  7. 49393: You bring up many good point on the issues that rural /semi rural areas have with staffing and retention. The amount of time required start to finish can be in the 3-4 hour range and thats if the weathers good and the roads are dry. The minimum time to run even a simple transport call here is 2 hrs , plus the clean & restock+ the electronic run report. Thats the trip to the nearest ER. If the Pt has to go to a level II with stroke/ cath lab/ trauma capabilities add 2 hours to the run time. edit for: fat fingered spelling corrections
  8. playing devils advocate here: Are you the 911 contract service for the area ??? Or just contracted ambulance hauler for the nursing home chain? Personally If they felt the pt needed to be taken emergency to the Emergency room then a call to 911 would be in order. However if your private service has a discount contract with them then thats why they didn't use 911. Did the pt need emergent care?? Yes definatly. running lights & sirens for 21 miles in a large city rush hour is asking for trouble. WAAY too big a risk of a crash. You didn't have much choice but to follow the company policy , but the questions your raising here should be discussed with your supervisor or branch manager in a calm manner and ask for the reason behind the corporate policy. Is it the empire by chance?
  9. That why they call it practicing medicine. There is SO much that we don't know . Many of the "old school" practices have been shown to have little positive effect on pt outcome as science is expanded and studies are done. The problem with most studies, is the authors are looking for a set of answers to specific questions and in reality often discover something else they hadn't even considered.
  10. Here's hoping you get good prognosis tomorrow. Now you get to start the long road of physical therapy to get your muscle strength and flexibility back. Been there ::: done that. Pain = gain in moderation.
  11. Comparing a heavily populated state like Maryland to the rural vastness of Colorado is a joke. Yes if you go out in the boonies you may not get the same response times or level of care as you get in a major North American city of over a million inhabitants. Many states have this same issue. There are parts of my state where your response time might be in excess of an hour, and transport to a hospital could be over two hours. thats what you need to expect when you move away from society and go out to homestead.
  12. In my four decades+ in EMS there have been many arrest calls. Far too many to remember except for the two that walked out of the hospital under their own power with no deficits. Both of them were witnessed and CPR started almost immediately. #1 was while working at a large race track venue in NH. My partner & I were sitting in our ambulance at the head of the pit lane. A race car came in , parked and the driver started to get out. he collapsed less than 20 feet from us. My partner took off running while I grabbed the trusty LP5. One pre-coidial thump and three shocks later he was awake and breathing on his own. Transported him and after a 4 way CAVG he walked out just fine. #2 was an elderly lady that was having chest pain on a ferry boat. We got to her & she described a burning pain in her upper chest. Suddenly she stopped and collapsed. 20 minutes later we arrived at the dock with an alert & conscious PT to be transferred to the local EMS crew. We got holiday cards from her for another 15 years.

  20. All depends if the Pt's at the top & needs to go down or bottom of the stairs needing to go up. In the going down scenario : no worries, give me my trusty stryker chair with the crawler tracks. If the Pt needs to go up::: Call the fire dept. :-}
  21. 3 AM cardiac call one time:: One of my crew dogs gets out of the back of the ambulance & comes walking into the PT's kitchen . We were we were assessing the PT & All of a sudden the Pt starts laughing hysterically::: My Basic in his haste to get to the ambulance forgot he was wearing his flannel Sponge Bob Jammie bottoms.
  22. Depends on where you go. A basic here starts around $8.00 / hr some places will go up to $12.00/hr for a new basic, but generally in places where the cost of living is 3 times higher, so not big money
  23. Check out your local Goodwill store. My wife found several of almost new EMS style cargo pants from propper for $5.00 per pair one day. She took them in for a couple of the students in the EMT class she was teaching. Think smarter and as Mike said above, don't go spending a lot until your sure this is what you will be doing.
  24. EHHHH :: Can ya speak up sonny???? Surprisingly after decades of exposure to loud race engines , high speed turbines, sirens and heavy equipment, I still have a pretty good range of hearing. I do have some loss at very high frequencies, but for my age not too bad. My wife calls me selectively deaf.
  25. We operate in conjunction with the Coastguard quite a bit. They hate it when a tourist drives a boat on the rocks in our area, because it means they have to thread their way up here tween the rocks & shoals. These are dangerous waters around our Islands and the 41 boats don't have training wheels on the bottoms of the hulls. :-} We also have some pretty large tides so they hate to come up inshore as they got caught a couple of years ago on the outbound tide and had to sit in the mud to await the next tide coming back in. Very embarrassing to have clam diggers bring drinks & sandwiches out to them across the mud. Funny thing about the Coasties. Many of them are from landlocked states like Kansas, Iowa, Colorado or Nebraska. Interesting to see them farm boys out on the big blue oceans.
