Ah, but the volunteers have an alternative. If they quit, government would be forced to provide professional EMS service. The medic schools aren't going to do anything different if you haven't been an EMT in the field. You end up missing out on a richer experience and the medic school still collects it's money and statistics for you passing your cert exam. But the people who really miss out are your future patients. -

Okay, I actually have to watch out, because I...I guess I'd say I have a high empathizing/sympathizing ability and high score on the self-introspection-intelligence rating. SO, it's very easy for me to feel exactly how someone else is feeling based on looking at them and having them describe their situation. There have been a few times I had a near syncopal (sounds got far away, field of vision narrowed, felt hot/cold, sweaty, light headed, needed to lie down immediately) while imagining someone else's problem. Once happened when I asked a friend to explain to me in detail how it feels to have a seizure...from aura to blackout (wasn't even a medical emergency...she said I was white as a ghost). Another time, person getting their haircut next to me fainted after he got a super minor cut from shears on his ear. After he came to, he explained how he used to pass out as a kid when he was cut (over a decade ago and not since). When medics showed up, I started getting light-headed as they interviewed him. I had to sit down. I've seen MUCH crazier stuff at work and have never felt this on-duty, probably because when I work, I'm concentrating more on what needs to be done medically and doing a good job than ruminating on the patient. It's usually disconnected. But I've found my solution is to just force a new thought into my head and concentrate on something specific.

At what age is a female not a child and an older man dating her no longer a predator? I ask this because I know a few, but specifically one girl, who are quite cognitive and understand consequences of their actions, more so than the older men who they like dating or flinging with. It's hard to think of the guys they date as predators, since nothing about them (except physical age) exudes innocence or ability to be taken advantage of, at least not anymore than women in their mid twenties. -
So, far all I can think of is some kind if -itis in the abd or meningities (but don't think I got my photosensitivity questions answered...Brudzinski's and Kernig's signs?...can't do all nerve tests b/c not following instructions...any muscle stiffness per parents?) or some kind of exposure (can we get more hx on what he did that day he stayed home alone). Maybe stool check ISN'T a bad idea....I volunteer Dust for this mission since he's more qualified. Umm...WBC count?

In LA, we use HAM (Hx, Allergies, Meds) for everything. When medics come on scene they always ask "Did you get a HAM yet"

I had a conversation with a medic about this yesterday, and he said it helped him by giving more meaning to all the topics discussed in medic school. He wouldn't be able to appreciate it (or even have context hooks) for all the new info and signs/symptoms/situations if he had never seen most of them and was just using his imagination. Since most schools don't have concurrent classroom and field training, having some experience as a basic seems the closest you can get to that. -

Ha, I spent my night in dispatch watching that yesterday. You know, I'm not from a small town, so I couldn't say for sure, but yes it doesn't seem like a good image of someone professional who has everything squared away and above issues like this. It reflects his work quality too. -

Looking for new tricks to play on new hires

If you do it, you do it because the guy's getting along with everyone well and he thinks it'd be funny, too. You don't do it simply because he's an easy shot being new. Nothing that's going to make him feel bad about his new position or question his being there. I'm pretty sure we've had trainees leave our company because of stuff like that. -
Oh and how does his condition change after O2. I'm to decide what to do with airway/breathing. Would 12-lead (enroute) be too much? I'm just thinking that b/c obviously not perfusing well. I'd start loading now.

Well, Johnny took that BP, and he took it most precise, The systolic over diastolic, palpation would not suffice, He stuck that needle in the arm, watched that line flash red, Then waited until, "You've got good flow" the Basic said Well wouldn't you know that girl was cured, cause Johnny was pure, cause he's the best medic there ever was. PS Don't hand her your EMT Pocket Guide and ask her to read you the steps to RSI...

Avian flu. I'm outtie! How is abdomen on palpation? What did his 'stomach' flu consist of? Cough? Pain anywhere? Soreness? Photosensitivity? Sore Throat? Fever (or is this new onset)? Diarrhea? Vomiting? Eating/Drinking? Any semi-recent injuries?

Interview Parents: What more can I get out of them? What was he doing prior to to this? What made them get up at 4AM? Was he crying out? What did he do yesterday and days before? Trips or outings? Has this happened before? Normal mental status and ability? Get a SAMPLE. Partner Can your partner do a scene survey of the room, kitchen, garage, and anywhere else they might find toxic cleaning substances, etc? Any smells in the room? Are medications all accounted for? Assessment Now that you're at his side, How is his airway, breathing (rate, quality, lung sounds, visual), circulation (pulse rate, quality, cap refill, PMSC of extremeties), GCS, pupils, BP. Do we have monitor, O2 sat, finger stick, thermometer. Physical exam, signs of trauma or anything else, signs of "SLUDGE"? Will he follow commands enough to tell you what's wrong or point where it hurts or get more of a neuro eval? Tx: At this point, [high] O2 and get IV ready, I suppose.

I wanted an obvious example where someone stated a fact, but it still didn't justify saying because of what it meant (harassment in the example I gave. And demeaning [in my opinion in] the reply to Larry in this case). It's not even that big of a deal, but I was trying to address the issue that's been brought up a few times about not resorting to harshness with others.

Yeah, Larry's post, right? I was just getting that vibe of correcting behavior with a hint of superiority the way he posted. Just remind him of grammar....unless it's an ongoing problem with him and I missed some hx. Maybe there was none and I was imagining it.
