  1. I could not find the old threads and my search skills are failing me. Anyways, I have a couple people interested in the online/distance medic courses out of Texas or other states if available or known. Keep in mind, these guys already have experience and are quite knowledgeable in EMS already as they are paramedics in their home country. They simply wish to take a US course to get exposure to American EMS and to enhance their job chances by holding American certs. Thanks for your help guys. AK...

  4. Good for them!!! I hope I am just as adventurous when I am that age, causing trouble wherever I go. Old does not mean dead and senior citizens still have desires despite the negative images we conjure up in our heads. I have had the pleasure/misfortune of walking in a room at a nursing home and interrupting a couple getting busy in the bed. The sheepish grin on the man's face as he quickly jumped up and the smile of contentment on the woman's face as she pulled the sheet up to her chin...it was classic! You would have thought they were a couple teenagers getting caught the way they acted. A great reminder that we go on living despite being cooped up all day in a facility.
  5. Start rooting through her fridge and cabinets cause if we are not transporting, we are chowing down. Then, I insist that we notify a friend,relative or neighbor that can check on her (if no one else lives with her) and then we discuss the importance of eating/diet control along with the timing of insulin injections (again assuming she has a hx of this). If this is her first time, no prior hx, then we get very persuasive and it is easy to do, especially with an elderly woman. Employ every trick in the book to convince her to take a ride with ya...
  6. In regards to your refusal issue, your contract/obligation/duty to act started the moment you went en route to their 911 call. They sent a distress signal, you responded. Your presence on scene is now "medical treatment". A quick assessment, is treatment. If you leave at this point without follow through, it is abandonment, unless an equal or higher medical authority has shown up and takes over or you transport. Plain and simple...ask any lawyer. Remember, it is THEIR emergency, NOT yours. Whether or not you think it is an emergency is only relevant when deciding on which hospital to take them to, whether or not to call for ALS or whether or not to travel with lights and sirens.
  7. Well said...it is ALWAYS the treating provider's decision on whether or not lights are used. Even if we roll up on a heavy traffic jam, the driver had better ask/request lights and sirens and have a good reason for wanting to do so. If I instruct no lights and sirens and you decide to trump me, I will shut you down right there on the spot. You will not risk me or my patient when there is no reason to do so. I will have supervisors, dispatch, whomever it takes to make you stop that vehicle and/or stand down. After the incident, I will ensure you will not drive for me again and possibly have you removed from the squad. Harsh? Yes it is but we are talking about my life and my patient's life regardless of whether the driver cares for their own well being or not. A non medical driver will not ever trump my decision making skills when it comes to how we should transport. Yet another reason, this should not be a hobby which functions under the guise of "helping others cause no one else will".
  8. I could be wrong but it seems PHTLS taught opening the airway as you were taking cspine. Seems like they think it is a good idea to maintain spinal precautions in a high impact case, especially such as this. The caveat is I have not taken PHTLS recently so I could be misinformed. Secondly, I have not had much issue with IO flow when placed properly... not saying you didn't, just merely noting the lack of troubles I experienced personally. I did not rule out an EJ entirely, however have you placed many of them? It would certainly be based on that particular presentation at that time, however EJs have a high failure rate for placement when proper positioning is not used. As I noted, best positioning is a lateral movement of the head, a severe compromise of the cspine protection. Would you want me to save your life so you could be a veggie?? I wouldn't... Now if I didn't know any better, I would say Mr.Dwayne has chose a topic to play devil's advocate cause he has been missing out on serious intelligent debates. However, I think he could have chosen a better topic... Just sayin.
  9. Well this is still easy question. You have to immobilize him, the collar is in the way and it is much easier to do an EJ by turning the head lateral. Since this is contraindicated in this particular MOI, I would do the IO.
  10. Are these failed attempts occurring while he is still entrapped? If you can not get an IV due to positioning what makes you think an EJ would be better or less risky? Regardless, you also seem to be implying that the anterior medial tibia is the only place to inset an IO; the humerus sounds more readily accessible.

  13. I may be dense but I do not see the harm in what they are doing... The flight program I was with in Alaska did something very similar. We would fly anyone regardless of ability to pay because we were the only option. However, we had a membership one could purchase which was $100/year or $1000 for a lifetime. If you were a member, you paid nothing if you needed medevac services, we would collect from your insurance only and never chase you down for a dime. This held true even if you did NOT have personal health insurance. So in the grand scheme, it seemed like a great investment. This sounds like a very similar program and I see no issue with it. To use the phrase "It is not Insurance...it is Assurance".

  18. Ok I looked at that and it made no sense to me. You have to talk to me as if I am a 5 year old when it comes to computers. Is there any combination of buttons I can press to make the computer reboot itself completely, you know, erase and start over?...There is nothing on it as I have never used it...I have no factory CDs with it or anything, just a case and a laptop.
  19. I have a brand new laptop I won back in January. When I got it, I loaded everything on it and I also password protected it. I have never used it aside from this first time of loading. I am now wishing to use it only I can not remember the password. Is there a way around this???

  22. Oh Squinters....what to say to you man. First, let me clarify the already resolved issue. 1. Truck module power on, main O2 off, regulator off = ALARM 2. Truck module power on, main O2 on, regulator off = ALARM 3. Truck module power on, main O2 on, regulator on = ALARM 4. Truck module power off, No O2, No Lights, no nothing Daily checks always include checking the pressure gauge attached to the regulator, so no issues there. Yes, I trust my crew to always turn it off as we are much more conscientious of these things due to our location and inability to rapidly replace or refill anything. Our failures will cause harm to others, so we can not allow that. What say you now Canuckster?
  23. No worries mate! Since I am the boss, there is no need to "accidentally" do anything. I got brave, removed the panel, traced the wires and actually found a plug adapter, so that is what I did. A simple unplugging as opposed to cutting...but I was prepared either way.
