  4. Steve: Like you , I am a gun owner and firm believer of the right for us to legally own them. However I do believe that a background check should be mandatory and over the counter gun shows sales & internet sales should all be required to have it done before the sale goes through. This is not gun registration, it is an attempt at keeping guns out of the hands of people that can't legally possess them. A poor attempt granted, but an attempt all the same. We all know that criminals can always get their hands on guns as there are millions of them in circulation, but eventually they will go away as they malfunction and are harder to get in the underworld. Here in Maine it is illegal for convicted felons to possess them and those who have been convicted of domestic violence can't have them either. Domestic violence deaths are on the rise , with the majority of them being caused by firearms. Do we not do anything to try and slow this trend?? When I worked in Hartford Ct it was a regular occurrence to have gang shootings and the majority of those weapons were in the hands of folks that would not pass a background check. a slow Friday night was only responding to 3 or 4 shootings.

  6. I think Mike is being politically correct in not using him/her. I always try a little personal one on one ,come to jesus meeting as a last ditch attempt to get a noob to explain whats going on and maybe they just need that one little extra step to be a quality partner. Then again as I said above, some folks just aren't cut out for this profession.
  7. The real problem is your employer has two main criteria for hiring A PULSE & A PATCH not everyone who manages to get the patch is cut out to be employed in EMS. If your noob can't cut it after FTO and a weeks worth of shifts as a driver. then it's time to have a come to jesus meeting with them away from the job. Ask them to stop & have a coffee after you get off shift. Explain to them your concerns and ask if there is something else you could do to help them improve. Tell it like it is, they are headed in the wrong direction for continued employment with the empire.
  8. Thats easy: your bag stays right where it is and your on the way to the hospital without it, or you can refuse treatment & transport and we will complete a refusal against our advice. I'm not risking my life or the life of my crew for anything. you called 911 for our assistance. You play by our rules or no bus ticket ! I have found handguns & knives while doing a "Trauma search" head to toe assessment. Little old lady with a ladysmith tucked in the pocket of her bathrobe, loaded and ready to fire. It was removed and handed over to a deputy at the hospital.
  9. <p>I wasn't talking about taking vitals.</p> <p>My thought process was geared toward how are they accomplishing their assessment and differential diagnosis.</p> <p>are they able to quickly identify and formulate TX for critical life threatening illnesses.</p> <p>Are they correctly identifying and coming up with the appropriate treatments that the Pt needs.</p> <p>&nbsp;</p> <p>If they can't take a set of vitals , then they should be back in the classroom learning how.</p>

  11. Mikey: It may be different out on the left coast, but around here people don't care what I'm wearing when they need our help. They are just glad to have our knowledge and talent come to their aid at the worst time of their lives. I respond with a job shirt or polo shirt and pants. I do have a jumpsuit for late nights or colder weather and they fit fine as long as you buy the correct size. you must have big balls and large breasts if you can't find one that fits. While I agree looking professional is a nice image, a fancy uniform does not make you a great provider or even a half way decent bandaid mechanic. If you want a tie go back to being a cop or sell insurance.

  14. Precepting on critical calls can be a mixed bag of do you trust them to provide important information to you, or do you verify the info to re-assure your self of the pt's actual status. If your pt's truly critical then you feel the need to be on the ball making the decisions and having a student in the process make it feel as you are delegating the responsibility. Don't have any easy solution to offer other than doing what you feel needs to be.

  16. as Mari said, population wise, we are getting to the boomer generation which is a large segment of the living population. As this large group ages past 60 they will become our greatest prehospital care patient load.multiple co-morbidities and a generation that has lead a hard life will increase the demands on emergency services for the next ten to fifteen years. We are already in the 60-70% range on our patient load ,although Maine has the oldest population in the country per capita according to the US Census 2010 edition. We are becoming a retirement home for many. It is a collection of diseases we treat in the elderly, with many of them having many different chronic illnesses to be treated. They will often not call 911 until it is late in the onset of symptoms, and don't want to be a burden to us. I will leave it now as I'm growing close to being "one of those old retired fogies.   23. The lucas is nothing more than a modern day thumper. I'm pretty sure the original thumper is still marketed. They could suck down a pair of "E" tanks in 10 minutes Unfortunately modern studies have shown a small or negative outcome in arrests where a mechanical compression devices are used. We bought the Zoll auto pulse system 5 years ago, and had some reasonable success with it. being able to palpate a pulse in a cardiac arrest pt tells me that it does improve perfusion. However the state has decided that even though they initially approved it for use, now they have removed it from approved devices list. Some areas have had better results than others with mechanical compression devices.
