Herbie: May the good memories always remind you of your nephew. Don't let the bad overpower them. Prayers for you & your family.

I did get the message Dwayne: Just wanted to put a few concepts out there for them to get started on. There is a lot more to dealing with this PT. :-}

If a racist cracker redneck wants to behave this way it is protected by the 1st amendment. Same as the klan being allowed to hold marches and cross burnings with a permit for the gathering. Same as the asshat's from the westboro baptist church protesting as funerals for our fallen soldiers. Does it make it right ??? NO Do others have the right to protest against this cracker? You bet. Why? because the constitution allows us that right. As a good friend who is a police officer states " You can't cure stupid" as there is no law against it.

Seen it many times in motorcycle racers , jet skiers, snowmobilers & skiers. Severe blunt force trauma to the chest wall, usually at high speed and hitting fixed objects like handlebars, guardrails or trees. Most are not this obvious, you will generally see the paradoximal movement when their are bilateral complete fractures that are only stabilized by muscle tissue and ligaments. This guy is in deep doo doo. I'd be looking for a pneumo, hemo , or tension pneumo underlying and possibly cardiac contusions. Possible underlying organ contusions / lacerations are also a high probability. I'll stop here and let others play. Nice find on the video

The most important trick is to know your limitations , and your partners. 250 lbs is easy with close co-ordination with your partner. It's the 350-500 pounders that will be a challenge. Make sure you are both ready and lift together so one partner is not taking more than half the load. Use proper mechanics ie keeping your back straight and use your legs to provide the lift to the stretcher. If you're not 100% sure you can manage the lift safely, don't try it until you can get some more manpower on scene. Police FF's or another crew should be called for rather than taking a chance of blowing your back out or dropping the PT. Knowing when to ask for help is the best trick to continuing to stay healthy in this business.

FM: It's great that you are allowed to custom tailor different reports to suit the required info. We also use the field bridge which we had to pay for in order to put it on our laptop. We are given a standard PCR format that is used for every call from a simple lift assist to a multisystem trauma with very few shortcuts to avoid unneeded pull down menus. The states computer geek is working to make changes to the data fields but after several years it's still nowhere as easy to use as other software programs. It's just not intuitive in he layout of info needed to complete a report.. While I do have a degree in prehospital emergency care, I don't have a degree in geek and I'm way too old to have to get one now. When I went to school there was no such invention as a computer. We still used abacus's and learned how to type on an Olivetti. If they want to make it user friendly they should cross platform it so We could use it on an I pad or Ipod touch.

May they rest in peace. A terrible loss for their families & co-workers

You have my deepest sympathy! We had imagetrend shoved down our throats 2 years ago. It SUCKS. We had been using another vendors product for close to ten years and could complete a quality run report in about 20 minutes. Imagetrend is 45 minute minimum and is very user unfriendly. Terrible layout of the data fields, drug listing is not current, no medical spell check dictionary, no learn this word/spelling feature. As an administrator , you need a degree in computer sciences to make it function and generate any reports. Did I mention it SUCKS the big one ?????

Great video Phil ! We worked the Bathhurst race back in 1976 while we were stationed in Australia. Incredible racetrack, wonderful people! Real race cars turn left & right!!!!

To Expose or Not To Expose, That is the Question.
island emt replied to spenac's topic in Patient Care
First off We are not gynecologist or proctologist. start doing pelvic exams is a sure bet to end up in court. If the chief complaint is pelvic pain of unknown origin , I would ask appropriate questions to make a determination of whether there is bleeding, foreign object impaled, or is it caused by trauma. Is the pain sudden onset or has the cheese factory been fermenting for weeks. Any unusual discharges ? if so color and texture, smell or no smell? Is she pregnant? Recent miscarriage or abortion? If there appear to be no life threatening problems , then it's a transport in position of comfort and treat for shock or pain as appropriate. Same goes for a male pt. Treat immediate life threats and let the ER folks sort out the fungus. -
There are many software developers who don't write code for Mac users. It's their loss!

Let us also not forget all those that worked the pile afterward and are sick or dying in record numbers. -
9/11 - Where were you, what were your initial thoughts ?

On Friday November 22nd of 1963 I was siting in Mrs. "K's" 7th grade english class when the principle came over the loud speaker and told us to pray for President Kennedy as he had been shot in dallas. That was a very scary day. We had just been through the Cuban missile crisis and the cold war was a constant thought on our young minds. We did regular civil defense drills where they had us all go to the halls and crouch down in case of a surprise nuclear attack. The shock of the nation and the world at the news will forever remain in my mind. Those same thoughts came to mind all these years later when I heard of the hijackings and the attack on the twin towers in New York , the pentagon, and the ill fated flight 93. This was an attack on our country and could lead to someone unleashing the nuclear demon across the planet. Like many others I got a call from a customer who lives in Manhattan telling me about the first plane and then watched the news coverage for the rest of the day. Seeing the collapse of the towers brought the thought of how many brothers & sisters had died in minutes and the thousands of innocent civilians that had also perished at that moment. Later that night I got a call from a friend telling us that 2 close friends had been on flight 93 and gone down in Shanksville PA. They were in 1st class and had been part of the group that tried to take control of the aircraft away from the terrorists. It brought the emotion of the days events even closer to home and made it personal. Two really great people had died trying to do what was right. Linda was an EMT that we had worked with for years and Joe was the class clown that brought a smile to everyones face . We'll always miss both of you. Later we found out another couples son who we had watched grow up had died in the towers as he was assigned to the USSS detail there. George left behind a wife and young children.. Enough of my ramblings for now. Let us never forget those who gave their all that day 9 years ago and don't ever let up the vigilance to hunt down the terrorists who were behind this criminal act of cowardice! -
In my experience you made the right choice: Having worked for small mom & pop services that treated us like family members and then worked for a large multi state operation where you were a warm body to use and abuse, there is no comparison. As stated the big boys will use you up and spit you out at a whim. They don't care about an individuals personal life , only the bottom line of business. Realize that as a basic on an ALS truck, you will spend most of your time driving , take every advantage to learn from your partner. Good luck and enjoy your new position.

There was a study recently [can't find it right now] that showed with the increase in adipose tissue being packed on by many current generations , that the shorter needles won't reach the depth of penetration needed. Makes sense to me!

Have I forgiven the terrorist who slandered their religion to commit crimes against innocent individuals ? NOT on your life ! However I do not blame all of the islamic faith for the heinous deeds of a few. They were believers in a twisted version of jihad that is not the teaching of the Koran. I cannot find it in my being to forgive them for I lost two close friends on flight 93 and the son of an old friend at the towers that day. Will the presence of an Islamic center in Manhattan change that or affect me in any appreciable way? NO Much of the rhetoric about this story is the work of the racist right, the same politicians who voted against providing health benefits to all the surviving brothers & sisters who worked the pile and now are suffering from life threatening illnesses.

This takes the term "AMBULANCE CHASERS" to a whole new low point.! They search the web to find a possible infringement and then purchase the rights to the story so they can sue for copy-write infringement. Sorta like the land sharks that follow ambulances to traffic accidents and hand out business card to the victims. Question ::: whats three lawyers at the bottom of the ocean?? A GOOD START!

Mother software is different for each model. It is expensive and they don't want just anyone to have it. You can go to the batwing forums and check with the folks there, on availability of the software for your radio model #. It is a "M" specific site frequented by radio geeks.

What Mike said: We've been using the Rad 57 for 5 years, both as a diagnostic tool and as a preventative check for all fire fighters when they come to rehab for bottle change.. It is as easy to use as a regular pulse oximeter and has proven to be well worth the cost.

TX city revives the no paddling in school rule
island emt replied to akflightmedic's topic in Archives
As someone who went to school in texas in the 60's and was sent to the asst principles office more than once for an arse whacking I'd say that if more kids had their butts warmed there would be more respect for the teachers and a definite fear of misbehaving. All you had to do was look crosseyed at a teacher to get the finger pointing you out the door and down the hall. I remember one day as I stood in line and was around number 15 waiting, By the time my turn came he could hardly swing the paddle, he was just tooo tuckered out from all the beatings handed out that day. Nothing piss@d them off more than laughing at their efforts as juvenile attitude correction. Nowadays if a few parents handled disciplining their heathen children, there might not be a need for the schools to do it for them. -
This sounds like an issue that has been going on a lot lately in PA. These "private rescue squads want towns to give them taxpayers monies , BUT don't want to have audits done or have accountability to anyone. How many times in recent years have there been cases of theft , fraud and embezzlement by the officers of these squads in the news? I can think of several right off the top of my head. Too much of the good ole boy network not willing to answer to anyone.

Call to an ACF (assisted care facility... basically low level nursing)
island emt replied to Eydawn's topic in Patient Care
She's 100 years old and existing in a nursing home ! how much longer do you want her to live for crying out loud. -
Obviously it's not sinking into your narrow minded approach for some form of an answer. SO LET ME EXPLAIN IT TO YOU AGAIN! There are no,- zero,- nada, Advanced life support procedures that will change the outcome of a sudden aneurism that lets go. At best supportive measures such as applying oxygen might help ,but probably not. Most people that suffer from brain aneurisms die before they get to the hospital, some before they hit the floor. When the blood vessel blows out there is little chance of stopping the bleed before the brain is displaced and herniation occurs, which squeezes the brain stem and death occurs. Even if the person were in a stroke center laying on the operating table with a neurosurgeon standing over them , it is still a low survivability event. investigate all you like , but look at the facts . People die every day from brain aneurisms.

Have copies of all certifications and licenses EMS, Drivers if asked for. Have a set of work clothes in your car to change into if asked to do skills testing [ stretcher lifts, stair chair carries] Have a list of questions you want to ask the interviewer [ type of shifts, benefits offered, starting pay, uniforms provided?] Be confident RELAX & SMILE GOOD luck !
