  3. My guess is that not being a doctor you didn't see the need for the test, that's pretty much why we have doctors instead of hospitals run by Basics, or even nurses. You seem to be under the impression that unlike us, doctors have no need to justify the care that they provide, and are then able to send near dead people willy nilly around the hospital for tests to run up their bill? Not really trying to bust your balls Brother, but that's a pretty bold statement, and accusation, from a basic that states to having only a few minutes with a patient before determining that she's irrevocably terminal. Unless you simply accepted the nurses diagnosis as your own? Because she had cancer, then you've assumed that that's all she had? Were you able to perform a stroke scale on her? Perhaps before a new onset CVA she didn't have the life ending respiratory issues? DNR means Do Not Resuscitate, it in no way means Do Not Treat. Because you've determined that her life had no more value at this point doesn't mean that she should be left to die without proper and appropriate care.
  4. Man, great scenario. I find it interesting that they give that description, upper and lower extremity weakness, etc, without a reference for the location of the cord damage? I'm trying to remember why the location wouldn't be relevant without Googling. I've got a tickle, but can't really resolve it in my head. What confuses me about this is that though I know many people, and love most of those that I know, I know no extremists. So why do we choose to argue these points only from the poles, and not the equator, where most of us philosophically live, so to speak? You should probably figure, depending on the population that you intend to serve, if you can get such a population which without political friends is unlikely, from $1Million, to many many millions for a start-up cost. There is not a single thing that is involved that isn't very expensive. Buying the ambulance and stocking it is coffee money compared to the rest of the costs mentioned above. Not trying to stone your dream to death Brother, but trying to be realistic.

  10. I think that this has been a productive thread. I can see no reason to lock it. He's chosen to post, I can't see anywhere where he's hurt himself in his statements, nor where the statements that follow have hurt him either. The post sounds to me like it was created by an adult and out of respect for that I'll consider locking his thread should he choose to have it locked. I won't limit his right to participate based on what others think is best for him. I find him to be alert and oriented and choose not to treat his threat without his consent....so to speak. Because some of you have given what you consider to be the best and ultimate piece of advice and can't imagine better advice being provided doesn't mean that there is not other, equally or more valuable advice available.   11. Man, I loved everything about that article and the spirit that it seemed to be written in. She's simply expressing the sentiment that many of us have expressed here before. The only shocking thing about it to me, at least at first when I compared the title to the picture, was the the picture wasn't of a fireman. And I think having that initial thought validates the content of her article. I've never been thanked for being a paramedic by someone that I'd not cared for, for the exact reason that she mentions. Because I didn't show up in a fire truck, no knows exactly what the hell it is that I do. They know I do medicine on accidents while helping the fire dept, but if I'm on my own without a fire truck, can I actually do anything? (speaking of the looks that I'd get in the store etc, while in uniform.) I've been hugged by many patients, of many nationalities, from all over, and it's never struck me as strange at all. It seemed like a natural conclusion to the treatment. I choose to put my heart into treatment and people normally, I think, want to touch and hug when they feel a lack of barriers. Do I accept thank yous graciously? Yeah, of course. I'd accept it if I'd stopped to change their tire on the side of the road, helped to carry groceries, and I'm certainly going to accept it from someone who's hand I've held while they were scared. Do I deserve it for being special? No, but they want to give it as it helps them believe that we're both special. It made me, not emotional, but I know what you're saying DFIB, it felt really good. The whole article had a great energy to it I think... Kaisu mentions an ego driven slippery slope. I think that we sometimes bounce back and forth between, "What the hell? It's not only firemen out there! We work here too!" And "Attention and thanks? I don't need that shit! I'm a medical purist that chooses to do what I do in a vacuum!" I'm calling bullshit on both extreme views, and for myself am saying....fuck yeah. That article felt really good, I'm glad it was written, I think it was productive and I'm going to have a better day for being one that participates in the occupation described.   15. I Haven't had experience in this but I have a ton of respect for you as a patient advocate. First, you can't take an alert and oriented patient against their will, not ethically anyhow. Second, I'm aware of no place that the police have any right to tell you to do ANYTHING, but perhaps move of of the way. If the police were confident that a crime had been committed than after you'd found her to be medically safe and competent then they should have transported her to the hospital for evidence collection, or convinced her to be transported by you. But in no world did they have the right to tell you who to put into your ambulance, or why. It sounds like they were pissed because you didn't transport for them and felt the need to spank you for that. If your story is accurate I'd speak to a lawyer not only about your arrest charges but civil rights violations as well. If it was me I would make it a hobby to make this issue as big and ugly as I could. I'd be grateful if you'd keep us updated... Dwayne

You know, I just received my company review, and without speaking with any of the people that I'm tasked with caring for, in fact not a single person in any role on my site, it was determined that "due to your near constant bad attitude and unwillingness to make any effort at cultural sensitivity it has been decided to extend your probation period." Yeah, whatever. But that got me thinking about the quote below, which I believe is absolutely true and I also believe that I see it happen often...

  18. Malaria? I don't know squat about xrays, but I'm wondering if that doesn't show a compression injury, the pressure from which maybe would explain the bilat symptoms? What confuses me about a spinal injury creating bilat, though only partial paralysis symptoms is that intuitively I wouldn't expect the symptoms to be mirrored unless the damage was exactly the same to each set of nerves, which seems unlikley? So, bottom line, I'm going with Malaria....
