Yeah, I had no issues with her picture as it's obvious that she was posed for it.... But in answer to your question, I don't think that I've ever worn gloves in the cab. I put mine on on scene when I worked the streets, and now I usually have quick access in the clinic, but if I'm in the ambulance I put them on enroute due to having so many unfamiliar as well as familiar diseases in PNG.

Kreamer, thanks for your response. And I certainly get what you're saying, but disagree. But man, I'll tell you what, I'm so grateful that you made your argument without statements such as, "It's in our blood. Helping is what we do no matter the danger to us." But lives should never he held less valuable than public opinion and/or property. We can hypothesize that these men were fighting a fire to aid in an extraction, at which point I would be more on board, though there's still no reason really for them not to have gotten naked and into their turnout gear. Without lives at stake I can't really imagine a scenario where this video didn't happen because of adrenaline and machismo instead of logic and professionalism. I'm all for recognizing those that have given their lives in the persuit of doing good, when it was done in a professional manner. But not so much for every yahoo that chose a rush over good sense. Not sure if that's right or not... -

ITC, Not mocking you, truly. I did get a little bit frustrated with your attitude towards the suggestion of an anxiety attack, and I'll explain why. I've dislocated both of my knees, broken both of my elbows, broken some fingers, ruptured two discs in my back and had at least two concussions. Yeah, I grew up redneck. But nothing in my life compared to having an anxiety attack, and I've seen enough in others since to know that they've all felt the same. During medic school I had an anxiety attack and it is truly the most horrifying thing that's ever happened to me. My chest hurt, I couldn't get my breath, I was praying for the ability to breath faster and deeper yet couldn't seem to get any air to move in and out of me. There was no question in my mind that I was in the last few moments of my life. Unfortunately this happened to me in the middle of a paramedic class, (Imagine about 100 monkeys trying to hump 120 footballs and you'll get a decent picture.) so despite tons of things happening really fast, I can't really rate the treatment as great. We were playing at some type of scenario, I wasn't involved, wasn't even nervous. In fact I was having a great time. And suddenly thought that I was going to die. I certainly wasn't panicked as I wasn't then, nor have become since a big panicker. In the situation that you mention it's truly not uncommon to feel a shortness of breath, begin to breathe faster to compensate, and faster still when that doesn't resolve the problem, and end up breathing fast enough so that it becomes part of the problem instead of the solution. See? I would have been more receptive had your response been something like, "What makes you think that? Do you believe that the doctor could have been mistaken? How come?" Than "It was not!" It appeared that you were offended by the thought, and there's very little educational value in being offended instead of curious. I'm confident of one thing only in your case. That being in that condition sucked. I must have been terrifying, and I'm truly taken back by your statement that the medics treated you really well. People much more often than not, when in pain and afraid, misunderstand so much of what happens in the back of an ambulance so that the medics are very often seen as clumsy villains instead of medical professionals doing the best that they can will very few tools at hand. If you felt that it was my goal to mock, or embarrass, or belittle you, then I'm truly sorry for that. My goal was to try and force you from a point of view where you seemed certain of the error of a possible diagnosis and push you to explore instead of simply deny. I'm glad that you're here! I hope that you'll stay. There is not requirement to be involved in EMS, only that you have a desire to teach, and to learn, and to explore. You have bigger chops than many medics I know, who would have turned tail and run away at the first contrary comment. It would be my guess that being a teacher is no less hardcore than being a medic sometimes is. Have a great day girl. Thanks for looking after our kids! Sorry Brother, as posted in the welcome thread, any effort to gain money for such a project will have to go through Admin. webmaster@emtcity.com It sounds like a possibly valuable project but I'm afraid that you've joined only for the purpose of advertising it, and that makes your post spam. We've no idea who you are, or the actual value of the game. The City's admin has always been a strong supporter of education so perhaps if you're willing to make the game available for review from some of the more experienced members on the board we could give him an opinion, otherwise please contact the email above and go through the proper advertising channels. -
Hi Tray, Welcome to the City! Is there a charge to play your game? If not, is it a money making endeavor for you in another way? If not, then I see no issues with you posting the link, even if it's something that you're developing but plan to make money on at some time in the distant future. If so, then you'll need to approach Admin directly for that decision. I certainly do appreciate the respect that you've shown by asking permission before doing so. Hey Doc, What would be the physiology behind the dyspnea secondary to sepsis without vasodilated pressure issues? Or perhaps there's not an answer there that's easily explainable at a level I'm likely to understand? OK ITC, I take back the anxiety attack comment. It's obvious that you're unwilling to consider any possible complications that you've not seen on House. No sense muddying up the thread with that conversation. I will tell you this though. If you're an EMS student, you're really, really far behind the eightball when you reject a possible diagnosis because you don't like how it sounds. Particularly when it's obvious that you have no idea what I'm even talking about as it relates to you. In EMS as in all other medicine you follow the signs and symptoms where they lead you, not where you wish that they would take you. And despite what you may believe, reading your discharge papers at your level of education can make you more confused than you might imagine. Google can be your friend if you also have the education or experience for the necessary context. A good example of this is your claim that I stated that your dyspnea was secondary to 'panic' when what I theorized was a panic/anxiety attack. (Most often I would use the term 'anxiety attack' when relating to other professionals, I added panic as it's a more common laymen term.) The two conditions are, except rarely, completely unrelated. But, as you find it to be such an incredibly vital point, I will from here on out consider my theory thoroughly disproved. -

Had you ever had sepsis before? Or PP pre eclampsia? Or PSVT? Have you had any of them since? Yet you had them in unison, right? Why is it that a possible anxiety attack is the only thing that you find just to outlandish for belief? It matters not at all to me if it was dyspnea from retarded stroke volume secondary to PSVT, an anxiety attack, or the after effects of being rudly probed by aliens. The symptoms as you've described them here leave plenty of room for interperetation, and that's all that I've been doing. You seem very committed to DOB from PSVT. Ok. Whatever happened you were certainly a very sick lady. You want it your way, then good on you. But until one of our docs or super cardiac folks chimes in to comment, then other explanations exist. This is a learning forum for medical professionals. Taking things apart and looking at the pieces is what we most often do. No one is trying to take away any of your cooler sounding pathologies. That's why I asked the question above about rate retarded stroke volume. 225 seems pretty slow to me for the conditions that you describe. I have a gym rat buddy that has a rate of 200 when he pushes his cardio during his workouts. You sound like you're getting pretty defensive. You need to stop assuming that you understand medical conditions that you don't. I never even implied that your entire issue was psychologically related. You sound as if you've come to believe that you know exactly what a panic/anxiety attack is, as if it's something that happens only to weak girls. You're very much mistaken. Some of the strongest people that I know have had them, myself included, when put into weakened conditions from stress or illness. You should try not to deny it, with an exclamation point, simply because you don't like the way that it sounds. I was under the impression that you wanted to discuss this issue but it now sounds as though what you're really looking for is validation of what you already believe happened. Either way is fine. But you're going to find that participation in supporting your likely misunderstandings is going to be pretty limited I'm afraid. Dwayne -

I don't see really how this call was mishandled, and depending on several criteria almost certainly running lights and sirens would have been inappropriate. As Mobe's said, there's no good evidence to show that it would have done you any good, but tons of evidence that it greatly increases the odds of the 'ambulance drivers' killing you and/or others. If all of this truly came on in a 10 minute span of time, and a PE has been ruled out, then the hyperventilation was likely psychologically induced and responsible for your feeling starved for air. But without significantly more information that we've got here it's impossible to know for sure. I've never heard of a fever that goes from normal to 104F within a 10 minute time frame, but there's much that I don't know about fevers. Though I'm confident that it came with the sepsis and that the sepsis didn't occur strictly within a 10 minute window. The thing that they were having you blow into was likely a syringe or something of the like in an effort to vasovagal your rate down. A reasonable intervention in my opinion at that rate. To the smart people on the board, I can't remember, but is there a relatively specific minimum rate where we can predict to within a reasonable degree of certainty that a patient will become in some way symptomatic based on retarded stroke volume alone? A couple of questions. What did they ultimately diagnose as the cause of your tachycardia? If you were altered to the point of hallucinations, and have no real experience in EMS, where did you get the history of the things that happened in the ER? I can tell you that I have stayed in the room with every patient that has confused me in any way when I could do so without delaying another call or being in the way of the ER staff. I've never had a doc tell me to "Stick around, maybe you'll learn something" but have often been invited to stay, and only once been criticized for it. It's common practice at every level of medicine to try and learn from our betters while handling real patients whenever the opportunity arises. Interesting call...thanks for bringing it to us. -
Yeah, @ DFIB, you've based your treatment so far on the stated diagnosis of someone that you don't know, and who might be, in fact, is likely, wrong. What the hell?? :-)

I think that most of these plans, though really good, also sort of have zombie apocalypse written all over them... I've got 12 gallons of water and about two weeks worth of not easily perishable, but also not very healthy food, 'just in case.' We camp pretty often and have found that with three of us drinking, washing, and cleaning dishes and such that we rarely go through one 6 gal container in a 4-5 day stay. The one thing that I don't have, and this might be area specific is an electric generator with extra fuel for running a small heater while the power is out. Because of Dylan's autism though, I do have about $200 worth of assorted batteries to run all kinds of electronics until someone cleans the shit off of the fan. Great lists!
