Usually, go back and get some more classes. Those professions Dust described usally, requires a B.S. but, some will allow to enter dependent on portfolio or allow them to get addtional classes. I have some that graduated from Regents, that was required to research statistics and other courses. Good luck, R/R 911

I will not continue to argue with such ignorance of specifics, as I expected, you were discussing basic nursing not advance practice or clinical nurse specialty. or even have the understanding of such. I was in a P.A. program, and quit because of the lack of speciality education or lack of. That is why they cannot practice medicine on their own licensure, where as NP's can. Nursing care plans are usually only related to generalized nursing practice, by even addressing these, you have again proved my point. I am in ACNP program extension and quite aware of philosophy, research, requirements. I am quite aware of comparative models of medical education levels and actually have really performed literately. scientific research, with comparison analysis, so google and "ask how" does not really impress me. Just like the hundred others "I'm going to be a Dr. someday".. come back, I will be glad to discuss with you after you get into medical school. Sorry folks for the hijack, and lame discussion...We now return you to your scheduled discussion... R/R 911

BLS report requirements on ALS calls
Ridryder 911 replied to Eyegor's topic in General EMS Discussion
It is medical charting and documentation. Remember, the purpose is documenting is whom, when, and what was found & performed. This includes all agencies and representing staff. This would be like no charting done in ER if the patient went to ICU ... R/r 911 -

Because you posting erroneous information. How could you take a semester of NP without being a RN/ B.S. /BSN first ? First semester of a nursing program ADN or even BSN is a lot different from advanced degree. In the first semester of general nursing, you are taught the general "basics".. nursing dx., etc.. This would be comparring the same as 1'st responder versus Paramedics education. Second, Paramedics cannot go to NP school without being an RN/BSN or at least be a RN with a BS. degree, or entering a RN/MS program prior to admissions. Then most require at least 5 yrs at a RN level/ You see they really don't care if you were ever a Paramedic. Believe it or not most health care professionals still don't recognize Paramedics as a profession yet. Second there is not a generic "medical model" as you described. Dependent on the "specialty" of the Nurse Practitioner program. Medical modules are sometimes intertwined into classes, not a totally separate class. Like medical school, they are imposed into areas. One must remember that the NP has alrady had at least 2 - 4 yrs of education prior to NP, not just starting out. My patient assessment module was 16 weeks long and the text is over 1100 pages & yes we covered it.. so an additonal 16 weeks in NP school or even medical school, would be to fine tune or review. Again, please research more. Yes, there is psycho social modules, like in medical school, and P.A. school, etc . Dependent on the type of NP (yes, there are several types, like physicians they have specialties) from Psychiatric, Family, Neonate, Gerontology, Acute Care, Emergency, etc... the courses is all dependent on the specialty you choose. Advanced nurses, are more medical with a nursing perspective. That is why they have been documented to have a higher accuracy in medical dx., and fewer return visits regarding the same illnesses, as well as more holistic approach. Insurance companies have also noted this. If I was to only review course descriptions, medical school, paramedic school, even aeronautic engineering would appear to be a breeze. Although, I can recognize that course descriptions are just that. One liners about the course content NOT the curriculum, which is a totally different thing. I totally disagree with "curriculum" is all that matters.. that is the problem with EMS today. Uneducated, curriculum trained people. Even those people who went to EMT class for 2 weeks met the national curriculum standards. Every one meets the curriculum standards and we still test and have textbooks at a 6'th grade level. R/R 911

Do you work with an EMT or a driver?
Ridryder 911 replied to chazmedic's topic in General EMS Discussion
Okay here is my view : Yes, you have to pry and force most of them to work. The "me" generation has become so lazy and it is really our fault. So many are so "tuned out". that if they were to catch on fire, many would not even roll- over to extinguish themselves. But, whom is to blame, but ourselves. We allowed to it to continue. So, I have recently been on the band wagon to at least decrease it. Want to pass that test ?.. guess what, you have to study.. I know it is that damn educational thing! Want to pass that certification exam ?.. guess what see above ... Show up with more piercings and jewelry than a hardware store... go home! Unprofessional appearance. Come in with a apathetic attitude.. Guess what ? Me too !... Go home ! Don't come in prepared to learn, have not study during your entire course, and you want me to what? ... Let you treat the patient ?... go home Think you already know it all... guess you are right.. go home!.. Guess, you did not know about that one .. huh ? Now, some basic life rules as an EMT. 1) Yes, your role is important.. what ever that might be. 2) Your expectation might have been too high or too low.. but, that is your problem & no can correct it, but you ! 3) You have a job to do.. NO, really it is not supposed to be fun all the time.. thus it is called a job! 4) I don't care if you are the EVOC, tech BLS calls all day.. be the best. Can you improve daily ?.. YES !.. Study the map books, appear more professional, learn to assist your partner. Treat each patient that it is their emergency or problem.. really, it is. They were nice enough to invite us to it. 5) Sorry you are not a PARAMEDIC.. NO, not even close to being one.. That is like calling a nurses aide a RN.. Yes, again you have a role.. the patient is cared for by the team.. Yes, the medic ultimately responsible, but both of you will be sued.. so, I would be concerned on all aspects of the care. 6) NO, not all basic EMT's should become medics.. but, at least do that job with enthusiasm, and do it right and the best it can be performed. 7) Get some education... No, it does not have to be EMS. It could be a degree in cake decorating, but at least you would had English, mathematics, and general science. Yes, education (NOT TRAINING .. we are not monkeys or dogs) is important, that is how we communicate with each other. Looking at some of the post... we definitely need to get started ASAP ! 8) Take your profession serious, not yourself so much. Encourage and support formal education, better pay and professionalism. With this take off that silly baseball cap with EMT in 6" letters, and remove those 3 pairs of hemostats from our side pocket (which you won't be able to use) & before you hurt someone.. (mainly yourself). Oh, by the way stop blousing your trousers from your boots.. you are not a SWAT team or special forces, as you arrive in your wanker mobile with lights, car tags etc.. Yes, you look silly. If I think you do, what do you think the general public thinks ? No one is impressed except you...! Now, grow up !.. I don't care what age you are!.. Be responsible for yourself, and guess what .. for others !!!! That is the job, to care of others.. your partner, the patient, the company equipment, the units. etc... I know, maturity is a scary thing, but either get with it or go home ! R/R 911 -
What's Happening on this one
Ridryder 911 replied to Just Plain Ruff's topic in Education and Training
Usually with trachea rupture, after a few minutes of ventiallations, sub-glottic emphysema would start to occur. Diaphragmatic herniation, I would find to be rare in thsi situation .. could be, but rare. R/R 911 -

Get used to it... all females from 12 years old to 40+ gets asked this. Don't want to know how many "surprises" we see on x-ray. That is why a pregnancy test is usually always ordered before any x-ray are performed. I ask it a non-judgmental way... and guess what you will probably be asked by the nurse, the physician and even the x-ray tech, before x-rays are performed. Simple history.. you should have the information. Not doing so you are negligent in information about the patient. Even if they proclaim to be a homosexual.. I had a patient that I found out after I was told don't worry she is gay ... she had been artificially inseminated and was pregnant. So yes, I ask on all female patients.. don't care even if they were nuns.... R/R 911

Uhummm... apparently you know nothing about nursing. There are several hundred kinds of nursing (especially, specialty nursing), Second, only a semester of of medical model WTF ? Apparently, I must be going to the wrong school and the $50k student loan as well. Before making an ass out of yourself (which you just did), be sure to research your statements.. NP programs are the minimum a graduate level, which is 2-3 years after... your 4 year BSN. Some are even PhD or DNSc level, which is the same or greater length college a M.D. has. I don't believe in PHRN's, if your going to be a Paramedic be one... if your are a Paramedic and going to work in the hospital as a nurse, then go to nursing school. The curriculum, the methodology, the models, clinical emphasis are totally different. R/R 911

BLS report requirements on ALS calls
Ridryder 911 replied to Eyegor's topic in General EMS Discussion
As always stated " you didn't chart it, it was not performed".. I would use the analogy, if 5 years from now, if the ALS provider only documented that BLS was performed P.T.A., then any action the performed or not performed that could had potential ill effects, how could they defend themselves ? R/R 911 -
Well, it would be kinda of stupid for the school to offers a class, that is not recognized by the state... if you understood the point. Another words, I doubt you find an Intermediate class in a state that does not recognize such....(my main point) I would look at your state EMS agency and their approved training sites, and look at their links..for schedules etc.. If you don't have time for that, doubt you have time for classes... R/r 911

I actually like it, although I come form the era when you used to see it, it really meant something. Now, I when I see it, it has appeared to lost its glitter & shine ...not because of the patch, but because of the lack of knowledge of the person wearing it. It used to have meaning to hear "they are a blue & gold".. now it is a joke. Although, everybody thinks their "own state" patch is unique and pretty, I have yet seen but a very little few that looked more classy than a scout-o-rama. State outlines, on a patch looks like a road crew or trooper patch, looks they have to know what state they live in. Large patches with scenery .. hey, I don't care what your terrain looks like. Most EMS (hospital based) usually has nice ones.. clean not much wording and straight to the point. R/R 911

Az is right. Yes, you could probably do all right because of your experience, but your education was not really not designed for prehospital care. The philosophy and intent of nursing education is a lot different than the Paramedic level, especially on autonomy and delivery of care. I suggest that you might check with local EMS education facilities to see if they would allow you to further your education to finish your Paramedic. In lieu of hospital clinicals, to get more field time. I do this with RN's, to increase their field experience. Most actually, describe learning a lot more cardiac, and it really is a different approach. I would also be concerned to as well for liability & licensure coverage. Most states EMS will not recognize the RN education level in the prehospital care level. One must have attended and passed a Paramedic course, along with this be sure to investigate your local scope of nursing practice making sure you are covered. As a former nurse educator, I know that I had to ask for "special exemptions and additional educational coverage" in some states to allow the nurse to perform at advanced skills, such as intubations, etc.. the portions not in in the NLNAC (national nursing curriculum) guidelines. In short be sure your arse is covered.. no matter whom, whatever suggested you. I am sure your are very competent but, be safe ..... also check with your malpractice insurance as well..most do not cover "EMS", I had to purchase an additional coverage. Good luck in your decision, we need as many competent & educated people we can get. R/R 911 Good luck, R/R 911

I have no problem asking them anything. From health care insurance to what they ate to how it came out. As a health care professional it is my job to inquire and to ask. Without asking you don't get answers. So every female under 40 gets the same routine questions: Last menstrual period (LMP) ? On birth control ? What type ? Last time you had intercourse ANY chance of being pregnant ? etc... I don't know how many times after asking any chance of being pregnant they would say: " oh NO way I could be, I have not had sex! " only to get a + HcG pregnancy test ...hmmm another immaculate conception! Every medical call gets asked last bowel movement ? and was it normal and urination okay (changes?)? So I guess, asking what their insurance company name is pretty lame by then ... R/R 911

I am not sure, but Mo. at one time did not recognize the Intermediate level, as others stated check with local EMS agencies. R/R 911

Been on both sides.. I do understand your concerns & yes they are valid. Have you really discussed with her .. I mean one on one not briefly converse. Tell her your true feelings and concerns. Now, saying this, I have to say you have to have trust. You cannot prevent anyone from being unfaithful... if they do become unfaithful, it is because other situations has made them to look else where. These are problems that should had been discussed in detail, & either worked out or compromised before she applied for the F.D. To start out with, being controlling, demanding, smothering or even over jealous will only increase her dissatisfaction of her own personal relationship. Supporting her in her decisions and listening to her when necessary, may increase the trust and closeness, she may want at home, and fulfill her needs. She needs a mate, friend, & lover, not a father figure. No you don't have to always agree with her, but is nice that you will be the one she turns to if there is trouble. It is nice that you care so much, let her know, but also respect her wishes... everyone wants to be validated. Respect her professional calling, even if you may not agree totally, you might be able to work out some compromises. Try develop some personal down time for her... most need a little escape. Then some personal time for the two of you.. to re-develop your friendship and bond. Yes, joking is a tension breaker... & yes, there are times it is very inappropriate. My suggestion to anyone is"would you do this or say in front of your mate ?" .. if not it is probably inappropriate. Equal is only fair, and she should be respectful, the same as you would be. I do suggest marital counseling ad soon as possible.. this does not mean tragedy, but they can give helpful communication techniques, to improve relationships and with the innocent 3'rd party.. both sides are heard. I wish you the best of luck, R/R 911
