neither. It wasn't intended as an EMS box. It 's original use was for fishing tackle. It has an upper compartment with small dividers and a deeper bottom compartment which works perfect for the BP cuff , stethoscope and bandaging, sam splint and a few other necessities. We still have the 727's on the ambulance for our statewide drug box system. Every service had to pay for a box for each ambulance, They are part of "the system" Every hospital in the state restocks the same meds in the same box. We use drugs from it and just swap out for a new sealed box. Our Narcs are kept in separate lock box and narc safe . -

In my over 40 years in EMS, I been called to assist with childbirth twice. The first was a 14 yo girl that had given birth in the basement of a tenement . The infant was dead the mother almost so from massive hemorrhage. The second time was a woman in labor at the racetrack. She was in her motorhome in the camping area. She was crowning when we got there and the only assistance she needed from us was a little suction and cleaning of the infants airway, a hemostat for the cord tied off with dental floss and a nice gentle ride to the hospital. Women have been birthing babies for thousands of years without assistance from us. The point I'm trying to get into your head is just because you read a couple pages in a book, it doesn't make you a gynecologist. EMR is just barely above red cross first aider in scope of knowledge.

The problem exists because it doesn't take a license , education or training to reproduce. The unintended consequences of idiots having sex. Not much difference in the spread of AIDS in certain 3 rd world nations where they choose not to use protection and have upwards of 70% of the population infected. They don't want the child so they were ending up in dumpsters and other places. This at least is an attempt to stop that from happening. Mandatory spaying and neuter programs come to mind!- 6 replies

Slider boards or transfer sheets. Stryker has a really slick setup that is like having a hovercraft under the pt. It's a polymer sheet . -
A cheapo anything is junk. And exactly what will a 5$ amazon OB kit have that will do you any good, besides lighten your wallet? Better to spend the 5 bucks on a couple condoms, then you won't need the OB kit. :-}

Yep: This was a simple general weakness dispatch: We're familiar with him and seemed like nothing seriously out of the "Norm" for him. BG was high as he missed his dialysis scheduled for early am today due to heavy snowfall and really dangerous road conditions. While we are slip sliding away onto the mainland and starting to head south, he got pale, grey and diaphoretic, vomited a little bile and brady'd right down to less than 10. Went asystolic and pulsless. Put the pads on , got capture and paced him back to high 40's. Didn't even get to compressions before his eyes popped open and he started yelling. We had him on 12 lead due to hx and he had been joking with us and reasonably normal vitals for him. We turned around and stopped at the small hospital ER for stabilization. Labs were all out of whack. ER doc gave him Atropine, insulin, calcium gluconate, Albuterol neb , Versed, Bi-Carb, all in attempt to raise his PH , lower his potassium levels and control his BG. Not sure why but it all worked to get him stable enough to make the additional hr south to the big city hospital for dialysis and follow up care. Need to do some follow up and research on this call.

A good story to relate: Picked up a gentleman today that needed to go to the big city hospital due to needing dialysis. It's snowing a blizzard and the roads are barely passable. 30 minutes from here & still an hour away from the big city hospital he decides to brady out & go into asystole. Pads on, paced and his eyes open up. Then he starts yelling about us zapping him. The good news is he was able to complain about the electrical therapy hurting him. Was touch & go for a few minutes. Beats the hell out of the alternatives.

The same reason newbs insist on having 15 different items hanging off their belts. I worked with a young medic years ago who had a duty belt that weighed 37 pounds with all his equipment hanging on it. You know the type. 5 different kinds of shears & hemostats, 2 glove pouches and holster with center punch seatbelt cutter mini mag lite. another holster with a "C cell maglight and electrical cable cutters, 5 pens and 2 different EMS guides, rolls of tape and a trauma dressing pack. I'd get a hernia toting all that stuff on my belt. My own trauma/ first response kit is a plano box that I've had for 30 years. It's not a big 747 , just a nice small tackle box that has lasted all these years. Yes: It's not cool, but holds all I need until the truck get's to me. -
Welcome To the City Ben: Slow down & take a deep breath. You are going to be a medical first responder You don't need a $200.00 trauma bag or about half of the things you listed above. You need a simple bag to hold trauma dressings , some sterile gauze, kling and a few airway supplies like a pocket mask and maybe oral or nasal airways. Stethoscope and BP cuff. A sam splint and tape will give stabilization for FX's or sprained ankles. The bags I buy from Boundtree are around $45.00 and contents cost about $50.00 in total. All of our licensed EMT's carry one of these in their personal vehicle along with an AED. We are a rural area and often first respond to the call if it's closer than going to the ambulance. I don't think the OP had any idea when he came here to ask the questions. It is easy to dream about wanting to do something , until that little thing called reality sets in and bites yer arse. My take on the PA ambulance service is consolidation and closing of many of the volunteer department run services over the last several years. At least that's what has been in the news. Regionalization seems to be where many rural area services are headed. -

There are a couple of issues to deal with here. Rhabdo caused by the circulation being restricted in lower extremities and compartment syndrome caused by restriction to lower extremities by the harness leg/crotch straps. Once the pressure is released the pool of "bad blood " will recirculate and cause electrolyte imbalances which can lead to acidoses and cardiac irritability & arrhythmia's . Rhabdo causes increases in potassium in the blood as the muscle tissue breaks down releasing myoglobin. these lead to the potentially fatal arrhythmia's TX to include multiple IV's with N,S, 0.9 and ringers lactate ,TKO, cardiac monitoring and gentle handling to limit cardiac shock. Consider bicarb as pt appears to be in toxic shock prior to our arrival. Position in semi fowlers and try to restrict movement of lower extremities. Be careful with fluid administration as the kidneys are often affected quickly and renal failure is common. -

Thanks for baring your soul to the world. Like it or not you are getting older and fit into the bracket of having possible issues in multiple places. We are not 20 year olds any longer.
