I would disagree with your belief that publication equals replication. Actually in the scientific community one of the major failures today is that there is almost no replication happening. There is a lot of money for original research, for getting your name in the paper for your school or company, but when was the last time you heard someone get famous because they duplicated someone else's breakthrough? Publication gains a lot of money for schools and scientists, but creats almost no verification. Also, I think you misunderstood my original point. Does it sound as if this is worthwhile area for study? sure. Anything that will increase functionality of those with spinal issues is a good thing. I wasn't dissin' the study, but the reporting. I absolutely abhor statements such as "The mice that were injected with the die walked again." All of them? Immediately? No serious side effects? Bullshit. I hate these articles because ignorant people consider them "knowledge" when in fact this article is no different than reading about Sasquach (sorry ak) in the Enquirer. It's a bunch of sensational nonesense that will get many desparate people all excited that perhaps there is a cure just around the corner. See what I mean? I don't really have any opinion of the research with the exception that I have little respect for a scientist that sits for interviews with idiots. Good to have you back here girl! Dwayne

Cute mouse? Sure. Useful article from a medical point of view? Not even a little bit. I'm grateful for the article, truly I love to see interesting new things, but it makes me a little bit batshit to see sentences such as, "When the die was injected into mice with spinal damage, the mice were again able to walk, those without the injection were not." (not a direct quote) That tells me that the rest of the article is just full of useless crap. If the scientists had been allowed to edit the article I'm guessing that he would not have allowed such a statement. All of the injected mice walked again? They all had a limp? None of the uninjected mice walked again? Bullshit. If they all were fixed with injections, and non regained sensation without them the entire spinal cord world would be in a tizzy. They conveiniently left out percentages because there simply wasn't anything to write about if they were left in. Basically, it's an article about a blue mouse.. Thanks for posting. This rant will hopefully in no way convince you that I believe you shouldn't have posted, only that I really, really hate pseudoscience reporting. See? Your curious mind wanting to share? Awesome! How so? In my basic class we did, I think, about 160 hours. How is the DOT limitting his ability to make strong basics? I have heard, and now found for myself, that often the more experienced basics have the least value. The vast majority are under educated, though because they had been behind the yellow tape, soon felt that their experience trumped the education of others, giving them value that they didn't truly have, and respect that they had never really earned. As Dust has asked many times, if experience before knowledge is a good thing, then why is EMS the only medical body that does it? Now if what you are saying by "better basic" is that the cream of the crop of basics, those educated well beyond necessary minimums, professional, hard working, healthy, that if they were to become I's and then Medics that that would be a good thing, perhaps I can get on board with that. But they, in my experience, are terribly, terribly rare. You are absolutely right. I meant ignorance and education. Certainly a lower certification doesn't denote inferior intelligence any more than the reverse would be true. It was a silly error, thank you for giving me the benefit of the doubt. Ok for now, but I can't promise for ever. I see your point, but don't agree that it's valid from a cost benefit point of view. If you could create a Physician in as little as three months? Absolutely I would think that only Docs belonged in ERs. How does having a less educated nurse there improve pt care? I can only see one way, and that is because there are duties that some begin to feel are 'below' them, and others, if educated less, and paid less don't feel the same. Can a nurse clean a bottom, document, administer medications better than a doctor? Certainly not if we hold to your 'everyone is competent' rules. It doesn't fit, where I can see, in the EMS that I have worked, again, simply because I haven't worked with medics that are 'too good' to do menial labor. When the basics I work with now tech a call I do all of the basic work. Clean and restock the rig, make the cot, chase down the necessary paperwork from the office...I have no issues with it nor have I seen any sign that the other medics do either. My first 4 weeks at this service I worked with another medic full time. When I teched calls he asissted, as did I on his calls. The only side effect that I noticed is that he and I both wanted the rigs cleaner than any of the basics normally kept them, and that each pt had the benefit of two educations and streams of experience instead of one. (Meaning somewhat more advanced educations/experiences)Well, that and the fact that if I wanted narcs he had them avail and loaded before I needed to call for them. What skill set does the nurse bring that the Dr. doesn't posses with the exception of a lower educational level and the willingness to work for less money? What else is she capable of that the doctor isn't? I just got done breezing through your response and am surprised by your arguments, in a good way, though I think that they are mostly incorrect, I'm truly shocked by the parts I believed to be logical and intelligent. Not because I didn't think you could make such points, but because I didn't believe ANYONE could make such points on this topic. No time to give an appropriate reply now, but will do so ASAP. Yeah, I get some of that, and having reread your posts I guess I can see where you're coming from, don't completely agree, but I think I get it. So, if no one objects, in the spirit of restarting the thread, how about if we start again here?: If this has been argued, or requires a new thread then that's ok too. I don't understand your balance. I don't know your cert level, but I can't imagine that you're a medic. Not because I don't think you're smart, but because if you were and had worked with other medics I think you'd find very few medics that have similar strengths. At my last job I worked regularly with a rotating crew of 6 medics. We all felt like strong medics, but none of us excelled at the same things. One had near xray vision where bone and muscle injuries were concerned, another loved derm issues, I tended to be strongest at following multi system physiological disease stuff, and on and on. We went to each other daily for 'second opinions' and learned from each other constantly. Also, I would never allow them to 'best' me if I could help it, and they felt the same. I grew stronger there in a few months than I expect to do here in a few years. So I guess my initial argument, without having spent a lot of time thinking on it so far, would be, how is combining ignorance and (hopefully) intelligence considered balance? I lift every pt, often making what I consider significant improvements over my Basics ideas for doing so. How is his lack of education an advantage to our team? And I'm not clear how a more advanced education is somehow a detriment to proper lifting? I'm just not seeing it so far. Perhaps you and I can do better with these issues... And in reference to our previous conversation. I can't really get what you're saying, and I see the same from you. But I do believe that one of the things that are lacking here are examples of good arguments. Arguments and debate are valuable skills, and I believe EMS would be much stronger if people could do it without anger. I don't see out previous dialog as wasted time, would be my convoluted point. I'll try in good faith to break this down for you, on each of your points, but have no faith that you'll get it. Here goes... Bullshit half statement. You state that basics are not better than medics, just systems with more basics and less medics. It seems to me that this statement speaks for itself, I'm not sure which part of it you're not getting? More basics and less medics means that, at least in some way, having more basics is superior to having all medics. Right? Perhaps it's possible that you mean that there is simply something pathologic about medics being in constant contact. I can't speak to that, though I have worked in such a system and found the opposite to be true, but will truly think about it, perhaps you're right, if that was your point. But we have no idea what part was your point and what part your 'clever trap,' so, how to tell? Yes, it certainly is. But this is where I believed I began to see your arrogance. Why not simply man up and give YOUR opinion, an opinion that you would have to stand behind and be responsible for? Why simply give 'an' opinion, one that you can run from later by saying, "Ha! I didn't really believe that! I was simply trying to educate the children!" I have no respect for your education when you haven't shown to be brave enough to have an opinion. But that's just me. I use the word 'retarded' regularly, just not to identify people. Retarded: To cause to move or proceed slowly; delay or impede. (www.answers.com/topic/retard ). The same as I refuse to stop using the word 'queer' simply because some have chosen a new, inaccurate definition. Some things or processes are retarded, or delayed, and some things are queer, or unusual. I disagree though when you state that retarded is a term you'll heard used in professional circles to define individuals. For instance, if my doctor said, "Dylan's social skills seem to be retarded." it wouldn't so much as cause me to blink. But if, on the other hand he said, "I can see Dylan is retarded because he has limitted social skills." I would immediately get a different doctor, as the use of retarded to label a person instead of a trait is very much out of favor with those that make their living with such people. Do all of these professionals choose different language because they are slaves to political correctness? No, simply because it's terribly rare to find an entire person that's 'retarded.' Traits or behaviors? Happens all the time, I have many such (some might say most), traits. But people? Without a history you simply describe your pt's behavior. Though an error I see many make is to assume that because a person has an abnormal presentation that they can not provide significant history for themselves. The vast majority of mentally 'different', (not trying to be PC, just accurate), have been fine historians for themselves. Might keep that in mind. If I do have a history I still document behavior that leads me to believe that the history is accurate. "Pt recoils from attempts at physical contact, eye contact with medic or others in the room near completely absent, pt exhibits repetitive rocking motions/hand flapping consistent with reported history of Autism." Or the like... It's all just what you see/hear/feel/are told...if you don't know the diagnosis, don't guess, just give enough facts to show that you attempted to do a complete and through examination to include attempts to offer appropriate emotional support. Other than being undoubtedly one of the most intelligent medical people on the City, she's always seemed to be committed to posting with an efficient, intelligent use of structure, spelling and grammar. Yeah, I can see that. It's just that that is a great goal if they also give you the tools to succeed at that goal. To apply that goal to newer students, to a skill they are uncomfortable with is simply bad teaching. Training animals is truly no different than training humans. You teach a skill, piece by piece, and once the skill is mastered you begin to "proof' it by adding stress. A new student can end up with an abortion of motions that may get some tubes in place in 10 seconds, but it will be a terribly rare student that will retain techniques for getting a majority of verified tubes using such a goal as motivation. Sorry man, it's just the behaviorist in me rearing it's ugly head. Doing intubations over and over as one skill will create habits, but rarely good ones. Practice doesn't make perfect, perfect practice makes perfect. Students need to slow down if they want to speed up. Thanks for your reply. Dwayne

I'm not sure why you're timing your intubations, or who taught you that this was a good idea. Stress is absolutely the worst thing that you can add to a new skill and those that are telling you that you should be doing things faster simply want to seem good, instead of be good. Esse quam videri. Any new skill should be done slowly. Practice each part seperately until doing it doesn't distract you from the act you will do next. Check your equipment and supplies. Pick your blade, install it, check the light. Take it apart, put the parts away, and then start over. Do not move past this until you are no longer uncomfortable with it. Then check your tube. Attach the syringe, inflate the balloon, inspet for leaks, deflate baloon, lubricated, set in a comfortable position. See? When both of those come naturally, then put them together as a set. When the set is comfortable, practice your next skill, position the head. Once that is done, add it to your "daisy chain" of behaviors, never adding a new behavior until the previous links of the chain are rock solid. Before long you will do all of the mindless stuff mindlessly, even under pressure, leaving your mind free for the many parts of pt care that are not mindless. Get it? Right now it sounds as if you consider intubation "a" skill, when in fact it's a chain of many skills. Right now you are thinking of things that don't need to be thought about. You can practice much of that mental constipation away, but only if you break it into parts and practice the parts before attempting to unify it as a whole. Give it a try, I think you'll see a difference right away. Good luck. Dwayne

What reasoning exactly Anthony? And what are the "many factors?" That he's different and spends time with kids? Serious question, not trying to bust your chops. What information do you have that didn't come from the popular media? Are you accessing evidence that wasn't put out to the public? Perhaps you're friends with one of the investigators that was forced to withhold some valuable information at trial? What superior logic are you using? And if the superior logic is that the addition of one 'reported' fact plus another 'reported' fact equals guilt, how can you then claim to be using superior logic when historically those reported facts have proven to be bullshit? For example. It's been reported that Jackson had children in his bed. Having had Jackson's head of security at Neverland Ranch as a personal friend for many years I can tell you this is certainly true. Yet what was not reported in the media was that Jackson's bed in the master bedroom was custom made and was approx 30' x 30'. It was not uncommon for he and the kids to simply fall on it with their own blankets or sleeping bags at the end of the day. If that makes him a pervert then I must certainly be a pervert too, as Dylan's bed is much closer than 30' from mine. I have, at least dozens of times, fallen asleep on my living room floor with my nieces and nephews and sometimes their friends at the end the night yet have never had to fight the impulse to touch one in an inappropriate manner. "Correlation does not imply causation", you should look that up. But see, simply allowing people to draw extreme conclusions by only reporting part of the facts is much more interesting than the truth. Anyway, there was a time when post after post of such silly conclusions would not have gone unchallenged regardless of the subject. Are those days at the City really gone? It would appear so. Dwayne

Amen man. I guess that was my point before. It's discouraging to see the ignorance in the statements such as, "He must be guilty or he wouldn't have been arrested!" Or "He's obviously guilty because he paid the family off!" The complete lack of critical thinking, use of historical perspective, or even blind adherence to "Innocent until proven guilty" that leads to these comments is so far below the level of reasoning and fairness that we used to sponsor here that it's heartbreaking. Yeah, a fantasy land for terminally ill children, what a beast. In my world "paedophile" is pretty strong language. I don't use it to describe those that have not been convicted of such a terrible crime. For you to convict anyone of such a thing based on some bullshit news stories is idiotic and disappointing. I started a thread on this a few years back I think when this article was new and a Brit replied with some intelligent comment and paper to back it up if I remember correctly... S/he was frustrated at the bad press and hated the fact that people seemed to believe that they would "choose" to ignore such calls when in fact they were obligated to ignore such calls. My point being, to our British friends, couldn't this medic have been sanctioned for taking this call during his meal break? I find it interesting that many of the same posters here claim that they wouldn't perform a lifesaving intervention if there was a possibility that it would jam them up professionally, but that this medic should ignore his protocols because it's "lunch." So is the general consensus that if it's lifesaving medicine, it's moral, ethical and logical to sit on your hands to protect your income, but if it's "just lunch" then you should be willing to endanger your professional status? Nah, tells me that 'they', the family and newspaper, considered the baby alive as long as CPR was being performed, and dead when and where it ceased. hopefully that is not your feeling as well. As far as working a "compassion Code", that was proven harmful to survivors about a gazillion years ago, wasn't it? And I can imagine a scenario where their very poorly written PCR could be accurate. For instance story never states that CPR was discontinued by the cop before being taken over by the supervisor. Right? I'm really not defending the crew, I simply hate giving the newspaper the benefit of the doubt. Not enough information here to even argue about really...
