good topic Ugly: We are on an island surrounded by the atlantic ocean, so we work fairly regularly with 2 agencies that provide dive teams. State police and county sheriff's office both have teams. Unfortunately they usually are in recovery mode. We also have a large Scallop &' urchin diving economy that puts divers in the water during the coldest months of the year due to the seasonal limits put on them by marine fisheries. We have had several training sessions with the team over the years and try to keep current contact info available. Our nearest hyperbaric chamber is at St Mary''s hospital in Lewiston Maine.

Working for me now. Thanks for putting all that info up for us Doc. It sounds that there is a lot more behind the story than we are getting from the news reports. In some areas there is a culture of flying pt's out when not necessarily needed,'Is it because of the numerous medevac services competing for their business or is it a case of not wanting to deal with a PT that might need more than see spot run "cookbook" medicine????

While he may have thought they needed a specialized pediatric hospital, it would have made more sense to load and go to the nearby hospital and let the medivac meet them there as necessary. My guess is the local ER could have handled these children and not cost the added $$$$. Far too often you read of reports where the PT was flown out by helicopter and treated & released within hours with minor injuries. AND a many thousand $$$$$$ bill for a helicopter ride.

The cow in the photo should have a escort chase vehicle with flashing lights. As the article said she was not identified until she went to a land shark and made herself public information. How that humiliated her and is keeping her homebound is a joke. Being so fat she won't fit through doorways is keeping her homebound. Don't like being called fat::: stop eating so much and burn more calories than you shove in your pie hole. Yeah I know :: We are supposed to be caring, warm & fuzzie when it comes to these folks. I'm getting old and don't have too any more. When you need to get ramps and powered winches fitted into ambulances to move these folks then it's not me with the problem.

Many of our protocols are being re-written due to evidence based medicine. Cardiac care has changed drastically over the past decade due to science. many of the standard first line meds for decades are no longer consider effective or even causing harm A lot of trauma care has been changed also due to studies showing what works , what doesn't & what their not sure of either way. Think permissive spinal immobilization or removal of MAST trousers from use. TX of shock and bleeding control with tourniquets have been developed with trauma care studies done by the military and passed along to EMS because they are effective.

I can't believe any service is still using non safed sharps. Engineered controls are required by OSHA or Dept of labor where ever possible. OSHA 29cfr10 covers this aspect. I haven't seen an old catheter in over 15 years on any of the 3 services I've worked at in that time. If I was that medic that got stabbed , there would be a lawsuit for not providing those engineered safety controls on equipment in favor of saving a few cents per catheter. Do you work for the empire by chance???? -

Is she alone except for the shady chickens? Does she have a patent airway? Signs of Ananphalaxis uticaria, swelling of the face / lips Was she eating raspberries ? & she's allergic to them? Any obvious injury site? Is she in V-fib / V-tach? Do we have her on an AED or monitor? How long has she been down? Given the topic header she's going to get aggressive tx with epi -

As others have said: Get some counseling from the VA for your issues with alcohol use/ abuse and possible ptsd. Most states will not allow you to take the course orissue an EMS license with 2 felony charges on your record. We have a full NCIC background check every time we renew our license, including a full criminal history. You will be asked when you apply for the course and they will check. With a pair of DWI's not even the evil empire would hire you, if you could get a license. -
