  7. Not everyone can or should work 3rd shift due to the inability to roll your schedule around by 12 hours. I began working as a 3rd shifter in high school and have always preferred the nights. I can come home in the mornings and relax a little while doing chores and then go to sleep and get restful sleep in the daylight hours. Not all folks can do this due to family circumstances or noise or just can't unwind. Then comes time to get ready to go to work when everyone else is unwinding for the night. You go to work tired and running short on sleep so you are just a wee bit testy, and then someone winds you up. There have been many studies done over the years about increased risks of heart disease, stroke & early death in shift workers. Recent results trend towards high risks involved in upset circadian rhythm in causing long term illnesses. Maybe you are not cut out to be a vampire
  8. Ah grasshopper: if you look back you were welcomed with warm greetings and open arms, so to speak. You came here with a willingness to learn. Many newbies come here with the ink still wet on their EMT cards and presume to instruct folks with decades of street experience on how we are doing things wrong. They know , because they have run a dozen or more calls and seen it all. Most newcomers are given the standard "welcome to the city" greeting from several of us, and are invited to participate & interact.
  9. We had the LMA about a decade ago. We found that while insertion is easy , it does not prevent aspiration and is very hard to keep in securely place as your bouncing down rural roads. We changed to the King LTD as a more positive airway control and once inserted it stays put and prevents aspiration.
  10. I well remember the 1 AM sign off with High flight and snow after that until 6 AM Now you can have a hundred shopping channels and more infomercials than you can spend $19.99 plus $14.99 shipping & handling on. I can get 4 channels on a clear day and thats 3 more than I can watch at one time
  11. I had a chance the other night to turn on the TV. Watched Mash reruns for an hour and then put on antiques roadshow. That was my fix for the week along with reading a 450 page paperback.
  12. you are not the only one rich. We didn't even own a tv set until about 10 years ago when my inlaws brought us one after my wife was laid up with a tri maleor FX and spent 3 weeks on the mend

  15. When you say "basic education" are you referring to EMT-b or general education such as high school or community college level courses? Are there not enough expats hiding out there to properly train the EMS providers?
  16. Well the finger pointing is finally heading up the food chain in DC. Chief Ellerbe has announced he is" retiring" and thanked the mayor for having faith in him. :::: YAWN ::::: Here's hoping that they will hire someone who is willing to take the bull by the balls and get the DC Fire Department on the path it needs to be to provide quality service to it's customers.
  17. What type of compliance issues are you having to rectify? Is it paper trail or actual lack of staff receiving the required training and documentation of it? Usually your companies risk management carrier has forms and training classes available for OSHA,Dept of labor and some state requirements. You can also look at the offerings from VFIS. they have quite a few programs available such as driver training , medical documentation, infection control,& others.
  18. fully taken as meant systemnet. Thats why I asked him if he was working in an urban environment. Thats where I found a larger percentage of those on the edge of needing restraint. Cities seem to be the dumping grounds for the mentally ill. Still , even when I worked in a major New england "old city" we didn't have to restrain too many folks we encountered. Did we encounter folks with psychotic episodes ? Certainly all the time. We just used our charm and wits to keep them under control and compliant.

The one & only time I ended up in court over the outcome of a case was where the Patient died several days after while in the ICU of a major medical center well known around the world for it's expertise in trauma care.. The investigation was done over the families claims of improper care provided to a trauma PT. While we were investigated and deposed the DA's office found no wrongdoing on anyones part and refused to go forward as did the grand jury. The family filed a civil suit naming the event promoter , the sanctioning body, the track, The EMS crew on scene and the Emergency room that provided initial care before transferring care to a speciality hospital with a neurosurgeon in house. Typical shotgun approach to a civil lawsuit by a large well know personal injury shark firm, that advertises on TV a lot. Think Dewey , cheat em & how with a baseball bat. In the end because we had followed protocols and done everything by the book, the jury threw out the EMS and the ER staff from the suit. The shark had a copy of our protocol book in one hand as he was questioning us on the stand, and our state run report in the other, looking for discrepancies to nail us on. He found neither that he could use to his advantage. The promoter and track ended up settling out of court from what I heard later for a small amount that probably didn't pay the sharks fees.

  21. I've read several different versions of this story. All point to the "winning moment" for the teens lawyer as the fact they didn't have oxygen or a defib with them when they made pt contact. This was the first story that mentioned the cause as a "private" nurse giving the child a steroid injection causing anaphylaxis . I'd need more info , such as what the crew was dispatched for, ie difficulty breathing or anaphylaxis before deciding if the crew may or may not have followed protocols.
  22. No I didn't miss that statement. Thats why I asked if you were working in the urban jungle with lots of homeless folks or outdoor adventurers as one group called themselves. The fact that you are regularly tying patients down worries me . As several of us have said, we can count one a hand or two the number of Patients / customers that we needed to restrain. These cases were usually drug induced or stupid drunks wanting to do harm to themselves. Those that have responded to you so far in this thread have decades of experience in the EMS field, dating back into the early 1970's , so take our word for it there IS something wrong with the need to tie folks down on your cot.

  24. are you working in an urban jungle environment by chance? This is what we in EMS do on a regular basis. We cannot just "tie them down to my cot with restraints" as you put it without reason. That constitutes assault, unless medically necessary. I'd be surprised if your state protocols allow that to happen. You need to talk with someone other than your partner who may be on the edge also based on the duct tape comment. Even said jokingly that is just wrong. Follow ruffs advice and get some training. Like him , I can only think of a few instances were restraints were required and they were mostly for Pt's who had done angel dust or bath salts types of illegal drugs. I have had the police put a Pt in handcuffs who is actively trying to commit suicide by any means possible. Don't want them jumping out of the back of my ambulance while driving down the road.
