you know you're in urban ems if...
island emt replied to lemonlimeEMT's topic in General EMS Discussion
Yes young grasshopper: I have been in those situations. I've heard the sound of 9mm autofire hitting the wall over our heads and had to take trucks out of service due to too many gsw's in the electrical system. I survived the heroin wars of the early 80's in Hartford Ct. and lived to tell the stories of the horrors of gang rivalries killing innocent children. A slow Saturday night was anything less than five shootings in the trauma bay At Hartford Hospital all that was long before you were born or even thought about. There are many here that have the same experiences and we don't denigrate any one race or ethnic minority. If you don't like the hood , don't work there If you feel the need to laugh at societies underprivileged, then this is not the profession for you. Coping old sad jokes is not cool.-1 -
Wierd call. Can you create a scenario that justifies it?
island emt replied to DwayneEMTP's topic in General EMS Discussion
She said he was initially alert but in pain. He went into cardiac arrest after being loaded into the ambulance. Morse said she performed cardiopulmonary resuscitation on him. The ambulance left for the hospital with her in the front passenger seat but she alleges ambulance personal left her on the side of the road after she asked to sit in the back and hold his hand as his condition worsened. She was told he died a few minutes later. SO Which was it???? Was she in the back doing CPR on her husband when he went into arrest""" OR was she in the front seat and asked to get out and get into the back to hold her husbands hand when his condition worsened ????? Way to many conflicting stories out there in the press. -

Probably a good sign that they are seriously considering an offer of employment. Next visit will probably have a test on them to see if you had the initiative to at least look through them and learn some of their organizations protocols. -

Actually , the course my wife taught last semester ended up at 185 hrs plus clinicals and ride times which brought the total time to around 240 hrs. We added in 2 8 hr Saturdays for hands on exercises such as applying a KED in a vehicle and used several different scenario based "calls" to get the students comfortable with what they had learned. Yes they all knew how to properly apply a KED while the Pt was sitting in a chair, now put them in a tiny car and make the adjustments to put it into practical knowledge. The current course practicum has lots of new requirements that require additional hrs to get through. -
Welcome to the City. Join in with questions you come up with after trying to figure out the answers. From a dino that used a stone tablet and chisel for my clinicals way back in the early 70's

The most recent shortages here are Valium , Versed and just this week Zofran. Fentanyl is in short supply and there are many other meds on the short list that are being watched carefully. We have been getting weekly updates on what is in short supply or hospitals have run out of.

IPMBA Bike Conference (St. Paul Minnesota) CARPOOL
island emt replied to Medic One's topic in Education and Training
Why are you not peddling your bike to the bike conference? Just so you know I 81 will not get you to Ohio. Better check your Rand McNally again. -
couldn't happen to a nicer bunch of fishbait! Question: What do you call 1000 lawyers at the bottom of the ocean?? Answer: A DAMN good start. :-}

All i can offer to you: is patience young grasshopper. Your time will come and then you can take care of the old farts like me. Look to some of Biebers post from when he started out. He has become what many of us hope for the future of EMS. Young , educated and willing to admit there is much they need to learn from everyday. Don't let your early years ruin it for the rest of your career. When I started out , I was 18 and couldn't wait to be at the top of the heap. Then I learned patience and learned to listen to my more experienced elders. A stint in the Navy also went a long way towards seasoning some of that attitude.

Your welcome! I've been doing this profession since 1971, and have seen hundreds if not thousands of newbies come into it full of dreams and passion, only to see them burnt to a crisp within months due to working in a situation like where you are now. I made that blunt in your face comment for a reason, to get the reaction from you that I thought was there deep down. A need to be better personally than a crappy transport service holding you back. It is good that you realize this and are moving forward.

The dispatch info is not necessarily covered by HIPAA. How the press gets the info is probably a violation of departmental policies and procedures. In the end though 911 calls are a matter of public record and have to be released to the press/public in the event someone files a Freedom of information request. Don't you think that the hollywood types thrive on the press they get. Who cares if a middle aged has been has a party and does legal/ illegal drugs and passes out. BUT , make it a big deal with the paparazzi and news helicopters and it gets their name back on the charts.

I am not afraid of any damn stiff. I shit myself because IT wasn't my damn patient and I let this guy spin down the drain quietly instead of doing what I should've done.....Pulling out a BVM and bagging his ass and when my medic got into an up roar I shouldve told him to fly a kite. Hell I could've deep suctioned him, did a chest decompression if needed, or re-intubated BUT I didn't have the damn patch to do it. Because of his lack of action it makes me look like an incompetent fuck. And yes I am concerned what people think about me. Wouldn't you if you were "some kid". People judge me because of my date of birth. Not my ability to care. And I feel like if I can acquire the knowledge and ability to care they will overlook my age. Am I off key to think this way? Im not afraid to act. Im scared of the consequences of due to my LACK of action. Also on a different note. I came to the ICU a few days ago. He was no longer there. He died. So I am wondering if Ill be appearing in court a few months from now because my partner and myself let this guys brain fry due to hypoxia? And im not doing no damn granny shuffle. Just last weekend we took a critical pedi. We are a ITFT that is under trained and underequipped. whatever drugs we need but dont have. we get the docs to write the orders (our protocols are the generic J-RAC and even than we dont carry all the drugs we are supposed to. so whatever we need we have get from the transferring doc) Hell the company is so damn cheap they are training all basics to give EPI IM instead of buying the damn EPI-pens. There you go Mike: Thats the response I was looking for! You came on and sounded bewildered and confused and whining about everything. I wanted to see if you had something else in your retinue. Yes you are working for a crappy transport company, that doesn't train it's staff, doesn't have the proper equipment on their trucks, doesn't have the required meds etc etc. When the high point of the past week is a single pedi call you are doing the shuttle. I wanted to see if there was some fire in your shorts and a need to be better. Sorry if that seemed cynical, but sometimes the truth hurts! Glad to see your seeking employment elsewhere, a crappy shuttle service will suck the life out of you::: & your patients!

island emt replied to DwayneEMTP's topic in General EMS Discussion
Like I said above: I wasn't there & I don't know any of the details of the call. I can only think that if the pt arrested within a short time of leaving the ski area and they were faced with an hour plus trip to deliver a body to the hospital ,or turn around and go back a few miles to the mountains medical room, the decision to return would make the most sense to me. -
Kaisu: May you find the peace that is avoiding you and hope your trip home helps to clear the gremlins from your mind! Be careful and get some rest.

Being from the same state and having been to the loaf on more than a few occaisions, All I will say is the service up there is very well regarded and the trip down the mountain on a nice sunny summer day is an interesting 50 minute ride to the nearest hospital. This event happened on a beautiful snowy Maine winter day with nasty driving conditions. The press report is from the victims home province in Canada, and they only info they printed in the story is from his widow. Yes we do have a termination of resuscitation protocol here. Yes they can choose to leave a body if protocol has been followed. The current thinking [read protocol] is to not transport bodies to the hospital. Other than that I will wait until the investigation has been completed and the report been made official. On another thought: How many traumatic arrests survive? Do you work and transport traumatic arrests more than an hour down the road to a tier 2 hospital? Do you have a field termination protocol at all? -
If the pt had a magnet placed on the AICD then it was no longer functioning. Hence the pt own heart was the only source of power . If the heart gave out then that could have been the pt's wishes . Thats why the magnet. Lose the "anyways" it makes you appear to be a 12 yo

I give a + when the poster deserves credit for going above the minimum. Those that make a post and continue to learn from others. I give a - to all the derelicts who come here as posers and intentionally try to be counter productive, are just rude, or are not giving any positive benefit to the site.

I Said NITRITES which are commonly used in curing raw meat to inhibit pathogens, they also cause cellular constriction of the tissue. http://www.extension.umn.edu/distribution/nutrition/DJ0974.html
-
I'm thinking it's a combination of the salts and nitrites used in curing that would cause the vasoconstriction..

Sounds like you have a homework assignment to do!
