love EMS but it definitely isn't for everyone.

In my remote experience with medicine, for years we would use expired ophthalmology drops, local anesthesia, analgesics, antibiotics and corticosteroids. All with very positive results. The surgeon said that he had talked directly to the rep. from xxxxx pharmaceutical company and had been told off he record that if the eye drops were still clear they were still good.

Location means a lot. A call to a second class shag hotel means crack heads whores and pimps. But location is not everything. Location doesn't mean more than all the other factors of scene safety. There are many things we consider before entering a scene. Some neighborhoods have strict leash laws; others have pit bulls in every yard. Poor doesn't mean violent. Poor doesn't mean criminal. The OP states that "The residence looked dilapidated and shabby". What does that say? and the partner didn't like the location. I mean the partner might buy his pot from that house or know some other info he doesn't want to disclose. There are some areas that LE will not enter or patrol after dark so, at least for me location is important. I mean who really ever knows. I have to discipline myself to be safe because my inclination is to ignore the signs or to figure I can handle it. Safe is better although I often don't follow my own advice. I don’t follow my own advice because as Dwayne said, I know my area and can make a reasoned decision about people and how they live, react, and behave.

I get your drift. An OD does not necessarily mean that the bobbies should be called in. It of course could be granny ingesting too many paracetamols but it also could be a serious criminal environment. There is a big difference between being toned out to the nursing home and being toned out to the local brothel or low class shag hotel. Many scenes are neither and not so obvious. They are then evaluated as to the odds of the scene being dangerous. The EMS personnel are trained to intue a dangerous situation and react accordingly to the environment, condition and energy of the scene. We will pause short of the door or before descending from our unit and verbally say “scene safe” the accompanying EMTs will confirm “scene safe” from their vantage point. Then we move. I have a little saying I teach our new EMTs. If I have to choose between people weeping at my house and people weeping at theirs ... well scene security comes first for me.

I would hate for some delusional junkie, just out of prison, or with warrants for his arrest, to wake up from a drug induced stupor to see two uniformed people bending over his main squeeze, "Hey, leave my chica alone!" Bang, Bang! The last patient the medics ever see, the last call they ever attend, the last breath they ever take. Scene safety. Not an option.

I think the scenario at hand refers to extreme situations. I mean, really why would I Inject IV if I could get a line in? What I am saying is that I agree that it is not the norm but am curious as to it's viability when other options are not available. I know the popliteal vein is behind the knee. I only mention it because the original post referred to a patient with difficult venous access elsewhere. I persisted because another poster (I forget who) kept acting like it was weird and would try IO and then thought I was referring to an arterial line. I would not start there but was looking for medic’s opinion as to its usefulness in a difficult access situation. In addition to Moby’s comment on SL injections I have heard of medics giving a sub lingual Epi injection through the bottom of the jaw in cases of swollen protruding tongue I am not taking a position but simply noting that your reasons were anecdotal to say the least. Do you have any science to back up this position? I understand that not all medications produce phlebitis when properly diluted and injected. Your thoughts? Since you express Ghetto is an economic condition ( I would assume it referes to poverty) As a child growing up in the Piney Woods of East Texas we were so ghetto that we ate so many collard green sandwiches, mom had to wrap our feet in rags soaked in coil oil to keep the cut worms from eating us. Doesn't that sound ghetto?

You might be a redneck if you have ever waxed your legs with duct tape! This is redneck. You know it's too bad we are not neighbors, I would have enjoyed waiting for the ice cream truck with you. You might be ghetto if your odds of getting shot are grater than getting a job. This statement would be racist and inapropriate in any other setting but since flaming made this the "say anything you want and it doesn't matter thread", it is OK. A local charity would get people to pay them to place a purple toilet in front of your business. The bussiness owner would then have to pay to have it removed. It only woked until my buddy planted flowers in it. and tried to get the charity to buy it from him!! Rednecks.

I don't think I would mind the sublingual needle that much. I means they could inject right under my jaw. Seems preferable to violating my "exit only" orifice.
