Fentanyl shortages, got us Morphine back in the formulary Now there's a MS shortage so they are looking at dilaudid. next month dilaudid will be on the shortage list , then what? The system is in trouble & that includes the hospitals. Some of the others on the shortage list: Valium, rocuronium, vecuronium, glucogon, Saline flushes, D-50. And the list goes on & on. Maybe we need to go way back into ancient medicine and just let the pt's smoke the opium pipe! just kidding folks! But then I grew up in an age where mothers rubbed Paragoric on a babies gums. No wonder we slept welll as infants. :-}

Didn't mean scoop and run at all: Like you I know a large percentage of our Pt's on a first name basis . the kind of small town where they call me at home and ask if they should call 911 or ask us to come check on grandpa,cause he's just not right today. Rather than hauling all your tools into the house, take the basics with you and make the decision to spend the morning sitting holding their hands or get off the pot and get them headed down the road at a safe comfortable pace while you do all the needed interventions. No need to be wasting all that travel time just holding hands because everything was done on scene. Yes there is always the diabetic that you wake up with a line and d-50 that takes an extended scene time . But you don't want to fall in the habit of 30 minutes becoming the norm. We do very little code 3 running here due to the roads and weather conditions, so there is plenty of time to get most everything including the PCR done.

Dwayne: I get where your head is on this notion of brotherhood/sisterhood. Like you , I've had a chance to travel to various parts of the world on the job thanks to uncle sam, and have the opportunity to travel on the job all over this country and most of Canada. We are still close friends with people we were stationed with in Western Australia in the mid 70's at a place, where the next town was 250 km away down a dirt track. We grew into adulthood there in a small close knit group of people. I worked with some really incredible folks along the way and met some blatant assholes at the same time. There are many of these folks over the past 40+ years that I could get along with as neighbors, if I wanted to live in a close contact environment. However as I age through my mid fifties, My wife of 37 years is all the more special and I just want to get away from the rat race and spend time walking down a quite dirt road together and not hearing anything but peace & quiet. Sitting on a rock in the middle of the woods and hearing the wind blow through the tree tops and the birds chirping is such a pleasant sound. May you find whatever it is that will let you find peace and happiness along with satisfaction of a job well done, while at the same time allow you to be home with Babs & Dylan. Don't let their lives pass you by! Peace Brother! By the way if your travel arrangements allow it make a stopover in Christchurch NZ. Incredible city, beautiful country. -
Why do you feel your scene times have increased? Is it because you are doing a more thorough assessment ? Are you doing procedures that can be done while enroute for an extended ride to hospital? Are you spending time getting more info on scene than you used to? from past experience, It always seemed that city medics tended to spend more time on scene as their transport times are short.<10min to a choice of ER's My average scene time in our rural setting is less than 10 min on every call unless their is a difficulty removing the pt or in case of a code where we work them for 20 minutes or ROSC. Many of our scene times are under 5 min. We've got a minimum of 30 min transport time in good weather to the nearest small ER and well over an hour to a level 1 trauma center with cath lab and stroke center capabilities. Plenty of time to do all our interventions while traveling down the road in my office!

Why do you only mention an adult taking advantage of teen girls? It would seem that in todays society there is a great a risk of someone taking advantage of either sex teenagers. Either that or your living in nirvana of the republican party. You know how to find a republican senator? playin footsie in the third stall of the airport bathroom. just sayin!

While this is turning into an interesting discussion of societal rules, the question is does a society have a right to control such things as shaving legs pits faces, or the use of water & soap on a reasonable basis? Did not Hitler try to create his idea of an ideal community? genetically pure , blond haired, blue eyes, aryan true. There are many societies in this world that look at what we would consider the norm, as being utterly ridiculous! Chris: you are correct on adapting to your environment as a form of self preservation. The term "going native " applies in many ways as a means of not sticking out in society. If Dwayne wants to have sliced white bread suzy Q's with shaved pits and legs then he is free to include that in the criteria for joining "HIS" community. That's what I observed in the Co Housing group I visited, selected carefully for the exact qualities the Group wanted. -

Dwayne: I used the above as an example of the crunchy overdose feeling I got when visiting this place. They felt they were doing good by helping poor children by knitting winter hats and had the most sincere reasons for doing so. the group breastfeeding was just one of many feel good things to show that they did truly fit into the mold or pattern of the group. They just seem to have lost touch with reality of daily life for the "average "person. The majority are graduates of expensive New England colleges. They are living in their little piece of nirvana and the majority of them are glad they chose to live in CO-Housing arrangements. As I stated there was an fairly high 6 figure buy in price so they kept those below their social standing out. -
In some states you may be classified a felon with a single dui/dwi conviction. In other states they don't even give jail time until the 4th 0r 5th time you get bagged. Read a news story here about a gent who had just been sentenced for his 15th OUI in 12 yrs time. He hasn't had a license for ten years, still doesn't stop him from drunk driving/ driving while impaired. He got 90 days in jail. Please don't take the this wrong, we know you are aware of that awful mistake in judgement you had as a teenager. We are just giving it to you straight. On to the future for you . stay in school , keep your grades up. go forward with your life lesson filed away , continue on to medical school. make yourself proud. Us to!

In Maine you will be asked about any criminal record on the class application. You might be allowed to take the class, but when it comes to licensing , your application will go to the investigations committee for review. You need to be very honest and contrite and have many good reasons that they should issue you an EMS license to practice. You will probably need character references to stand testament to your changed behavior and attitude since your DWI.. That might or might not get you a license to practice. Then you need to get hired! Whole nother story here my young friend. I could not hire you in any way shape or form with a recent DUI and under 21 yo. Our insurance will not cover you. need a 10 yr clean record after a DUI. That's just the way it is! Life sucks and then you get older.

The big draw here was to the granola crunchy types who wanted to have a sense of belonging to a small group of similar minded folks. many of them are home schoolers and share the teaching and childcare duties and also share in the communal cooking and gardening duties. Others in the group help the unit with home maintenance or grounds keeping. There is a sense of community well being in place and with the price of the "buy in" there are few slackers or welfare types in the group. They are of the well educated professional category,with artists, musicians, college professors and medical professionals included in the group. Some might consider them elitist in the segregation of rich versus poor working class , but this is an added benefit to them as a group. Won't find many drug addicted welfare mom's on their property. I find it an interesting experiment in social mapping by creating an environment of "select neighbors". Not a whole lot different than living in a gated community where you control access to who can enter. Would I choose to live this lifestyle? No I don't think so. We have our own ideas of how we want to live, such as getting away from people when we go to our camp in the northern Maine woods. The nearest neighbors are not visible or heard, unless they're target shooting. -

Dwayne: do a search on the "CO-Housing " movement. It is a modern day shared society where they take a parcel of land , build smaller energy efficient homes that are grouped together around a central meeting house kitchen/ dining/ community building. there are several of these in my area and the overall thought process is to lessen the demand for energy by sharing some facilities and sharing in the gardens and natural space while still having your own private space. -

Well Seth : I've been in EMS since 1971 and even though i practice my profession everyday, I still don't know everything there is to know about Prehospital Emergency Care. There is no quick way to become proficient in everything. -
Lets see : What are the facts. 3 AM pt falls down & sustains head injury. someone calls 911. Ambulance crew evaluates and packages pt. sometime during transport pt regains consciousness and claims to be raped/or assaulted in back of ambulance. pt doesn't report it until after she was released from hospital. Is it possible? of course it is. Anything is possible. Is it likely to have happened???? I don't know based on very little info given.

You shouldn't need to stop in order to start a peripheral IV unless it's on a really squirmy PEDI. I have a hard time doing them in clean ,non moving well lit ER rooms.
