I don't think Aaron jumped the gun, I thought it was funny and took the comment as it was intended...a joke. It had nothing to do with the names as only one of them was an Arabic sounding name, the other 2 were typical.

Transported a Patient with potential swine flu. Now what?
akflightmedic replied to ghurty's topic in General EMS Discussion
The poster is worried because in his original post he asked "should any precautions be taken around the EMTs" who did transport. He is worried he will get cooties from the ones that tx the pt with potential cooties. -
Transported a Patient with potential swine flu. Now what?
akflightmedic replied to ghurty's topic in General EMS Discussion
Where are you located and what made the doctor say "potential swine flu"? Was fever the only symptom? What made that potential dx versus the dozens of other potential dx's for that age and with a fever, for example bacterial meningitis? What more do you know? -

Conscious FBAO algorhythm (Aussies only PLEASE)
akflightmedic replied to akflightmedic's topic in General EMS Discussion
For those that are confused or not aware, the abdominal thrusts I refer to are what some may still refer to as Heimlich, which we stopped calling that several years ago. Abdominal thrusts are just that, thrusts to the abdomen usually from a standing position behind the victim who is still conscious and choking. -
Ok me Aussie mates...heres me problem. Had a delightful debate today regarding FBAO in the conscious victim. For this discussion, the airway is totally obstructed, no coughing, no breathing, nothing and the patient is conscious. We had members of several different nations taking part in this discussion and the one that stood out were the Aussies. I also have a few questions I need direct factual support for, not "I think I know" or "I suppose". I do have a copy of the Australian Resus Council guidelines and if you go by what is written, this is the procedures to be performed. 1. After safety check and confirming the victim is choking 2. Give 5 back blows...you will check in between each back blow to see if there is relief. 3. If this fails, you go to chest thrusts Pausing now, because it is the chest thrusts that is the crux of the situation. One said you lay the victim down and perform it like CPR compressions and the other mentioned a side chest thrust. While I know what is written and you have guidelines to follow, how in the world are you going to convince a panic stricken, hypoxic person to lay down so you can chest thrust??? I asked the Aussies if you could perform them from a behind position and do it like we would do it on a pregnant woman instead of abdominal thrusts. They supposed you could but stated they were instructed to lay the victim down. Another medic said what is written and what he would do is two different things. For example, he said if the back blows were ineffective, he admits the victim would not lay down and he would go to abdominal thrusts even though they are NOT recommended in Australia. He said he would document otherwise. Why would a system be in place where in order to do the right thing, one must falsify a document to best serve the patient? He said his hands are tied, because officially all he can do is back blows and knowing the victim wont lay down, he will have to wait until they get WORSE in order to do something more for them, such as chest thrusts or laryngoscope and magills. Can you please tell me why Australia no longer recognizes abdominal thrusts? Can you tell me when Australia did away with abdominal thrusts? Can you tell me why a medic (if following the guidelines as written) would have to wait for the patient to decompensate before being able to assist if the back blows don't work and they don't lay down? If you have any further questions, please ask away. Confused in a far away land....

To study or not to study
akflightmedic replied to Riblett's topic in NREMT - National Registry of EMT's
There are questions specific to Florida, so yes you should study. Example: DNR laws, state trauma alert guidelines to name a couple. -
I Fucking Hate You...Godsmack You have to tell a patient they are terminal.

Tips for memorizing 2 protocol sets
akflightmedic replied to jwraider's topic in Education and Training
Welcome Don, please note he said counties not countries and in America, they can vary greatly. There can be massive differences between treatments rendered simply by driving 20 minutes north. As far as country to country, those differences are significant as well based on my exposure to multiple nations in all my travels. -
EMS based refusal of transport
akflightmedic replied to Just Plain Ruff's topic in General EMS Discussion
EMTALA applies to medical facilities not transport agencies (at least this is my understanding). -
I need help with ambulance specs
akflightmedic replied to akflightmedic's topic in Equiqment and Apparatus
No longer needed. I have purchased the units and now await their arrival. -
Awesome! Finally a bump in the road which may start to limit the dominating NJ volunteer system and force towns to hire full time paid providers, thereby creating more jobs and ensuring guaranteed coverage 24/7 for the citizens and also moving EMS forward throughout the state. What was the problem again? I wish I could find organizations that would pay for my personal hobby and all it's associated costs year after year.

Brainstorming EMS Paper Topics
akflightmedic replied to AnthonyM83's topic in Education and Training
Ambulance design safety. Lights and sirens response to every call (probably done before and not unique). -
Yeh that list is bogus. I can debunk two of those states "wages" personally. I am thinking they have combined the total salary and by that I mean they added the cost of benefits to the wages to show you those hourly rates. This was similar to the propaganda when the IAFF was assisting my FL FD with taking over our EMS system. They put annual salary figures of 65-80K a year as our salary to the general public, but of course that was out total benefits not our actual pay which was 34-39K annually. In Alaska, I made 30/hour as a flight medic. There are fewer than 10 paid FDs in Alaska, and their annual posted salaries were in the 45-55K range and this was for fire medics. There was an interfacility ambo in Acnhorage and they paid less than 20/hour/ In Delaware, the medics hourly rate is 17-24/hour. I know this because I have interviewed there and considered going there at one point because I like the ALS fly cars they use.

The program is licensed by the state, the courses have numbers assigned to them. It is not an "online" course per se, it is a home study course. You download the material, then fill it out by hand, then mail it in for review and grading. They are sticklers for full and complete answers, no shortcuts allowed. In all honesty, I have gotten more out of these two refreshers than ones attended in person previously. I do not sit around listening to BS stories from other walks of life, I do not need to wait on idiots who have not kept up try to do so in that short week. I get my packet and complete it. Completion requires utilizing many different resources. I spent several hours online doing research as well as consulted several different books. I also had to look up certain state statutes. Having to do all the research myself, led me to read many new articles I had not previously seen, it forced me to review certain things I had forgotten, otherwise I would not have the correct answer. Overall it was a great review which is why I opted to utilize them again this year and do recommend them to others. It is inexpensive to sign up, you save gas, you save time. With the economy the way it is and everyone complaining about funds, here ya go.

I have posted this before and I myself have recerted my state and NR twice with this refresher. eeii.org It is a home study refresher. Download the content, fill it out completely, should take 3 days to a week depending on how much time you put into it, mail it in to be graded and then you are mailed a certificate of completion. As I said, the State of FL has accepted it twice as well as National Registry. With the abundance of online EMT and Paramedic programs, why do you think a refresher would be hard to find and unapproved?
