Welcome to the business cowgirl. Don't let one bad experience sour your outlook on prehospital medicine. Actually right now Dwayne has been banished to Mongolia for sleeping with a rocky mountain sheep. :-{ Who knew they had high speed internet in Mongolia ????

After watching last nights debate. Romney and Huntsman are clearly the frontrunners : at least as far as intelligence and political savvy are concerned. Can they put together a team with the strength to beat the Dems??????? we will see. the rest of them need so much to be considered legitimate candidates. as for Cain's accusers and the fact that this info was brought up only after he started rising in prominence. Don't rule out the republican party dirty tricks squad. They allowed him to run in an attempt to show that they "could allow" a Black man to be part of their party. But then the powers that be don't want him to get too strong in the polls.

Could you use that cellophane from that pack of smokes for?

While possibly better than locking lips with someone you don't know: your scenario will not stop the transmission of diseases as there is no one way check valve to stop the pt's breath from coming back to you along with any bacteria or diseases they may have. This is the primary reason for AHA recommending that bystanders only do compressions in case of sudden cardiac arrest. Don't worry about breathing for them until later in the process. The thinking is that many folks wouldn't do CPR because it meant giving breaths to someone they don't know. -
Thank you Herbie: as You well know , it's not easy coming to grips with the loss of a family member anytime. But to have them killed so far from home makes the loss even harder. Still dealing with the paperwork and getting his belongings back from the sandbox. It's been very hard on my wife and her elderly parents these past few months. Lone Star you are my hero: thanks for a well spoken tribute to ALL of us Vets. We all give a lot in defense of freedoms that many take for granted. Ed

To all my fellow veterans : May you have a day of peace and solitude. Let us remember all of our comrade that gave their ALL in support of our values. Pray that todays wars will be the war to end all wars and let us have peace across the globe. Dwayne: You have my vote in keeping a thread of respect on track. Thank you ! herbie: I can feel your loss as my brother in law was sent home in a box from afghanistan this July. That doesn't mean we should allow a thread of respect to turn into a circus of dissent or disruption from our honoring those that gave their all.

I went to visit two different Pt's today. One was a 91 yo wit a minor injury that I stopped by to see if his daughter [ caregiver] had any questions or needed anything like a walker. & the second was a 64 yo that was transported with suspected large MI. His wife asked me to stop in & check on him after 4 way CABG . This is a normal & regular part of small town rural EMS. -
Please don't take my comments as disrespect in any way for our brothers in arms who gave their all defending this country. I'm a vet and have been through the process of recovering "remains" of brothers lost. War is not like the car crash or trauma pt's we deal with on a regular basis. Many of the fatalities of todays wars are serious blast induced high explosive in nature and destructive in outcome. Recovery is not an easy or pleasant task. We [ our military] do the very best to bring home all of our brothers remains that they can. They are handled with the greatest of love and respect by their" brothers" assigned this task. It is not a task for the squeamish or weak kneed feel good types.

HIPAA Rule - Do we know what we need to know
island emt replied to hatelilpeepees's topic in General EMS Discussion
One of the exact points I was trying to make earlier. Many providers have never read the complete law or attended a training session put on by a lawyer to explain the ACTUAL meaning of what is HIPAA. Many still call it hippa and use it to include any and all definitions because they read something on the internet. Our state did a big roll out of HIPAA ten years ago and the state attorney insisted that" all prehospital providers were covered entities". I then handed her a copy of the rule that shows that a non billing entity is not a covered entity. Hence the new state law was incorrectly written and passed. Didn't make a whole lot of friends in the state office over that one. :-} -
Ruff: the alleged missing pieces of remains are a collection of fragments retrieved from the crash site of a military jet fighter aircraft that impacted the earth at high speed and exploded. Not sure how many of you have ever been to such a crash site, but you sift and bag anything that may have been human tissue to be sent home for identification. They are not talking about loosing an arm or torso here ,just possible small fragments that might have been part of a human being at one time. The graves registration folks do an incredible job under the most gruesome conditions possible. Many ID's are made strictly by DNA testing of minute pieces of tissue only.

Too many people have minimal understanding of WHAT HIPAA really means. It only affect's you if you are a billing service that uses various forms of electronic means to transmit Privileged Pt healthcare information. IE sending pt billing service your electronic run forms or transferring them to your central data processing office. Privacy laws are a completely different thing. If you have been talking about your Pt's at the beauty parlor then you are violating privacy laws, not HIPAA. Page- Wolfburg has a great program on what HIPAA really means to prehospital providers Can't find the link right off the top of my head, but I'm sure a google will find it. About the beauty parlor thing: You might want to ask for a refund cause it ain't working ! -
If thats what happens when you eat airline food then count me out. Always knew their was a good reason not to fly. Where are they coming from? did they have exposure to something prior to boarding the aircraft? What was the trigger event? What type of medical hx do the brothers share? 1st thing would be to get an ALS crew standing by on the tarmac. Put out a call to line up enough trucks for a possible mass casualty event. Notify the airport security of possible need for a quarantine of all pax & crew until they can be cleared. Notify FAA & homeland security & customs agency contacts responsible for your jurisdiction. Notify the medical examiner for the deceased individual Have the hazmat team respond to clear the aircraft for possible toxins coming in from foreign country. Notify local level 1 hospital of events so they can set up and notify of possible incoming mass triage . Thats a good start until the aircraft gets wheels down on the tarmac.

Bernhard The unit in your first link is close but still quite a bit smaller. More like what we call mini-modulars, which are often used for city trucks. Your second link is similar to a type II van based chassis with raised top. Companies like the empire & other privates use these because their cheap to purchase. We have some transport companies using the sprinter based units for their long distance transport, usually hospital to hospital or interstate transfers because of improved fuel economy . Here's a link to the truck we use. The photo in the brochure is a Portland Medcu unit , twins to ours and many other services in our area. Plenty of room for the stretcher and another pt on the squad bench. http://www.plcustom.com/PLShowroomMedESasp.asp

I've come home and found lobsters, clams , shrimp and home baked pies sitting on the front porch, from grateful family members who want to do something for us. Never got paid in chickens, but have been given a dozen fresh eggs. gotta love small town rural EMS

Bernhard: We use large type iii module ambulances which have plenty of room to carry two pt's and several providers. Better to have several crew working together on 2 critical pt's in the ALS truck than passing off the least likely to survive on the BLS crew. Doing it this way would allow additional crew to be called in from home to pick up and staff the BLS truck while you are off on a several hour trip to the big city hospital. We don't have auto respirators on board. Yes in an ideal world they would both be flown out to the trauma center by air, but the reality is helicopters are dangerous and not able to fly in bad weather conditions, which we have a lot of. quite often when they are requested they refuse due to flight minimums caused by weather, fog , snow, heavy rains, are all reasons not to fly. Hells bells: It depends on the type of ambulances you run. In a vanbulance or a sprinter type, not enough room. in a modular type III, plenty of room ,unless your trying to due thoracic surgery or birthing babies.

It's a scary world out their kids. Do a google search of you name and see what pops up. whether it's in a newspaper article , or sports event , or tv coverage of an event, or comments you made at a town meeting, the volume of hits tied to your name is scary. While I don't use my name here, it would not take a whole lot of sleuthing to find me by my profile. another site I do use my name. I have nothing to fear from anyone knowing who I am , or what my feelings or thoughts are on any subject. The only person I have to answer to is my wife of 37 years.
