  2. another additional thought is that there are many folks that just "sweat" heavily all the time, My brother in law who is now in his mid 40's sweats so bad that he has to change shirts and socks several times per day. I've seen him remove his boots and literally pour sweat out of them after 6 hrs of wearing them as a state trooper. Not a thing wrong with him medically, just an aberrant physiological process that he lives with every day.

Well from my past exposure to folks with similar signs & symptoms they have an unknown etiology of arrhythmia , so do we want to start the guessing game of which medication to use when we don't truly have an identifiable diagnostic rhythm? Cardioversion after a mild sedation is less invasive and more likely to cause less harm than choosing the wrong drug to maybe slow the rate. Good discussion Fiznat

Most builders purchase electrical components built by various vendors and installed on the line in the hope that they will all work together in harmony PL makes their own wiring harnesses and build the circuit control boards in house. each one is made for a specific truck on the build line. their electrical compartment is a thing of beauty to the eye of an engineer. With some builders it works and others are renown for electrical gremlins, bad grounds & things that stop working when you hit a bump and electrical drains that keep killing the batteries. We had one truck from a southern builder that every bump in the road would turn things on or off in a random pattern. Another the snow & rain came in through the door seals in the pt compartment. Never a problem venting smells in that truck. Another from a major midwest builder the cab to box seal leaked from day 1 and after several trips back to get it fixed ,it came back with a big rubber molding slobberred in silicone to fill the gap where the boy and chassis moved as it flexed Others are known for cracks in the welding of the body compartments and doors that need a 100 pound shove to get them to latch. How a truck rides is directly related to the heavier chassis we are now using. If you want a decent ride then don't spec a 14,000 gvw truck chassis. However with all the required equipment we have now, anything lighter is pushing the limits of safety and overload Balance of the weight on the chassis is important in obtaining a smooth suspension preloading. springs need to be loaded by static load to make them operate effectively , in conjunction with heavy duty gas shocks and tires rated for the load. There is a lot more to buying a new truck than looking at the shiny lights and stripes on the outside! PS Be Very leery of the new fangled touchscreen electrical controllers used in some builders trucks , nothing worse than waiting for the correct screen to come up so you can turn the emergency lights on.
  5. No connection to them at all other than after 40+ years in the business they make the best built and safest truck I've ever worked in the back of. When we were looking for a new truck in 2009 , I did a LOT of research and got information from the top 5 builders in this country. Comparing the quality of construction, total ambulance conversion warrantee package and best use of space., they were the winners. They offer a lifetime warrantee on the structural body, 10 year paint finish, 10 year/100 k mile electrical systems , and will renew the warrantee with a remount on new chassis. If I find a quality product that the manufacturer will stand behind, then I will gladly spread the word. By the same token if I find a product that is junk , I will tell anyone that wants to know about it. "Remember a satisfied customer is the best salesman you can have.   7. I already pointed him to the builder with the best warrantee in the business , bar none. They also happen to be located in his state and build a top quality vehicle at a competitive price. Ugly let us know what you think of the Demo & talk the salesman into taking you on a tour of their manufacturing facility.

  9. Ok Fiznat: Dwayne made me wait. :-} What was the Pt doing when this started? Has he had any previous episodes of this? Does he have any other co-morbidity's? diabetes, hypertension? O2, load up & head to ER, En-route Iv access with a little bolus, get him stripped down & dried off enough to get the pads on . Be prepared for this rate to change drastically. For now it's a wide complex tach with a perfusing rhythm and anxiety making him worse. Calm him down to the best of your ability and have the electrical therapy ready to cardiovert Might even have a go at him trying a vagal maneuver to see if it has any effect. Next :::

Once you've dealt with the dregs of society for years , with an occasional sweet little old lady who really needs your compassion and hand holding, let us know how it works for you. In reality you will spend most of your calls trying to figure out which agency needs to take charge of your pt. The corrections system, the mental health system or the local undertaker. My guess is you won't last very long based on the thousands of newbies I met over the decades. tough love is sometimes needed. It's time for you to open your eyes and have a reality check sunshine.

  12. Mobey: Like you I'm not into blowing smoke up a young-uns ass.. I come from a far different generation where corporal punishment was handed out in school and the nuns were to be feared. There were no excuses allowed and if one screwed up , you answered for it. Blunt & to the point. If they want warm & fuzzy they should get a job changing diapers at a day care!
  13. Be there EARLY, offer to help wash the truck or clean inside while the crew does their morning checks. Pay attention to what the crew says and depending on how busy they are take every chance you can to go to the bathroom. You never know when you might be running for several hours straight without a break to P. Leave your cell phone in your car, you are there to learn ,not talk or text on the phone. Keep your eyes and ears open wide and if you have a question about something you see, wait until it's appropriate to ask. Once a crew gets to know you a little they will give you plenty of hands on things to do. Carry a pen and a small notebook in your pocket to take note on or write down questions for later. God luck
  14. You will find that once you have earned the respect of the older members of the profession, they will gladly share the wisdom learned over many years of surviving in the field. We also don't suffer fools lightly, so when a newbie that goes by the name of DUDE posts an insulting attempt at humor at the expense of an unfortunate member of societies travails, then it's time for a BITCH slap. You will find we can eat our young in order to weed out the weak or feeble minded. And we can strongly help educate those young pups that want to learn and educate themselves into becoming the best providers they can be. Read Tyler's response above for an example of how a young member of the profession has learned and become an excellent example of a quality provider.

KNDOUG: Since you apparently have a problem with my expectation of intelligence and compassion for members of the profession I've been part of since 1971, Why do you feel it's not appropriate for me to relegate him to the senior level of burger flipper, that being the fry manager? A fry manager must have the knowledge and skills to be able to safely open a bag of frozen potatoes and pour them into a basket, all while not sticking his hands in the hot oil. Then they must have a degree in salting, not too much or too little. it's a highly skilled profession If the OP Behaves this way on an anonymous web forum, imagine what type of prehospital emergency medical caregiver he will be. I don't think we need punks , dudes or whackers to represent our profession. The balls in your court!

  17. Since you are in new jersey, take a road trip over to the shore , to visit PL Custom. They are located in Manasquan. www.plcustom.com I've been in EMS since the early 70's and have worked in trucks from just about every manufacturer in the U.S. and a couple from Canada. They are a family owned business and build in my opinion the nicest ambulance with the strongest body available. There warrantees are without question the best in the industry. We bought a new Medallion 170 type III last year. I went down to the factory to do the final inspection and acceptance, and was greeted personally by every employee from the owner Jean Smock on down to the janitor with a handshake and thanks for allowing them to build our new ambulance. Many of the employees have been there for 25 + years. Very impressive manufacturing facility from the welding & fabrication dept, the Paint shop to building their own electrical control systems all in house. I think the only thing they sub out is the upholstery cushions which are done by a local business. When we wrote our RFP and went out to bid , they were actually lower than several lesser quality trucks. They can build a truck to meet your exact needs and have a Ford QVM factory authorized warrantee service facility onsite.' edited to add PL website link

  19. There are many steps in the process: But first a little background info on your service: Do you need to follow municipal purchasing policy? If not what are your departments purchasing requirements for capitol items? Do you have a preferred vendor or sales rep? What type of set up do you need, ALS or BLS? How soon do you need the new truck in service? After we get these answers then we can go on to the next steps.
  20. If you really want to make a good living, you will look to another field of trade besides being an EMT-B. Burger flippers at mickey D's start at a higher hourly wage. Even the stock boys AT WALLY MART make MORE PER HOUR . Not being harsh , just giving you a Reality check. You will find that we don't spend a whole lot of time sugar coating things or blowin smoke up your ass to make you feel good about life.

  22. Mannitol is readily available and low cost. Most drug dealers use it as "Cut " I would think that someone in the city would know this. If I was the person receiving the E-mail listed several pages ago it would be photo-copied and distributed for every Dr. in the city to see. It would be all the reason needed to reduce your protocols down to "mother may I " for anything more than a simple band-aid. Unless you can write at an advanced collage level and learn to use a standard APA format for your writing, it will be ignored. These are just plain simple facts of life. Peer reviewed studies published in journals such as NEJM would be more appropriate than a vague reference to an article in the Bombay times. While your ideas are laudable , your methods are not going to get you out of the starting gate. Possibly enlist the services of someone familiar with technical medical writing.   23. We'll give you the benefit of the doubt as a newbie in the business Bieber. As long as you work for a transport company: Yes it is your job to haul people from one place to another. Did this PT have needs that weren't being met by leaving him alone at home? More than likely, but did you ask him if there were family members or neighbors that could assist him? Did you offer to call them for him? Does the daily home care aide help him with personal care and making meals for him? Does he have a wheelchair or did you offer to see about getting him one from the hospital? There are many ways we can advocate for our customers, however we cannot go against their wishes as long as they are capable of making informed consent. As far as refusing to "tech" the transport call based on your principles, :: You would have been standing on the curb looking for work in most places. Pick your battles where they will make a real difference , young Jedi!

  25. Really : You still have LP 10's. Wow , I guess it's just slightly behind the curve. We dumped our "10" over ten years ago. I guess the biggest "improvement " of 2010 was fully implementing electronic run reporting to the state.
