Paramedics Rescue Boy On Oxygen From Burning Home POSTED: 6:55 am EDT July 11, 2008 UPDATED: 7:31 am EDT July 11, 2008 FORT WASHINGTON, Md. -- Prince George's County paramedics rescued a disabled teenage boy who is dependent on breathing equipment from a burning home on Thursday. The fire department said it responded just after noon to a report of a fire at the two-story Fort Washington home. Family members were attempting to get the boy out of his basement bedroom. A paramedic crew was able to enter the basement and remove the boy from multiple monitors and breathing machines before carrying him to safety along with two other siblings. The boy was transported to a nearby hospital because of his medical condition. There were no fire- or smoke-related injuries. The fire caused about $50,000 in damage. Investigators concluded it was started by a child playing with a lighter. Copyright 2008 by The Associated Press. And follow up information.... The box alarm stated a report of one person trapped in the basement. On the fireground channel, supplemental information reported "now two people trapped in the basement". Medic 847 was the first unit to arrive and reported smoke showing, second floor of a two story private dwelling. The medics (no PPE provided) pulled a medical device dependant teenager and a sibling from the basement as well as assisted family members who were earlier trying to do the same. Press release, "Just after 12 Noon, Thursday, July 10, 2008, Prince George’s County Firefighters and Paramedics were alerted to a house fire in the Fort Washington area. Fire/EMS units responded to the 13200 Block of Park Lane and encountered a 2-story single family home with smoke coming from the roof and fire showing from the second floor. Callers to 911 frantically described to public safety dispatchers that there was a teen-aged male located in a basement bedroom that has limited mobility, is attached to several monitors and breathing equipment and is dependent on oxygen. Family members were attempting to remove him from medical equipment and to escape the burning house. Dispatchers relayed the information about the occupants to responding firefighters and paramedics. The paramedic unit from the Fort Washington Fire/EMS Station #847 was one of the first arriving units. The crew entered the basement and removed multiple medical monitors and breathing machines from the patient and carried him to safety along with two other siblings. The teen-aged male has an oxygen dependent medical condition so paramedics assisted with his breathing and transported him to an area hospital for continued treatment. There were no fire/smoke related injuries. There were a total of 30 firefighters and paramedics that took about 15 minutes to extinguish the fire. The fire caused an estimated $50,000 in fire loss and the fire was determined to have been started by child playing with a lighter. The family of 2 adults and 4 children will be temporarily displaced and are being assisted by the Fire/EMS Department Citizen Services Unit."

I will share this story as it is an important teaching tool. In my career, I have always been quick to recognize the fakers due to the obvious signs that we all are aware of. However, this one time, I had a patient demonstrating all the faking signs, eyelids flickering, arm drop, the guy twisted his head with the NPA insertion, yet after another attempt, he then allowed it after I verbally accused him of faking. His eyelids were fluttering, he would move his fingers and arms with purposeful movement, yet "pretend" to be unresponsive. Also, not that it should make a difference in my clinical judgement, I did allow it to influence me. He was from a very depressed socio economic area, in an apartment complex known for its very low subsidized rent along with all the other typical problems that are present in that type of area. So, I placed him on the stretcher and moved him to the truck. I did typical ALS stuff, IV, O2, Monitor and transport. I did not waste a lot of "my time" giving this guy the assessment he deserved as I was 100% positive he was faking. I handed him off to the ER and relayed my thoughts on this patient. Due to my reputation, the receiving nurse allowed me to tunnel vision her down the same line of thinking. She quickly assumed and supported my opinion. When I spoke to the doctor and gave my report to him as well, he also said good job and I will get to the guy in a few. I left and when I returned to the ER about 2 hours later, as I wheeled my current pt by the bed of this previous patient, imagine my surprise to see the guy on a ventilator!! After I handed over care I went to inquire about the patient and learned that he had a massive intracranial bleed. I felt like the biggest ass on the planet. Moral of the story: Regardless of you presuming them to be faking or not, give a thorough assessment to the best of your abillity, cause you just never know... -
Shane it is not often that I request someone stop posting, so I wont; however, may I suggest a few things? I would strongly encourage you to practice BLS scenarios only for a while. Please leave the ALS to ALS providers. Start simple, pay attention to the responses and respond accordingly to what the people have posted. Definitely stick to BLS for a while, otherwise these "scenarios" of yours will continue to derail way off topic due to your failure to post a comprehensible case and inability to respond appropriately, the threads will also be locked with these derailments. I also wish to suggest a medical terminology textbook. I am disappointed in your spelling ability at the "paramedic" level. You use extremely simplistic terms to describe medical conditions that just should not be utilized at the medic level, or even the BLS level for that matter. Feel free to PM me with any complaints, comments or suggestions....AK

paramedic wages in volusia county florida

Volusia is one of the few remaining counties where you can still work as a 911 medic without being a firefighter. This is your only option http://www.evacamb.org/ for 911 calls in Volusia County. There may be some private outfits, but I don't know who. The pay is low as well, especially due to them being single role only. There are several Floridians here on this site, so feel free to ask questions. My question to you is Why Volusia County? Good Luck. -

Exactly the point I was making....who says they are illegal. How do we know that we are not occupying "their" planet....hmmmm? Food for thought: North is an arbitray direction assigned to a point on a compass...ask the people in the southern hemispehere why are they always on bottom. -

Firedoc, I was NREMTP at age 19. However, as far as this scenario goes, it is a great example of what not to do. There are way too many questions unanswered and details that seem ludicrous along with the mispellings of common medic practices or treatments. Anyways, this thread will be closed for now.

When the ambulance pulled into the hospital, the crew could barely keep the woman on the stretcher, but the doctor on duty didn't understand why no one took a medical history or started an IV line en route. “It's a different world,” she said. “It's a completely different experience.” While rescue squads and emergency rooms perform similar life-saving procedures under extreme and difficult conditions, there often is little professional interaction, emergency medicine experts say. Bucks County Rescue Squad in Bristol Township, where Chacko works, is one of the few suburban ambulance squads helping prepare future ER doctors by showing them what happens before a patient gets to the hospital.
