Since Dwayne has come back from the bush & is probably one of our more frequent flyers. OK lets go off the rail in a slightly different track: How about airport screening where they make you go through a body scanner???? I don't fly so It won't affect me, but isn't it an X-ray scan? medical procedure????? Do You consent to that by wanting to get in an aluminum tube with engines for $$$ -

depends on what state you are in. Common decency would make one think you would stop and at least offer assistance until the agency having jurisdiction arrived on scene.

Initially I had the same thought Ruff: But the fact that it started 24 hrs earlier after a trip outside makes me wonder about other pathophysiology behind this call. Initial impression based on info given would point towards possible pneumonia or bronchial infection. We don't have a temp so would get that and go from there . With the vitals that are noted , she would definitely get a 12 lead and IV access along with ETCO and see how well she is actually moving air. My gut tells me she is a sick pt that needs transport. HX of pulmonary hypertension makes me a little edgy here. Don;t think I'm going to jump on giving breathing TX right off the bat, even though she is wheezy. Don't see an anti coagulant in her meds list that one would expect to see with A-fib HX.

Welcome to the City Swede: While I don't know how your educational requirements would match up to US standards, You could sent transcripts of your education process to the National Registry or the state EMS office where you would like to practice, & see if they will offer reciprocity. You never know until you try.

I'll add in another thought here. My wife is a med surg nurse and works second shift .When she goes in at 14:30 the ER will call up to the floor and tell them they have 6-8 pt's coming up for admission. Many of these have been in the ER all day and had tests, X-rays, Cat scans etc. They wait until the end of the shift to dump them and clear their plate downstairs, while jamming the med-surg floor with all the new intakes that have to be done at once. We took in a lady with stroke like symptoms at 3 AM last week. She was in Cat scan before we left the ER. My wife got her as a new admit for observation @ 8 PM that night after she sat in the ER all day & evening.

I know what you mean Doc. My wifes brother got KIA in Afghanistan on his sisters wedding anniversary. needless to say they don't celebrate on that day any longer. -

been there done that Mike. Call was for a woman that had fallen on driveway outside her home. First in crew was concerned as she had been unconscious , so they collared & boarded her . When I arrived on scene they were loading into ambulance. The Pt was a large lady and had on a full length LL Bean down coat. Topical exam showed me that besides head injury from smacking back of skull on pavement she had angular rotation of one leg with severe pain to hip/femur and was having difficulty breathing. Pulse was irregular and rapid. WELL HELL !!! They had done a very good job of packaging her and didn't remove the coat as they were outside in minus 10 f temps Don't have much choice here as I need to get cardiac monitor on and IV started:::: SOOOOO out came the shears and it was like a chicken farm in the back of the truck. Got everything accomplished during the 1 1/2 hr ride to the trauma center, But when we pulled up in the ambulance bay and opened the doors the flood of goose down went flying every where. They sure do pack a lot of feathers in those coats, it was like a snow storm in the trauma bay. :-} Told the staff that sometimes ya just gotta do whats necessary to get to the PT. They all had a good laugh, except the housekeeping crew who were chasing down for days. -

Nice article Mike: I have heard of the NM cases before and heard some of the far right wing talk show host commentaries on the police state that exists out there. Have also heard that based on the officers dog "alerting " this whole thing came about. Also that the same dog has a history of false "alerts". Wonder if the dog is drug certified and by what agency? Here dogs have to be trained and then certified by an independent agency that qualifies the dog to be used on the street for law enforcement purposes. We have lab pups of our breeding that are search & rescue certified and bomb dog certified, so we have seen the process. Drug cert is one of the hardest to acquire. Lets take this down a slightly different path for discussions sake while we are talking about medico/legal ethics. You are at the scene of a motor vehicle crash. the police officer on scene comes over to the back of the ambulance and requests that you do a blood draw for him/her to retain as evidence of possible intoxication. Do you comply or not????? I know what my answer is as we have dealt with this in the past and have a written policy to cover what to do. How about the staff at your local ER? Will they do a draw for law enforcement? I'm not talking about blood draw for treatment reasons, but a specific kit drawn for law enforcement that will be used for possible prosecution. Our deputies and state troopers all carry these sealed kits in their cruisers. -

Not only did the arresting officer overstep the bounds of the law, he & his superiors went way overboard in following through with this whole action. The fact that the original ER Doc refused to participate in this should have given them a clue that they might be stepping outside the limits of the law, They then took him to a different hospital in a different county where the original warrant was not valid to find someone to do the rectal exam and follow up colonoscopy. If it was me they would be looking at far more than a 1 million dollar lawsuit. I would demand 10 million and all those involved be fired from their jobs for violating my civil rights to clench my butt cheeks. :-} What info we don't have is: Did the defendant have a history of drug trafficking ??? Was he a convicted former felon? or was this just a beat cop with a hardon for this guy??? What Did the prosecutor tell the judge to get the body cavity search warrant? why did the 2nd hospital Doc and his facility go along with this blatant violation of the accused parties civil rights? What were they all thinking? I have seen folks at the border to canada get taken down the hall for a cavity search after something was found in their vehicles that was illegal. But that was after they had been found smuggling contraband into the country, not based on a beat cops assertion that he was clenching his butt cheeks. -
A few years ago diversion was quite common for us. We have two hospitals in the next town 20 miles up the peninsula on the mainland. They both have small ER's with 6-10 beds and all serious trauma , chest pain & strokes would be diverted to the level 1 an additional hour south. then all of a sudden they figured out that they were losing out on patients that would generally be admitted to the med surg or ICU units which were big paying customers that were being sent south.. I'm guessing that the bean counters were looking at all the lost revenue that was being diverted. Now about the only thing we bring south are STEMI's going direct to the Cath lab and major multi system or serious head traumas as the level 1 has chest cutters and neuro on staff 24/7. The other regular transport south is patients on dialysis as the local hospitals can't handle renal pts.

Welcome to the city: Just be sure to keep up on your reading and pay attention during the lectures.All the materials on the test are covered in one or the other. Don't over read info into the test questions. What part of Mass are you in? Where are you taking the class?

my original license # back in 1973 was a 3 digit: 873. They are now into the 400,000 range last I knew. We met Pat Ivey about 15 years ago at a conference. Great speaker and motivator.
