  6. We did this experiment in class, and I found it quite interesting, and wanted to get some more opinions from people out of the area. Your job is you chose to play is to create a society. WW3 has started and the nuclear weapons went off, you and 9 others (total of 10) are in a bunker with no knowledge of whats going on outside the bunker. You may be the only 10 people left, however there is only enough food for 7 people, you must choose the 7 people to keep and the 3 people to outcast/kill. The 10 people are: Priest 16 year old blind pregnant girl Farmer Police officer History professor Prostitute 70yr old Doctor Lawyer Army Captain Medical Student ------------------ So basically tell us which 7 you would keep and list a reason, and which 3 you would outcast/kill and why. Having already done this I'm going to just play devils advocate and ask you questions about your society and your choices.
  7. That's complete crap and is absolutely a violation of their civil rights IF there is no policy about wearing those shirts bandannas on any other day of the year. There is no reason for any such restriction on their clothing choices, to send them home or demand they change is ridiculous. I hope they follow up and fight this.

  10. In my medic class our standard answer was pad 30ml and tampon 15ml I have no proof of this anywhere nor do I know if it was just a answer someone pulled out of a hat but that was the right answer on the exam!

  13. Just thought maybe we might play a little trivia game, One person posts a question, then the person who answers it posts a new one, anything related to Emergency Medicine is fair game. Whether the question is at the most basic level or most advanced is fine. Simple stuff to start. What is the function of arrector pili muscle?
  14. Again speaking from only what I do day in and day out. If a dispatcher cancels me for whatever the reason then I am canceled. I do not question how or why I get canceled I just keep doing what I need to do. The intent of my statements is not to absolve anyone, nor take sides. Simply stating a point of view. I agree with most that this is a system error not that of a crew. Ive also always been told you can't be abandoning a patient until you have patient contact. There are a lot of variations of when patient contact occurs that point of view is up to you. I don't view patient contact as occurring when dispatched. Where do the rules of Pittsburgh stand? As with almost every occurrence that hits the news paper mistakes were made I don't think these crews should be hung out to dry.

If I attempt to make patient contact but for whatever reason I'm unable, and I ask the dispatcher for a call back and the dispatcher "cancels" me ( an 87 for us nyc people) I no longer have any responsibility for said patient/potential patient. I will admit I have not read the transcripts but reading here, and other threads it seems the crews were "canceled" by dispatchers. I agree there is no reason for them to not walk in the snow, however as soon as a dispatcher cancels me and returns me to service that is the end of that call for me. I think some people here on the board forget every city has its own way of doing things, had I needed to walk blocks to a patient because of the snow no problem easily enough done, now that I got the pt what am I going to do? Call Rabbit for a helo lift to the hospital? Now I'm stuck until someone clears the street well enough for it to be safe for all involved to get this patient the blocks back to my ambulance, or clears it enough so the ambulance can get to the patient. Every city has their own issues, here in nyc, although not conventional it can be managed. I would have called for an engine company, upon their arrival I would have directed them to flag down sanitation on the main road(s) [which is who clears our streets here] and have them clear it enough to get the ambulance through. Oh and we take RMA's through OLMC over the phone and or radio yes we are being the Dr's eyes and giving them our opinions, but ultimately still his decision. EMS crews would be needed to transport these patients even after they have been diagnosed just because someone is diagnosed with an MI over the phone wouldn't help them get any care from anyone.
  16. Franco has been an EMT less then a year ... He got that job through a friend, who will probably never be able to refer someone for employment again. Shame he was a bright guy ... not bright enough though!

  18. Yes its possible, no it does not fit all situations. If there is a lot of radio traffic as is common in NYC sometimes we start moving, and get the notification in when we can, or we (against advice of all supervisors) call directly to the receiving hospital via cell to give the notification. I have sat and waited for over 10 minutes to have clear opportunity to speak to dispatch because of radio traffic, obviously not for a notification but in general. Notificaions are of course a courtesy, and not required, was just giving a 'for instance' with something that may be ill advised but is common practice. I'm not a fan of cell phones in a personal manner. I do see the valuable uses while on the job, for the job. Telemetry/olmc over the radio is near impossible here, and EKG transmission over the radio also near impossible over the radio, both very easy via cell. Each ambulance is provided with a cell phone plus our personal cells kept with us at all times as we stage, no base to come back to, to whomever said they have no place on the ambulance, I don't see why they cant be used between calls when sitting around? but I guess you don't stage, you sit at a base so that would make more sense.
  19. On a side note I don't see where it mentioned using personal cell phones for personal calls .. it is talking about using these things for job functions while on the job. Calling the dispatcher to give a notification for example would be given by the driver as the person in the back is scrambling to take care of pt care and get everything done that is a 'must' in the 4 minute window between here and the hospital etc... Not justifying anything just trying to paint a picture of whats going on for you.

  21. CC-EMT-P is not really recognized in NYC but it is an available option mostly have tot ravel out of state to obtain it, but you can get a high paying interfacility transport job with it specifically with NY Presbyterian hospital.

The bottom line with this has to be if you have personally assured you will be able to finish your in hospital time, and your ambulance time there is no reason for them to charge you more money or delay you a year. I would be outraged at the lack of flexibility. Even so I have to agree with FireMedic65 here, my medic program, didn't stop for people with surgeries, broken bones, car accidents, lost family members or even pregnancy. It was the students responsibility to work it all out and make the dead lines. I am truly sorry for your loss, however, to be perfectly candid, our bereavement leave where I work is 3 days from date on the death certificate whether or not they fall on work days. I can understand you have trouble focusing, but perhaps asking for 6 weeks off was a little much. Maybe as an alternate you can lighten your load in other areas of your life.. in case you are unable to get through the "pecking" order because that in and of itself will take a lot of time, which will not help your case, or help you have time to destresss and relax, I mean if your going to go through all of that might as well just go to the shift on the ambulance and be stressed!

  25. Doesn't matter if they work 4's 8's 12's ... . 96's. They treat the station as their house. They pool money together to go grocery shopping to get food to prepare meals for the entire house. The fact they bring the "rig" with them should be leaving passerby's feeling secured and reminded of their presence in the area. Yea they spent some "city" money on gas etc.. but that type of miscellaneous use should be budgeted for. Right but even being retained to the property they do get a break where they are not working right ? Fire nor EMS here get a break where they can not receive an assignment. Every job has advantages/disadvantages. ***Side note: Personal Pet peeve, we post on a street corner sometimes these corners are all residential, nothing more annoying then someone coming out of their home to tell us to shut the engine off when its 100degrees out or 20 degrees out. Why don't people appreciate the fact if they need an ambulance they have one within seconds!
