The initial Phiily study showed that from EMS time of arrival at the penetrating trauma pt to delivery in to the ER often exceeded 18 minutes whereas the pt who was tossed into a car and hauled by his homies arrived at the ER in less than 7 minutes. Make a big difference getting them into definitive treatment and the OR when time from scene to ER is less than half. EMS has time to dispatch , time to drive to scene, arrive and assess the pt. do initial TX before packaging and then time to load & transport. Add in 2 large bore IV's to thin down the remaining blood in the system and you have a recipe for lower survival rates as we were not fixing the issues of massive internal damage & bleeding. Yes it is hard to quantify how many of these severly injured pt's were going to die no matter what , but the study did a pretty fair representation of the pros & cons of rapid delivery to the ER versus Paramedic level intervention in a large urban environment.

We bought the Veinlite about 5 years ago. Works well in finding the deeper veins in obese population and especially for those hypothermic pt's with extremity shunting.

Remember one thing Trevor: unlike working for a nice family owned business such as yours:: the folks that deny it is a problem probably work for those crappy companies who stage their employees / slaves in Mickey D's parking lots or convenience store / gas stn parking lots to save on having a station for them to operate out of . they brainwash their lackeys to believe it is good for response times instead of saving the company $$$.

I remember when the previous study results came out and rocked the EMS world. WHAT do you mean doing all our tricks and interventions is leading to a lower survival rate???? It makes perfect sense in that specific population with penetrating trauma that NEEDS an immediate trip to the Surgical suite.

i stand with my brothers & sisters and am sure that we all suffer from it to some extent. Too many years of horrific calls and far too much human suffering and misery that we are expected to deal with on a daily basis will take it's toll on anyone....... Finding someone to talk to in confidence can be difficult and admitting to the powers that you need help, be can be career ending. Stand Tall my Brothers & Sisters We are STRONG and can work though our issues with the support of each other. -
very little info to go on Doc. maybe the distraught teen was unrecognizable due to her WHINING and the operator could get any usable info out of her to be able to send assistance. Hysterical people are very hard to make sense of .

From washington state to Utah is going to be a long and dangerous transport for this Patient. I for one would not want to be on this transport without legally required restraint and a plan along the way for rest stops and feeding him in secure places. Getting an order for the above will not be easy . You cannot have someone confined and restrained for that length of time in the back of an ambulance, especially against their will. Ruff: his honor the grand mayor of Sasquatch land has plenty of time on his hands to deal with blowup dolls :-}

sorry not touching this one with a ten foot pole. You said it above, The mother has long ago caved in allowing the behaviors to flourish & providing the violent video games and there's the blame. I am very familiar with aspergers syndrome as my wife taught special needs and directed a school with severe autistic and aspergers students for over 10 years.. Providing them medication will not help as it often works the opposite with them causing severe effects. go to his medical team and get help.

An air traffic control tower suddenly lost communication with a small twin engine aircraft. A moment later the tower land line rang and was answered by one of the employees. The passenger riding with the pilot who lost communications was on a cellular phone and yelled "Mayday, mayday!! The pilot had an instant and fatal heart attack. I grabbed his cell phone out of his pocket and he had told me before we took off he had the tower on his speed dial memory. I am flying upside down at 18,000 feet and traveling at 180 mph. Mayday, mayday!!" The employee in the tower had put him on speaker phone immediately. "Calm down, we acknowledge you and we’ll guide you down after a few questions. The first thing is not to panic, remain calm!!". He began his series of questions: Tower: "How do you know you are traveling at 18,000 feet??" Aircraft: "I can see that it reads 18,000 feet on the Altimeter dial in front of me." Tower: "Okay, that’s good, remain calm. How do you know you’re traveling at 180 mph?" Aircraft: "I can see that it reads 180 mph on the Airspeed dial in front of me." Tower: Okay, this is great so far, but it’s heavily overcast, so how do you know you're flying upside down?" Aircraft” “The shit in my pants is running out of my shirt collar.” -
the majority of ambulance crashes are avoidable so it's not the same comparison. A few happen without any chance of avoiding , such as another vehicle suddenly crossing the center line. but many occur at intersections where if due regaurd was used could be avoided. Like michael , I've lost several friends over my career and the worst was one who killed himself & was found by one of his kids. She will need counseling for the rest of her life.

Are they offering a transition course in Texas to go from I-85? Should be similar to a refresher class with new updates to the scope of practice. Here We had the I99 level and they just changed the name on the license at renewal. They actually dumbed down the scope of practice in the change over to EMT-A

there has long been the stigma attached to EMS providers who " can't handle it". Just suck it up and move on has long been the mantra. Some folks can go through a career & never have "that " case that sets you off, or the combined group of calls that are your trigger. Recognition of these sentinel events isn't taught to providers, nor is the avenue to find help when you become overwhelmed. I'll admit to having a couple of events that are my triggers. the smell of burnt hair & flesh will send me into a very dark place in my past. the kind of place nightmares are made in. As far as the general population understanding the things we deal with on a regular business, there is little hope that many of them will ever understand the good , bad & ugly , that are a normal part of our lives. They couldn't handle it. PTSD can take many forms and will affect each individual differently. Some turn to booze or drugs, others tie it up and store it in their subconscious until it boils over. and others just let it out to a loved one or friend they can talk to. Others still , it becomes the thousand pound weight on them and causes severe depression from which they can see no way out from under. It usually is the ones who seen steady as a rock that have the worst time with it as they keep it locked in until the powder keg explodes.

OK now that I see the rest of your Questions in the title... Cutters rarely need any medical treatment other than cursory covering the the slices to keep them clean. Cutting is a cry for help: " to notice me, I'm hurting myself" We had one cutter that was a daily occurrence for several months and finally got to the point we said just come by the station if you need to talk to someone. They did regularly after that and the cutting slowed enough that person was able to go to college and get a degree in psychiatry. Jumpers are ones that will usually allow them selves to be talked down. Serious mental health issues going on there. postpartum depression brings on the ones who drive their kids into the lake in a final act of depression. All of the above are facets of mental heath issues that you can never truly understand the" why " Yes they treat chemical imbalances with medications , But WHY do some folks go through life and then suddenly decide to kill them selves??? I don't think anyone here really has that answer.

generally speaking: if you are responding to a hanging the Pt will be gone long before you arrive on scene if they are still hanging. Now if you are talking about asphyxia sexual games then that's a little different as they don't usually tie a correct hangmans knot which crushes the trachea, it 's more of a squeezing of the carotids effect to cause them to black out. Yes : I've responded to many over my career of both types and they are usually coroners cases It seems to have been more prevalent during the late 90's and 2000's in younger folks, than in previous decades. If you believe they are not deceased then it will be easier to cut away from the know anyway. Less rope to cut through. If they show signs & symptoms such as lividity or rigor then just walk away & wait for law enforcement , it is a crime scene.

The CBT is a computer generated test from a bank of thousands of questions based on the the knowledge taught in the particular level of class. It asks a series of questions and based on the testee's answers adjusts the next series of questions accordingly. Unlike the old style paper tests that were supposed to be kept in a safe and guarded like bearer bonds, and had a printed answer sheet. -

it was an assumption they were taking the emtb based on how the OP started the question. deer in the headlights style" So I failed my" What would you rather see as a response???? " Well now now little one, it's ok to not achieve your goals in life and don't worry about not being able to pass a basic course exam. We'll just make it easier for you and draw the questions out with crayons so you can color in the answers and achieve your goals of being an EMT". -
