You know if you are a first responder if...........................
island emt replied to FR911's topic in Funny Stuff
You have a galls catalog next to every toilet in the house. you carry enough crap in your personal response bag to outfit a small hospital -
What are your goals for the future? You list your occupation as an EMT-B yet in your first line above you state "I've "been" an EMT since 2009, but I've never been able to freaking work as one. " What are you doing currently? If you are out of school by several years and have not used the skills or knowledge, then you might want to re-assess where your mindset is. PS: not getting hired to work for the empire is not all that bad a thing. Unless they are the only game in town.

Remote Location "Medical Clinic"
island emt replied to Hooch.NREMT's topic in General EMS Discussion
How far are you traveling? How old are the Kids? Do you have a release from the parents for medical treatment if needed.? Do you plan to have a way for a sick camper to be evaced out to a hospital along the route if needed from any point along your travels? Are you hiking the Appalachian trail by chance? I'm thinking your adult to youth ratio is a little lean. -
Actually: it did happen in Upstate NY a couple years ago . One EMS provider was killed.

Cops: Secretary stole 'staggering' $643k from ambulance co.
island emt replied to EMT City Administrator's topic in EMS News
When are these so called Non Profits going to learn proper cash management and auditing procedures. How in the hell does someone steal $642,000 over a period of 4 years and nobody notices???? Where are the checks & balances in the financial system of this department? -
Transport Times and Protocol Decisions
island emt replied to scubanurse's topic in General EMS Discussion
Nobody is advocating not providing appropriate care that is clinically sound and needed right now to correct life threatening medical problems. Fixing a diabetic issue or giving pain relief before moving a pt with FX is common sense. Same as giving a nebulizer tx to a pt with bronchospasm. It's the difference between starting an IV , doing a set of blood draws and a 12 lead on scene for a fall that has a possible fx to the wrist. They are doing things because they can , not because it was medically indicated. Instead of transporting to the ER 5 minutes away they are spending 30 minutes on scene doing everything their protocol allows. We have all seen it and should know the difference in required care versus care because we can! or cookbook Paramedicine because thats how they were taught. Monkey see this monkey do that! -
OK Admin: What have you been feeding your news bot???? It seems to be doing pharmaceuticals !!! Making lots of new threads by repeating itself 10 fold.

TSD: What everyone else has offered is 100% right on the money. In this profession we sometimes see some horrific things and the human mind is not designed to handle these at times. Talking it out with someone who is not connected with the incidents is often a smart thing to do. there is no shame in this.

Chris: I don't think there is any hard evidence either way. But as I posted above having worked with folks that went from beginning to end with a paramedicine degree and outside of clinicals have never worked on a truck, there were some who found out that this job was not for them. To me this seems like an awful waste of time & money to get that far and find out you can't or don't want to continue on in the profession. I've seen a lot less of this with folks who worked the street for a while or while attending Paramedic school as a EMT-B. Anecdotal at best, but my personal observations over the last 40+ years of working the streets.
-
Can you afford to go to get a Paramedicine degree without working to pay for it? Last I knew a 2 yr degree program will run between 14 & 20 grand. Some schools are even higher depending where in the country your located. What about living expenses? Personally I think working for a while before attending Paramedic school gives a better understanding of what the job entails. I've seen some folks who went straight from EMTB to Paramedic without ever working on an ambulance. After spending all that time , effort & money, they discovered they can't deal with blood or vomit, & get carsick because of riding sideways or backwards.

We're having mud season. The magical time of year when the days warm up , the maple sap starts to run, & the frost is still down a couple of feet in the ground. Every day the top layers thaw out and turn to soup. That time of year when all the gravel roads are posted to not allow trucks to drive on them in fear of sinking out of sight. The time for the annual town meeting the harbinger of spring. The Cardinals coming back to sing in the trees. -
Transport Times and Protocol Decisions
island emt replied to scubanurse's topic in General EMS Discussion
You raise a valid point Kate. I have worked urban, suburban and Rural. Seems like in the city with a hospital ER under 5 minute transport time, that a lot more things are done on scene before loading pt and lengthening time from contact to Transport . Here in Semi rural Maine our scene times rarely exceed 10 minutes , unless the Pt is in extremis and needs intervention right now. We have a 30+ minute transport time to nearest ER's and hour + to a cardiac cath lab or trauma center so there is plenty of time to do the majority of interventions while in the office enroute to ER. A quick assessment ,vitals and maybe O2 then onto the stairchair or stretcher and lets go to the truck. In a chest pain Pt , do you take the 3 minutes to do the 12 lead in the house or move to truck and do it while enroute? I don't usually need the 12 lead to tell me a Pt is having cardiac related compromise and use the 12 lead as a diagnostic tool after basic interventions. The old mark 1 eyeball can tell me what is going on along with decades of seeing Pt's having chest pain & MI's. If it shows a STEMI , then we have plenty of time to call in to the cardiac specialty center and put the cath lab team on alert You can give them a good assessment and package in less than 5 minutes along with ASA O@ and then move on down the road. In the city it's hard to get more than basic interventions done during a short transport so more tends to be done on scene. I'd much rather do my work while heading towards more definitive care than stay and play for 25 minutes on scene and then have a 30 minute ride to the ER. All depends on whats best for the PT. -
Thank goodness crotchity is long gone

Gooday & welcome to the City.

Legal, Medical, Ethical Question - Emancipation
island emt replied to AnatomyChick's topic in General EMS Discussion
Hi Doc: I didn't mean to make a blanket statement like that. In the five states I've held a license to practice a 15 yo is considered a minor unable to provide consent or deny care or treatment of life threatening injury/illness. In all of those same states a 15 yo who had sex with a legal adult is considered to have been statutorily raped as they are not capable of consent. If she was 16 then the law is applied differently for some reason. I don't make the laws or even understand all of them. Just try to follow them the best we can. We dealt with a 13 yo who had CC of abd pain. At first we were leaning towards appendicitis , until she said " I could be pregnant" We transported and made our report to the ER DOC and explained our conversation to the DOC . Come to find out the pt had been molested for several years by an adult and it finally caught up with him. Last I knew , he's doing 50 in the state pen for statutory rape of a minor under 15 yo. -
Paramedics With Drug Problems
island emt replied to mikeymedic1984's topic in Burnout, Stress, & Health
Is the person protected by a union or civil service contract? How it is handled is usually documented in the contract or bargaining agreement. It sounds as if the RUMOR MILL has already convicted the possible offender without due process. Unless the person was found to be legally altered or in violation of state statute then it's nothing more than supposition on your part. How do you know that it wasn't reported and the proper channels followed? Oh I know::: the rumor mill! In most cases an employee with a substance problem is offered assistance and medical treatment, unless a crime has occured. Think EAP programs. Be careful about any involvement in the rumor mill, you could be found liable for slander and end up getting sued. -

Who cares if she is engaged to the sperm donor. If he is over 18 then it's a case of statutory rape of a minor. 15 yo can't legally consent. Not sure what you are asking about consent? Your title states emancipation. If she is truly an emancipated minor with the court documentation to prove it , then she has the legal ability to make her own decisions, If no documentation then the parents or guardian should be contacted. If they are not available, then a call to the ER DOC is in order to explain what is going on and asking them for permission to treat life threatening medical issues. Pass the responsibility to the Doc & let them figure out the legalities. -

RIP Joe; Too young to go. Sorry for the loss of a friend DOC -
No! The companies insurance carrier is just using your insured status to make their risk management dept feel better. Last time I was asked for it was in CT back 30+ years ago as I was under 25 at the time.

Congrats on the new gig. Sounds like all the hard work is finally paying off.

In some states they ask to see that as a means of showing insurability. Basically showing that someone else has checked your driving record and determined your safe to allow to drive a company vehicle & be covered under their insurance.

Richard: You state " I am legally both Momma B's medical proxy and acting with Power of Attorney," Are you legally appointed as such? If so then you should be listed as primary point of contact for ALL medical decisions. Make sure the nursing home has you listed as such and being the primary contact. Don't know what your family dynamics are< > but it should be worked out with your sister & brother so there are no misunderstandings & put in writing so there are no questions. Another thing that should be done is to make sure your mothers final wishes are documented and filed with an attorney. Have been through this with my parents, so I feel for you.
